Intrathecal synthesis of antibodies to human T lymphotropic virus type I and the presence of IgG oligoclonal bands in the cerebrospinal fluid of patients with endemic tropical spastic paraparesis.

Tropical spastic paraparesis (TSP), a neuromyelopathy predominantly involving the pyramidal tract and commonly observed in tropical and equatorial areas, was recently found to be associated with human T lymphotropic virus type I (HTLV-I). We investigated sera and cerebrospinal fluid (CSF) from 19 patients with TSP who were from the Caribbean area, French Guiana, and Africa. Our results showed an elevated intra-blood-brain barrier IgG synthesis rate and an elevated IgG index, with an increased HTLV-I antibody-to-albumin ratio and the presence of CSF oligoclonal bands in the majority of the pati

The burden of liver cirrhosis and underlying etiologies: results from the global burden of disease study 2017.

BACKGROUND: To evaluate the pattern and prevalence trends of liver cirrhosis caused by specific etiologies. RESULTS: Globally, the number of prevalent cases increased 74.53% from 1990 to 2017. The ASR increased 0.75 per year. The most pronounced increases were found in middle-high and high socio-demographic index (SDI) regions, especially in the Caribbean and Latin America. Among the etiologies, non-alcoholic steatohepatitis (NASH) related liver cirrhosis accounted for 59.46% of the cases. The ASR increased 1.74 per year, and the increase was observed in all 5 SDI regions. In addition, the AS

Tackling Infectious Diseases in the Caribbean and South America: Epidemiological Insights, Antibiotic Resistance, Associated Infectious Diseases in Immunological Disorders, Global Infection Response, and Experimental Anti-Idiotypic Vaccine Candidates Against Microorganisms of Public Health Importance.

This paper explores various aspects of microbiology and immunology, with a particular focus on the epidemiology and molecular characterisation of infectious diseases in the Caribbean and South America. Key areas of investigation include tuberculosis (TB), experimental vaccines, and bloodborne pathogens. A retrospective study conducted in Jamaica highlights the significance of early HIV screening, timely diagnosis, and inte-grated care. The paper also examines the challenges posed by nosocomial infections, particularly those caused by antibiotic-resistant Gram-negative bacteria and methicillin-

First case of yellow fever vaccine-associated viscerotropic disease (YEL-AVD) in Hong Kong.

Yellow fever is an important and potentially fatal infection in tropical regions of Africa, South America, eastern Panama in Central America and Trinidad in the Caribbean. Yellow fever vaccination is not only crucial to reduce the disease risk and mortality in individuals travelling to these areas, but also an important public health measure to prevent the spread of the disease. Despite generally considered as a safe vaccine, yellow fever vaccine can rarely be associated with severe adverse reactions including yellow fever vaccine-associated viscerotropic disease (YEL-AVD). Here, we report the

Global and regional burden, trends, and future projections of laryngeal cancer (1990-2041).

Laryngeal cancer remains a significant global health challenge, with notable disparities across regions, socioeconomic strata, age groups, and genders. Using Global Burden of Disease data (1990-2021), we evaluated prevalence, incidence, mortality, and disability-adjusted life years across 204 countries and 21 regions stratified by the Socio-demographic Index. Through estimated annual percentage change analyses and the autoregressive integrated moving average modeling, we projected trends to 2041. Globally, age-standardized prevalence and incidence rates declined, with sharper mortality and dis

[Investigation of anti-HTLV I/II seroprevalence in healthy blood donors in Izmir region, Turkey].

Almost 10-20 million people in the world are thought to be infected by human deltaretroviruses, namely human T-cell lymphotropic virus (HTLV) type I and II, recently. HTLV-I is endemic in southwestern Japan, the Caribbean and sub-Saharan Africa, whereas HTLV-II is more prevalent in intravenous drug addicts, and in American indian populations, endemically. HTLV-I is mainly responsible for adult T-cell leukemia (ATL) and HTLV-I-associated myelopathy/tropical spastic paraparesis (HAM/TSP), however, HTLVII is not clearly associated with a known clinical disease. Both viruses may be transmitted by

A nationwide joint spatial modelling of simultaneous epidemics of dengue, chikungunya, and Zika in Colombia.

BACKGROUND: Chikungunya, and Zika emerged in the 2010s in the Americas, causing simultaneous epidemics with dengue. However, little is known of these Aedes-borne diseases (ABDs) joint patterns and contributors at the population-level. METHODS: We applied a novel Poisson-multinomial spatial model to the registered cases of dengue (n = 291,820), chikungunya (n = 75,913), and Zika (n = 72,031) by municipality in Colombia, 2014-2016. This model estimates the relative risk of total ABDs cases and associated factors, and, simultaneously, the odds of presence and contributors of each disease using d

Parental income and drug use disorder among second-generation individuals in Sweden: effect modification by immigrant background and region of origin.

