Emergence of co-resistance to imipenem/relebactam and ceftazidime/avibactam in clinical Klebsiella pneumoniae ST11 clone due to KPC-2 N132S and CTX-M-65 S130G/P167S substitutions.

Two episodes of imipenem/relebactam and ceftazidime/avibactam co-resistant Klebsiella pneumoniae infections were identified in a Chinese hospital with an 8-month interval. All strains belonged to the ST11-KL64 clone, with pairwise SNP distances ranging from 1 to 9, indicating clonal spread. Plasmid-borne blaKPC-157 (KPC-2 N132S substitution) and blaCTX-M-249 (CTX-M-65 S130G/P167S substitutions) conferred resistance to imipenem/relebactam and ceftazidime/avibactam, respectively. The emergence of these novel resistance determinants in the high-risk clone underscores the need for enhanced surveil…

Cure of recurring Klebsiella pneumoniae carbapenemase-producing Klebsiella pneumoniae septic shock episodes due to complicated soft tissue infection using a ceftazidime and avibactam-based regimen: a case report.

BACKGROUND: Infections caused by multidrug-resistant Enterobacteriaceae are hard to treat and life-threatening due to reduced therapeutic options. Systemic infections caused by Klebsiella pneumoniae carbapenemase-producing Klebsiella pneumoniae strains have increased in many European regions, becoming frequent in many clinical settings, and are associated with high mortality. The co-formulation of ceftazidime, a third-generation cephalosporin, with avibactam, a new suicide inhibitor beta-lactamase inhibitor able to block most Klebsiella pneumoniae carbapenemases, has been recently licensed, wi…

Deciphering variable resistance to novel carbapenem-based β-lactamase inhibitor combinations in a multi-clonal outbreak caused by Klebsiella pneumoniae carbapenemase (KPC)-producing Klebsiella pneumoniae resistant to ceftazidime/avibactam.

OBJECTIVES: Carbapenemase-producing Enterobacterales represent a major cause of difficult-to-treat infections world-wide. Novel β-lactam/β-lactamase inhibitor combinations, including ceftazidime/avibactam (CZA), meropenem/vaborbactam (MVB), and imipenem/relebactam (IMR), represented a break-through in the treatment of some carbapenemase-producing Enterobacterales infections. However, acquired resistance to these agents has been reported in Klebsiella pneumoniae carbapenemase (KPC)-producing Enterobacterales. Herein, we reported an outbreak caused by CZA-resistant, KPC-producing Klebsiella pneu…

A systematic literature review and (network) meta-analysis of the effectiveness of ceftolozane/tazobactam versus aminoglycosides/polymyxins and ceftazidime/avibactam for treating adult patients with multidrug-resistant Pseudomonas aeruginosa infections.

Multidrug-resistantant (MDR) Pseudomonas aeruginosa is a major public health concern necessitating new antimicrobials. There are new antimicrobials available with activity against MDR-P. aeruginosa, but there is a lack of robust evidence synthesis to guide clinical decision-making for patients with infections caused by MDR-P. aeruginosa. This study, which was supported by Merck & Co., Inc., aimed to evaluate the real-world effectiveness of ceftolozane/tazobactam (C/T) compared to other commonly used therapies (aminoglycosides/polymyxins and ceftazidime/avibactam [CZA]) among adults with MDR-P.…
