Atypical primary sclerosing cholangitis cases associated with unusual pancreatitis.

BACKGROUND/AIMS: We encountered one group of primary sclerosing cholangitis cases which met classic criteria, but demonstrated better clinical courses. METHODOLOGY: We evaluated the clinical characteristics of several cases of primary sclerosing cholangitis, especially those associated with chronic pancreatitis. RESULTS: Of a total of 25 cases, eight had a background of pancreatitis with unusual irregularly segmental or diffuse narrowing features on pancreatography. Abdominal ultrasonography and computed tomography also revealed segmental or diffuse enlargement of the pancreas. Pathological …

Autoimmune cholangitis within the spectrum of autoimmune liver disease.

Autoimmune cholangitis is an idiopathic disorder with mixed hepatocellular and cholestatic findings. Our goal was to characterize the disease prospectively by application of uniform diagnostic criteria. Twenty patients were identified and compared with 242 patients with conventional forms of autoimmune liver disease. Patients with autoimmune cholangitis were distinguished from type 1 autoimmune hepatitis (AIH) by lower serum levels of aspartate transaminase (AST), gamma-globulin, and immunoglobulin G; higher serum levels of alkaline phosphatase; and lower frequencies of autoantibodies. They we…

A revised natural history model for primary sclerosing cholangitis.

OBJECTIVE: To describe a natural history model for primary sclerosing cholangitis (PSC) that is based on routine clinical findings and test results and eliminates the need for liver biopsy. PATIENTS AND METHODS: Using the Cox proportional hazards analysis, we created a survival model based on 405 patients with PSC from 5 clinical centers. Independent validation of the model was undertaken by applying it to 124 patients who were not included in the model creation. RESULTS: Based on the multivariate analysis of 405 patients, a risk score was defined by the following formula: R = 0.03 (age [y])…

[Autoimmune cholangitis (AIC) associated with myositis in monozygotic twins].

A 53-year-old female suffered from joint pain and muscle weakness in all extremities in 1991. Laboratory data showed increased creatine phos … of monozygotic twins with both conditions are very rare. The difference between PBC and autoimmune cholangitis (AIC) which has recently reported and the genetic influence on the pathogenesis of DM and AIC are also discussed.

Sclerosing cholangitis associated to cryptosporidiosis in liver-transplanted children.

UNLABELLED: Three children of a series of 461 pediatric liver transplant recipients developed diffuse cholangitis associated with intestinal cryptosporidium carriage. All three received immunosuppression consisting of tacrolimus and prednisone. Cryprosporidium carriage was treated with paramomycin, while immunosuppression was decreased according to graft tolerance. No other infectious pathogens were found, and no vascular problems were detected. Bile duct anastomosis was reoperated in all three, but biliary cirrhosis developed in one patient, requiring retransplantation. All three patients are…

Autoimmune cholangitis and primary biliary cirrhosis--an autoimmune enigma.

AIMS/BACKGROUND: Autoimmune cholangitis/cholangiopathy (AIC) is an enigmatic disease marked by chronic cholangitis, antinuclear antibodies (ANA) and sero-negativity for conventionally detected antimitochondrial antibodies (AMA). We examined whether AIC is a distinct entity, an AMA-negative variant of primary biliary cirrhosis (PBC), or a cholangiopathic variant of autoimmune hepatitis (AIH) by comparing the clinical, laboratory and autoantibody profiles of 21 cases of AIC, 37 cases PBC and 16 cases of AIH from selected Japanese patients. METHODS: The specificities of AMA and ANA were determin…

[Autoimmune cholangitis or primary biliary cirrhosis without antimitochondrial antibodies?].

The term cholangitis or autoimmune cholangiopathy is applied to patients with a disease similar to primary biliary cirrhosis because of its clinical and histologic characteristics but in whom there is a repeated absence of antimitochondrial antibodies and, by the contrary, the presence of antinuclear antibodies. The initial description of these cases was carried out in 1987. Good response was observed to treatment with glucocorticoids in these first cases and others described later. A series of 13 female patients with features of autoimmune cholangitis diagnosed from 1987 to 1993 in the Hospit…

Primary sclerosing cholangitis in Japan--analysis of 192 cases.

Primary sclerosing cholangitis is very rare in Japan. The aim of the present study was to identify the characteristics of such patients in Japan. A questionnaire was sent to the members of the Japanese Society of Gastroenterology and responses for 192 cases were analyzed. There was male predominancy (61%), and two peaks in the age distribution at diagnosis (20-30 years and 50-70 years). Bile duct damage was mainly intra + extrahepatic (69%) versus intrahepatic (17%) or extrahepatic (14%). The incidences of eosinophilia and positivity for anti-nuclear antibody were 27% and 30%, respectively. Th…

Bile duct calculi in patients with primary sclerosing cholangitis.

PURPOSE: To determine the imaging appearance and frequency of detection of bile duct calculi in patients with primary sclerosing cholangitis. MATERIALS AND METHODS: Images (169 computed tomographic [CT] scans, 155 sonograms, and 109 cholangiograms) of 189 patients with primary sclerosing cholangitis, five of whom were prospectively included, were reviewed. Pathologic records were reviewed for proof of the presence of calculi. RESULTS: Bile duct calculi were visualized on images in 14 (7.6%) of the 184 retrospective patients and in all five of the prospective patients. The presence of calculi…

Natural history and prognostic variables in primary sclerosing cholangitis.

The clinical features at the time of presentation and the outcome in 126 patients with primary sclerosing cholangitis were studied to clarify the natural history and prognosis in symptomatic and asymptomatic individuals. The median age of the patients at the time of presentation was 36 years, 62% were male, and 16% were asymptomatic. The median follow-up from time of presentation was 5.8 years. There were more patients who had liver transplants (21%) than patients who died of liver-related disease (16%); the estimated median survival to these end points was 12 years. Cholangiocarcinoma was fou…

Symptom Network Analysis in Patients With Primary Sclerosing Cholangitis.

INTRODUCTION: Patients with primary sclerosing cholangitis (PSC) experience numerous symptoms that impact quality of life. We sought to identify PSC-specific symptom clusters and relationships between symptoms to better understand the lived experience of PSC patients. METHODS: Presence and severity of 16 symptoms were identified from the international "Our Voices" survey collected by PSC Partners Seeking A Cure in 2020. LASSO-regularized partial correlation network analysis was performed using Spearman correlations. The primary analysis utilized symptoms reported within the past 6 months of s…

Cardiovascular risk profiles in patients with primary sclerosing cholangitis.

INTRODUCTION: Chronic inflammation as seen in patients with primary sclerosing cholangitis (PSC) is linked to accelerated development and progression of atherosclerosis. This study aimed to assess atherosclerotic cardiovascular disease (ASCVD) risk and cardiovascular health (CVH) profiles in patients with PSC. METHODS: This cross-sectional study included PSC patients with and without concomitant inflammatory bowel disease (IBD), assessing cardiovascular risk with anthropometric measurements, serum samples, self-reported questionnaires, and medical chart review. The ideal CVH score was calcula…

A hepatic enigma: Pediatric presentation of primary biliary cholangitis.

Primary biliary cholangitis (PBC) is a chronic autoimmune condition characterized by destruction of intrahepatic bile ducts, leading to fibrosis and cirrhosis of the liver. It is an extremely rare pediatric disease with very few pediatric cases reported to date. Here, we report the case of a 14-year-old female who presented with elevated liver enzymes and was subsequently diagnosed with PBC. Our case raises awareness of the possibility of PBC in children.
