[Restrictive cardiomyopathy in childhood].

Restrictive cardiomyopathy is a rare heart muscle disease in childhood. By presenting a case report of a 15 year old adolescent, the aim of this article is to describe diagnosis, pathogenesis and therapy of restrictive cardiomyopathy. A review of the literature comprising pediatric studies on restrictive cardiomyopathy serves as a basis to discuss recommendations for therapeutic strategies in pediatric patients with this rare disease.

The pathology of hypertrophic cardiomyopathy.

Sudden cardiac death (SCD) is devastating at any age, but even more so when the individual affected is young and asymptomatic, and the death is entirely unexpected. SCD is a catastrophic complication of hypertrophic cardiomyopathy (HCM) and may be the first manifestation of this disease. HCM is an inherited intrinsic disease of the myocardium characterized by left ventricular hypertrophy without chamber dilatation, in the absence of either a systemic or other cardiac disease, which may cause a similar magnitude of hypertrophy. HCM may be a clinically silent disease. Indeed, the pathologist may

[Arrhythmogenic dilated cardiomyopathy].

Sustained or chronic tachyarrhythmia can produce reversible changes of atria or/and ventricles that induce systolic or/and diastolic disfunction, dilatation of heart chambers and progressive symptoms of heart failure. The changes regress after restoration of sinus rhythm. This is the clinical case, when at the beginning of the treatment, primary dilated cardiomyopathy is diagnosed to the patient having chronic atrial fibrillation and later after restoration of sinus rhythm and subsequent reduction of heart chambers as well as improvement of systolic function, past changes was assessed as rever

[Genetics of dilated cardiomyopathy].

Dilated cardiomyopathy is one of the leading causes of heart failure and a primary cause for heart transplantation in patients below the age of 40 years. Despite major advances in diagnostic procedures such as examination of myocardial biopsies, the etiology remains unknown in many patients. Chronic inflammation or myocarditis and chronic alcohol abuse are considered two main etiologic factors in dilated cardiomyopathy. A third causal factor, namely genetic transmission of the disease, is at least as common as myocardial inflammation or toxic damage. Several prospective studies of relatives of

[Tachycardia-induced cardiomyopathy].

Dilated cardiomyopathy induces circulatory insufficiency with poor prognosis. Persistent tachyarrhythmias may be the cause of this disease. At particular high risk for heart damage and insufficiency are young people and children. The group of most dangerous arrhythmias consist of: incessant tachycardia in patients with preexcitation syndrome and supraventricular tachyarrhythmias (atrial flutter and fibrillation, ectopic atrial tachycardia) with high rate of ventricles. The result of arrhythmias is dilatation of the heart and thinness of ventricular walls with hemodynamic disorders. The effecti

[Cardiac pacing in cardiomyopathy].

Pacing is useful not only for therapies of arrhythmias but also for other cardiac disorders. In this article, the pacing therapies for idiopathic cardiomyopathies were reviewed. AV sequential pacing has been reported as beneficial methods to improve symptoms of hypertrophic obstructive cardiomyopathy(HOCM) patients, but the mechanisms and expectancy of efficacy of pacemaker implantation are not clarified. Prospective double blind studies to see the efficacy of pacing on HOCM patients were done and reported in the two continents, one was PIC study in Europe and the other was M-PATHY in the nort…

Anagrelide-induced cardiomyopathy.

Anagrelide, a selective thrombocytopenic agent, is administered to treat a variety of hematologic disorders. Despite limited clinical experience with this drug, serious cardiovascular events, including congestive heart failure, have been reported. The proposed mechanism of cardiotoxicity is attributed to inhibition of phosphodiesterase, resulting in positive inotropic activity and vasodilation. A 48-year-old woman with polycythemia vera developed cardiotoxicity manifested by congestive heart failure and palpitations. It was suspected to be temporally related to titrating dosages of anagrelide.

Nemaline myopathy and cardiomyopathy.

A case report is presented in which a 4-year-old male is diagnosed with hypertrophic cardiomyopathy, respiratory distress, muscle hypotonia, and psychomotor retardation. Electron microscopic study of skeletal muscle biopsy revealed pathologic changes typical of congenital nemaline myopathy, and biochemical analysis revealed a disorder of mitochondrial fatty acid oxidation. Therefore a previously undescribed combination of a structural and metabolic myopathy is reported.

