Placenta Percreta Grade 3D - Lifeline to Morbid Catastrophe: An Invincible Challenge to Contemporary Obstetrics.

The global incidence of placenta accreta spectrum is increasing rapidly, mirroring the trend of increased cesarean delivery. We report a case series of four patients diagnosed with placenta percreta who presented to a single unit of the department of obstetrics and gynecology at a tertiary care center in North India from March 2024 to May 2024 with a pathologically proven diagnosis. In all cases, a hysterectomy was the outcome. We have included cases with prenatal diagnoses in this case series. For appropriate planning, a prenatal diagnosis of placenta percreta is necessary, as the multidiscip

When injury costs too much: the catastrophic costs for spinal cord injury in Iran.

INTRODUCTION: The main objective of this study was to determine the percentage of families experiencing catastrophic costs as a result of spinal cord injury. Patients with spinal cord injuries often encounter significant expenses related to treatment, rehabilitation, and caregiving. Catastrophic expenditures are the direct consequence of the lack of effective health insurance. This financial burden, along with the potential for catastrophic costs, presents a serious challenge for these individuals. METHODS: Data were obtained from the International Spinal Cord Injury Survey in Iran (InSCI-IR),

Catastrophic Outcomes: Rapid Multi-Organ Failure from Paraquat Poisoning- A Case Report.

Paraquat dichloride, a potent herbicide widely used in agriculture, poses a severe health risk due to its high toxicity. Ingesting even small amounts can cause fatal multi-organ failure. We report a case of a 19-year-old male who ingested approximately 5 ml of 24% paraquat dichloride. He presented with stable vital signs, but initial laboratory results showed elevated urea (26mg/dL), creatinine (0.97 mg/dL), and hypokalemia (3.2 mEq/L). Despite interventions including gastric lavage, N-Acetyl cysteine, methylprednisolone, and supportive care, he developed severe metabolic acidosis (HCO3- 22.8

Insights on Health Burden, Needs, and Prevention Strategies After the Flood Catastrophe in Southern Brazil.

Brazil has experienced a significant increase in climate emergencies and environmental disasters. Rio Grande do Sul (RS), the southern state of the country, shows an annual rise in climate, hydrological, and meteorological disasters, underscoring its vulnerability to extreme events and the urgent need for proactive mitigation and adaptation strategies. Despite this growing concern, the literature has provided limited focus on the specific public health demands arising from these disasters, particularly regarding prevention. This debate article highlights the short-, medium-, and long-term heal

Unravelling the fatal chain: A detailed investigation into catastrophic events following anaesthesia in a feline patient.

An 8-year-old male neutered British Shorthair cat, weighing 5.12 kg, was referred for chronic mild inappetence, lethargy, bilirubinuria, weight loss and icterus. Diagnostic imaging and serum biochemistry suggested a partial biliary tract obstruction caused by an enlarged pancreas. An exploratory laparotomy was scheduled to perform biopsies of the liver, pancreas and duodenum. Preanaesthetic medication included fentanyl (10 μg kg-1) and midazolam (0.3 mg kg-1), given intravenously (IV) through a catheter in a cephalic vein. General anaesthesia was induced with IV propofol (3 mg kg-1). After end

Terrorism Catastrophizing and Sociodemographic Correlates Among Croatian Nursing Students: A Cross-Sectional Study.

Background/Objectives: Fear of terrorism can impact psychological functioning and behavior even without direct exposure. Little is known about how anticipatory terrorism fears manifest among nursing students in European contexts. This study assessed terrorism catastrophizing among Croatian nursing students and examined sociodemographic predictors. Methods: A cross-sectional correlational study was conducted between October and December 2024 among 348 nursing students, using the validated Terrorism Catastrophizing Scale (TCS). Behavioral and habitual changes related to the terrorism threat were

Out of pocket and catastrophic health expenditure in Tanzania: recent evidence on the incidence, intensity and distribution.

BACKGROUND: In most low-income countries (LICs), health is mainly financed by out-of-pocket (OOP) expenditures. However, it is claimed that this form of payment causes a massive burden on poor households. This study investigates the catastrophic impact of out-of-pocket health expenditure by estimating the levels, intensities and distribution of catastrophic health expenditure among households in Tanzania. METHODS: The study applied the Wagstaff & va-Doorslaer methodology to measure the incidence and intensity of catastrophic expenditure and the concentration index to measure the distribution

Clinical Outcomes and Treatment Strategies in Catastrophic High-Risk Pulmonary Embolism: A Retrospective Analysis.

