The clinical relevance of autoantibodies in scleroderma.

Scleroderma (systemic sclerosis) is associated with several autoantibodies, each of which is useful in the diagnosis of affected patients and in determining their prognosis. Anti-centromere antibodies (ACA) and anti-Scl-70 antibodies are very useful in distinguishing patients with systemic sclerosis (SSc) from healthy controls, from patients with other connective tissue disease, and from unaffected family members. Whereas ACA often predict a limited skin involvement and the absence of pulmonary involvement, the presence of anti-Scl-70 antibodies increases the risk for diffuse skin involvement …

[Reynolds syndrome: the combination of scleroderma and primary biliary cirrhosis. Case report].

A 57-year--woman with Reynolds syndrome (primary biliary cirrhosis and scleroderma) is reported. Diagnosis of primary biliary cirrhosis is based on clinical findings, laboratory tests results and histological result of liver biopsy. Scleroderma was confirmed by anticentromere antibodies presence, and typical skin lesions. Although, antimitochondrial antibodies are very typical for PBC (primary biliary cirrhosis), in this case the were not found.

Calcinosis and the anticentromere antibody: its clinical, radiological and immunogenetic aspects.

The relationship between calcinosis and the anticentromere antibody (ACA) was studied from a clinical, radiological and immunogenetic standpoint. Ten ACA-positive scleroderma patients, 34 ACA-negative scleroderma patients, 31 ACA-positive patients without sclerodermatous skin changes and 140 ACA-negative patients with various rheumatic diseases were compared with regard to the incidence of calcinosis as measured by radiographs of hand and/or foot. Calcinosis was found in 10 (100%), 12 (35%), 13 (42%) and eight (6%) patients in each group respectively. Frequent sites for calcinosis in the ACA-p…

Systemic sclerosis and digital gangrene without scleroderma.

We report a case of a 67-year-old man with digital gangrene and no other cutaneous evidence of scleroderma. The presence of anticentromere antibody, scleroderma pattern capillary abnormalities, and the histology of the digital artery all supported a diagnosis of systemic sclerosis. Newer diagnostic tools may identify patients who do not fit easily into the current classification of that disease.

Prevalence and gender - specific analysis of a systemic sclerosis cohort in Latvia.

BACKGROUND: Systemic sclerosis (SSc) is considered by many to be one of the most severe autoimmune rheumatic diseases with lower prevalence … ear (6.75:1). Antinuclear antibodies were present in 82.58% of patients, without gender difference. Centromere pattern was more frequently observed in females (40.19% vs. 19.04%), in contrast to speckled pattern (50.98% vs. 57.14%). At disease onset females tended to be younger (46.51 ± 13.52) than males (50.5 ± 16.64). Males had more diffuse cutaneous subty …

A high degree of aneuploidy, loss of p53 gene, and low soluble p53 protein serum levels are detected in ulcerative colitis patients.

PURPOSE: The causes for the increased risk of colorectal cancer associated with ulcerative colitis have not been fully defined. Colonic tissue of ulcerative colitis patients was examined for changes in chromosome-17-centromere copy number, loss of the p53 gene, and alterations in serum levels of the 53-kDa protein. This study was performed under the assumption that these molecular events correlate with ulcerative colitis status and duration. METHODS: Ulcerative colitis patients (n = 42) and healthy controls (n = 37) participated in the study. All participants were histopathologically and medi…

Validation of chromosome painting. II. A detailed analysis of aberrations following high doses of ionizing radiation in vitro.

Fluorescence in situ hybridization with chromosome-specific composite DNA probes ('chromosome painting') is useful for quantifying radiation … be explained by an excess of one class of translocated chromosomes, specifically those in which the centromere is from an unpainted chromosome. Translocated chromosomes in which the centromere is painted were found to occur at a frequency equal to dicentrics. These results should help clarify the use of painting for radiation biodosimetry by improving our un …

Y;autosome translocations and mosaicism in the aetiology of 45,X maleness: assignment of fertility factor to distal Yq11.

Three 45,X males have been studied with Y-DNA probes by Southern blotting and in situ hybridization. Southern blotting studies with a panel of mapped Y-DNA probes showed that in all three individuals contiguous portions of the Y chromosome including all of the short arm, the centromere, and part of the euchromatic portion of the long arm were present. The breakpoint was different in each case. The individual with the largest portion (intervals 1-6) is a fertile male belonging to a family in which the translocation is inherited in four generations. The second adult patient, who has intervals 1-…

Bicaudal D2 is a novel autoantibody target in systemic sclerosis that shares a key epitope with CENP-A but has a distinct clinical phenotype.