Hypertrophic cardiomyopathy and pregnancy.

The case of a young primiparous woman with defibrillator-assisted familial hypertrophic cardiomyopathy (HCM) has led us to review the literature on this pathology, which is exceptional because of its scarcity and the originality of the problems encountered. To our knowledge, this is the first observation ever reported of defibrillator-assisted activation during pregnancy in a woman with HCM. Several questions raised from this particular case, e.g. what are the risks caused by pregnancy in these patients, what is the impact of therapeutics, does the activation of an internal defibrillator invol

[Current knowledge of cardiomyopathy].

In 1961, 1964 and 1970, Goodwin proposed a somewhat unwield definition of cardiomyopathies, which was subsequently shortened to "Heart muscle disease of unknown cause" and separated from heart muscle diseases due to conditions involving organs other than the heart: the "specific heart muscle diseases". The cardiomyopathies thus defined were divided into hypertrophic, dilated and restrictive types and their classification was confirmed by the WHO/ISFC Task Force and the report of the WHO Expert Committee on cardiomyopathies in 1984. The cardiomyopathies are now classified by the dominant pathop…

MR imaging of cardiomyopathy.

Spin echo and cine MR imaging provides reliable and reproducible quantitative information about the hearts of patients with cardiomyopathy. In particular, these techniques are useful in demonstrating the severity and distribution of left and right ventricular myocardial thickness, changes in myocardial thickness during ventricular contraction, ventricular chamber volume, and change in volume through the cardiac cycle. Furthermore, direct examination of the ventricular myocardium by spin echo MR imaging is beginning to demonstrate its utility in characterizing abnormal myocardium and in providi

Prognosis of hypertrophic cardiomyopathy.

BACKGROUND: The actual prognosis of hypertrophic cardiomyopathy, a disorder previously thought of to be notorious for an increased risk of untimely death is poorly defined. OBJECTIVE: The present study describes the findings of patients with Hypertrophic Cardiomyopathy followed for more than nineteen years in a large clinic population. MATERIALS AND METHODS: A clinic population of 113 patients with Hypertrophic Cardiomyopathy was prospectively studied to assess cardiac mortality in the overall groups and in selected subgroups commonly thought to be at high risk for sudden death. Diagnosis of

Hypertrophic cardiomyopathy: an update.

Hypertrophic cardiomyopathy (HCM) is defined as an idiopathic heart muscle disorder which characterised by the presence of left and/or right ventricular hypertrophy in the absence of a systemic or cardiac cause. At post mortem examination the characteristic histology shows myocyte disarray surrounding areas of increased loose connective tissue. The spectrum of disease is wider than the phenotype which relies on the diagnostic presence of unexplained left ventricular hypertrophy. During the past 5 years, one of the genes which causes HCM has been discovered, several additional loci have been id

Hypertrophic cardiomyopathy in a dog.

A 12-year-old spayed Corgi with heart failure did not respond to conventional medical therapy for congestive cardiomyopathy (furosemide, 2.5 mg/kg of body weight, IV, q 8 h; captopril 1 mg/kg, PO, q 8 h). Cardiac ultrasonography subsequently revealed a hypertrophic left ventricle and interventricular septum. Because a causative agent could not be identified, a diagnosis of primary hypertrophic cardiomyopathy was made. Administration of diltiazem (1.25 mg/kg, PO, q 12 h) resulted in sustained clinical improvement for several weeks. Complete cardiac evaluation, including echocardiography, was es

Scorpion venom cardiomyopathy.

Cardiac function was evaluated by serial echocardiography in 30 children affected by scorpion stings. They could be separated into two groups on the basis of the initial echocardiogram. Group 1, consisting of 18 children, had normal left ventricular function, whereas group 2, consisting of 12 children, showed compromised left ventricular function (ejection fraction less than 0.55; fractional shortening less than 27%). In group 2 the left ventricular end-systolic dimension was increased significantly and the interventricular septal thickening fraction was depressed significantly, compared with