BACKGROUND: Evidence is limited for the association between parental income during childhood and adolescence and the subsequent risk of drug use disorder (DUD) in the second generation and whether the association is modified by region of origin. Identifying the association and effect modification by immigrant background and region of origin would provide an important knowledge base to consider tailored policies. METHODS: We used nationwide longitudinal Swedish data on 1,137,721 non-immigrants and 293,636 s-generation individuals who turned 15 years of age between 2005 and 2020. Cox proportion

Exposure of protected areas in Central America to extreme weather events.

Central America and the Caribbean are regularly battered by megadroughts, heavy rainfall, heat waves, and tropical cyclones. Although 21st-century climate change is expected to increase the frequency, intensity, and duration of these extreme weather events (EWEs), their incidence in regional protected areas (PAs) remains poorly explored. We examined historical and projected EWEs across the region based on 32 metrics that describe distinct dimensions (i.e., intensity, duration, and frequency) of heat waves, cyclones, droughts, and rainfall and compared trends in PAs with trends in unprotected l

Environmental exposures, microbiome dynamics and chronic disease risk in climate-vulnerable regions: interdisciplinary perspectives from Puerto Rico.

Environmental factors such as air pollution, weather events, and ambient toxins are major contributors to human disease, with disproportionate impacts on vulnerable populations. In Puerto Rico, chronic exposure to air pollution and ecological disruption poses significant public health risks, particularly for cancer and other chronic conditions. These risks are unevenly distributed, disproportionately affecting children and older adults, groups central to community resilience yet highly susceptible to pollution-related health effects. This perspective review synthesizes emerging evidence linkin

Histoplasmosis in Immunocompromised and Immunocompetent Patients in Guadeloupe.

Background:Histoplasma capsulatum is an environmentally acquired dimorphic fungus. Infection results in histoplasmosis, a clinical syndrome often underdiagnosed and that may progress to life-threatening disseminated infection not only in immunocompromised individuals but also, following high-level exposure, in immunocompetent hosts. Epidemiological data from Caribbean regions, and particularly from Guadeloupe, remain limited. Methods: We performed a retrospective cohort study of all microbiologically confirmed histoplasmosis cases managed at the University Hospital of Guadeloupe between Januar…

Clinical, paraclinical, and genetic profile of patients with cystic fibrosis from Colombian Caribbean.

BACKGROUND: Cystic fibrosis (CF) is a serious autosomal recessive disorder. Early diagnosis, comorbidity prevention, and control are cornerstones for a quality life and for improving life expectancy. In Colombian Caribbean, where there is a genetically admixed population, CF is an orphan disease affecting children and adults, and it remains a challenging issue to be addressed carefully. This work describes the genetic, clinical, and paraclinical profiles of CF patients from Cartagena de Indias, Colombia. METHODS: Thirty-six patients were included in the study. The subjects were identified and

Gone with the wind: An unexpected Sargassum inundation in the mid-Atlantic Azores archipelago.

Large masses of pelagic Sargassum occur in the Atlantic Ocean between the latitudes 5°S and 38°N. Since 2011, inundations have happened in the Gulf of Mexico, the Caribbean, and West Africa, affecting biological communities and economies. A series of severe weather events in the Azores led to a Sargassum inundation between mid-December 2023 and early April 2024, here reported for the first time. Although the sea reclaimed most of the stranded algae, 555 metric tons were removed. Periodic inundations may represent an introduction pathway for non-native species since massive amounts of organisms

The epidemiology of dengue in the americas over the last three decades: a worrisome reality.

We have reported the epidemic patterns of dengue disease in the Region of the Americas from 1980 through 2007. Dengue cases reported to the Pan American Health Organization were analyzed from three periods: 1980-1989 (80s), 1990-1999 (90s), and 2000-2007 (2000-7). Age distribution data were examined from Brazil, Venezuela, Honduras, and Mexico. Cases increased over time: 1,033,417 (80s) to 2,725,405 (90s) to 4,759,007 (2000-7). The highest concentrations were reported in the Hispanic Caribbean (39.1%) in the 80s shifting to the Southern Cone in the 90s (55%) and 2000-7 (62.9%). From 1980 throu

Differential diagnosis of HTLV-I and HTLV-II infections by restriction enzyme analysis of 'nested' PCR products.

A 'nested' polymerase chain reaction (PCR) assay is described which is capable of detecting single copies of human T-cell lymphotropic virus (HTLV) in genomic DNA extracted from peripheral blood mononuclear cells (PBMCs). A single set of 'nested' oligonucleotide primers, based on the highly conserved tax/rex region of the viral genome, was able to detect both HTLV-I and HTLV-II proviral sequences in clinical samples of diverse geographical origins, from the United States, Great Britain, Japan, the Caribbean, Italy, Greece, Iraq and West Africa. Rapid discrimination between HTLV-I and HTLV-II i