We studied the clinical correlations and epitopes of autoantibodies directed to a novel autoantigen, Bicaudal D (BICD2), in systemic sclerosis (SSc) and reviewed its relationship to centromere protein A (CENP-A). 451 SSc sera were tested for anti-BICD2 using a paramagnetic bead immunoassay and then univariate and multivariate logistic regression was used to study the association between anti-BICD2 and demographic and clinical parameters as well as other SSc-related autoantibodies. Epitope mapping was performed on solid phase matrices. 25.7% (116/451) SSc sera were anti-BICD2 positive, of which…

[An elderly woman with stiffness of the thumb].

A 81-year-old woman presented with progressive pain, stiffness and swelling of the right thumb. She also complained of dysphagia and Raynaud's phenomenon. Physical examination revealed sclerodactyly and telangiectasia. A plain X-ray showed marked calcifications of the flexor tendon of the right thumb and esophagography demonstrated decreased motility of the lower esophagus. Additionally, anticentromere antibodies were positive. As a result, the patient was diagnosed with calcinosis as a presenting symptom of limited systemic scleroderma.

Speckled pattern antinuclear antibodies resembling anticentromere antibodies.

Two distinct nuclear antigens, designated NSpI and NSpII, have been characterized and differentiated from the centromeric antigen that reacts with sera from patients with the CREST syndrome. Both NSpI and NSpII produce a speckled pattern of indirect immunofluorescence on HEp-2 cells that resembles the pattern seen with anticentromere antibodies (ACA). They are differentiated from the ACA staining pattern by the absence of metaphase chromatin staining by NSpI antisera and by the absence of a discrete speckled pattern of staining by NSpII. Further, both NSpI and NSpII stain predominantly the per…

Digital gangrene without sclerodactyly in the presence of anticentromere antibodies: A unique presentation of scleroderma.

Peripheral vascular disorders associated with anti-centromere antibody (ACA) present generally with sclerodactyly in connective tissue disorders (CTD). It is unusual for ACA-associated digital necrosis to develop without preexisting raynaud's phenomena or vascular risk factors. We report a novel case of a 60-year-old non-smoker, non-diabetic woman with ACA and isolated finger necrosis without any other features of CTD. This case underscores the importance of considering an autoimmune contribution to the development of digital necrosis even without any identifiable CTD. It highlights the need t…

A Case of Intravascular Diffuse Large B-Cell Lymphoma Initially Suspected as Interstitial Pneumonia Associated With Systemic Scleroderma.

Intravascular large B-cell lymphoma (IVLBCL) is a rare form of diffuse LBCL. The patient was a 71-year-old female admitted to our hospital with hypoxia. On admission, chest computed tomography revealed a ground-glass opacity. Interstitial pneumonia associated with systemic scleroderma was suspected because of positive anti-centromere antibody. Thereafter, steroid pulse therapy and plasma exchange were performed. Although ground-glass opacity improved, bilateral pleural effusion appeared, so we performed a random skin biopsy because of her elevated serum lactate dehydrogenase and soluble interl…

Acute right heart failure and achalasia-like syndrome in a patient with limited cutaneous systemic sclerosis and primary biliary cirrhosis.

We report a case of a 63-year-old woman who developed acute right heart failure and an achalasia-like syndrome with limited cutaneous systemic sclerosis (lcSSc) and primary biliary cirrhosis. Intravenous administration of diuretics improved her acute heart failure. Anti-centromere antibodies and anti-mitochondria antibodies were present. A coronary angiogram and a Swan-Ganz catheter revealed no abnormalities. Thallium-201 scan at rest demonstrated mild perfusion defects in both the apex and the anteroseptal and the inferior myocardium. A cine-esophagram revealed an achalasia-like syndrome. Tho…

Pure and complete trisomy 18p due to a supernumerary marker chromosome associated with moderate mental retardation.

Trisomy for the short arm of chromosome 18 or trisomy 18p, is rarely described. We report on a 13-year-old boy with minor facial anomalies, mental retardation, bilateral cryptorchidism associated with a de novo supernumerary marker chromosome (SMC). Using fluorescence in situ hybridization and comparative genomic hybridization analyses, this SMC corresponded to the p arm of chromosome 18 associated with a centromere of either chromosome 13 or 21 and nucleolus organizing regions (NORs). We report here the first case of a pure and complete trisomy 18p due to a SMC. This report and review of lite…