High-risk pulmonary embolism (PE) is a life-threatening condition characterized by hemodynamic instability, often leading to catastrophic outcomes such as cardiac arrest and cardiogenic shock. We conducted a retrospective analysis of patients diagnosed with high-risk PE at a single tertiary center between 2018 and 2024. Catastrophic PE was defined as high-risk PE with hemodynamic collapse, including cardiac arrest and/or the requirement for high-dose vasopressors. Data on clinical characteristics, treatments, and outcomes were analyzed. Catastrophic PE accounted for 59% of cases. Systemic thro

Relationship of pain relief with catastrophizing following interventional pain procedures for low back pain.

INTRODUCTION: Catastrophizing is associated with worse pain outcomes after various procedures suggesting its utility in predicting response. However, the stability of pain catastrophizing as a static predictor has been challenged. We assess, among patients undergoing steroid injections for chronic low back pain (cLBP), whether catastrophizing changes with the clinical response to pain interventions. METHODS: This prospective study enrolled patients undergoing fluoroscopic-guided injections for cLBP. Patients filled out Brief Pain Inventory (BPI) and Pain Catastrophizing Scale (PCS) at baselin

Catastrophizing as a Predictor for Pain Perception and Disability Among Patients Undergoing Spinal Cord Stimulation.

Background and Objectives: The International Society for Modulation defines persistent spinal pain syndrome type 2 (PSPS-type 2), formerly known as failed back surgery syndrome, as a condition where patients continue to experience pain or develop new pain following spinal surgery intended to alleviate back or lower-limb discomfort. PSPS-type 2 is characterized by pain and significant disability, affecting quality of life. Spinal cord stimulation has proven effective in treating this syndrome, although the role of psychological factors, such as pain catastrophizing and central sensitization, re

A prospective study of the association between pain and catastrophizing after selective nerve root blockade.

INTRODUCTION: Pain, comprising sensory and emotional elements, is influenced by pain catastrophizing, which magnifies pain and promotes helplessness and rumination. This study explores the relationship between pain catastrophizing and outcomes following selective nerve root blockade (SNRB) in patients with lumbar radicular pain (LRP). METHODS: A prospective cohort study of 103 LRP patients, confirmed by MRI, was conducted. All participants underwent SNRB at Umeå University Hospital. Outcomes were measured using PROMIS-29 and the Pain Catastrophizing Scale (PCS) at baseline and several interva

Catastrophic wound dehiscence in early onset scoliosis secondary to hyperinflammatory response: a case report.

Early onset scoliosis (EOS) poses significant treatment challenges, often exacerbated by postoperative wound complications. A novel case of hyperinflammatory wound complications in a child with a confirmed NFKB1 mutation is presented, successfully managed with immunomodulation. A 6-year-old boy experienced wound dehiscence and persistent inflammation following rod placement, unresponsive to conventional treatments including surgical debridement and antibiotics. Further evaluation identified an NFKB1 mutation associated with hyperinflammatory states. Targeted treatment with Anakinra, an interle

Leptospirosis Cases During the 2024 Catastrophic Flood in Rio Grande Do Sul, Brazil.

Leptospirosis is a well-known disease that frequently occurs after floods. At the beginning of May 2024, a catastrophic flood occurred in the state of Rio Grande do Sul, Brazil, affecting two million people and leading to a state of calamity. Given the State's high pre-flood leptospirosis rates, the disease became a major concern for decision-makers. This study aimed to estimate the rise in leptospirosis cases after the flood, assess the changes in case patterns before and after the disaster, and document the response of the state health surveillance center to the outbreak. We estimated the in

Dominant Occipital Sinus: A Rare Anatomical Variant With Potentially Catastrophic Consequences if Unrecognized Preoperatively.

The occipital sinus is often thought of as a redundant vestigial structure in adults. However, in rare cases, it can form the dominant route of intracerebral venous drainage, with a risk of significant surgical morbidity if unrecognised. We present an illustrative case describing this anatomical variant and tailoring of a midline suboccipital craniotomy to allow resection of a fourth ventricular epidermoid tumour with preservation of a dominant occipital sinus, and a review of the published literature. A 48-year-old female patient was diagnosed with a large fourth ventricular tumour with marke
