Choroidal tuberculoma with membranous glomerulonephritis.

We report treatment of a 24-year-old man with membranous glomerulonephritis (MGN) who developed a solitary choroidal tuberculoma in association with miliary tuberculosis during steroid therapy. In June 1995, the patient had developed nephrotic syndrome. He had refused renal biopsy at that time. So we treated him with corticosteroids having assumed a diagnosis of minimal change nephrotic syndrome. After initial corticosteroids and diuretics therapy for 5 months, his generalized edema resolved but proteinuria (3 positive) continued, suggesting the presence of other forms of glomerulonephritis. R…

[Subretinal surgery for choroidal neovascularization].

BACKGROUND: Ten years have passed since subretinal surgery for choroidal neovascularization (CNV) was developed. Many reports regarding the visual outcome after subretinal surgery have been published, which have revealed the role of the subretinal surgery, including long-term visual outcome and surgical complications. METHOD: Reports associated with subretinal surgery published in the past decade were used to clarify the indication for surgery, visual outcome, and surgical complications. RESULTS: The visual prognosis for patients with surgically excited CNV is variable, depending on the unde…

[Choroidal tubercles in miliary tuberculosis].

OBJECTIVE/METHOD: We present 2 cases of miliary tuberculosis with choroidal tubercles. RESULTS/CONCLUSIONS: Miliary tuberculosis is a severe disease that may cause death in case of misdiagnosis, but might be resolved with an adequate mycobacterial therapy. The diagnosis was based on a combínation of findings and the presence of choroidal tubercles may play an important role in the final diagnosis and correct treatment.

Confluent choroidal infiltrates with sarcoidosis.

PURPOSE: To report the occurrence of confluent plaquelike choroidal infiltrates in four patients with sarcoidosis. METHODS: The medical records of patients with choroidal plaquelike infiltrates and presumed systemic sarcoidosis seen in the Mayo Clinic were reviewed. RESULTS: The cohort included four patients with confluent plaquelike choroidal infiltrates and systemic sarcoidosis. The most common ophthalmic symptom experienced by the patients was mild blurring of vision. The salient ophthalmic findings were choroidal infiltrates generally unaccompanied by other features frequently seen with …

Choroidal calcification in Bartter syndrome.

PURPOSE: Bartter syndrome is characterized by hyperplasia of the renal juxtaglomerular apparatus, hyperaldosteronism, and hypokalemic alkalosis. We report a case of Bartter syndrome associated with normal serum calcium levels and posterior choroidal calcification. METHODS: Case report. A 59-year-old man with bilateral cataract and Bartter syndrome underwent a complete ophthalmic examination, including standardized echography before and after cataract surgery. RESULTS: Before cataract surgery, echography identified small, hyperreflective, multifocal, bilateral choroidal lesions with posterior…

Transpupillary thermotherapy in choroidal melanomas.

OBJECTIVE: To determine safety and efficacy of transpupillary thermotherapy (TTT) as a new treatment for choroidal melanoma. METHODS AND PATIENTS: To perform TTT, diode laser energy at 810 nm was used with a beam diameter of 1.5 to 4.5 mm for a 1-minute exposure. All 12 patients had choroidal melanoma. Six had had insufficient response to ruthenium 106 (106Ru) brachytherapy. Three patients with tumors more than 5 mm in height were treated simultaneously with 106Ru and TTT. Three patients with juxtapapillary or macular tumors were treated by TTT only. RESULTS: All but one tumor exhibited a re…

Choroidal tubercles with tuberculous meningitis.

We found choroidal tubercles in two children with meningitis. This finding supposed an important clue in establishing a tuberculous etiology. Following, we discuss the evolution and fluorescein angiographic findings of choroidal tubercles. As many authors have remarked, a thorough fundus examination is of great value in cases of fever of undetermined origin and meningitis. Choroidal tubercles can also be the first sign of a common pulmonary or extrapulmonary tuberculosis.

Macular choroidal occlusion in dysplasminogenemia.

The authors describe a family with dysplasminogenemia. This diagnosis was based on a lowered ratio of functional plasminogen activity to immunologic plasminogen antigen level, associated with fundus abnormalities. The proband is a 31-year-old woman with Graves disease who had choroidal occlusive lesions of the macula in both eyes. The results of the laboratory examination indicated that the patient was a homozygote for dysplasminogenemia. Similar examinations of the patient's parents and siblings showed their plasminogen levels to be 50% below normal. The thrombotic tendency in this disorder a…

[Optic disk and choroidal coloboma].

Most of the optic nerve head abnormalities are clinically innocuous, but sometimes they can cause significant symptoms and lead to visual disturbance, or even to visual loss; optic pists can cause defects in the visual fields, not necessarily explicable by the serous maculopathy, that is the most important complication associated with optic pits. Our purpose is to present a case of an optic pit with sensory macular detachment associated with a choroidal coloboma located at a distance of two optic disc diameters from the optic nervehead. This finding to our knowledge has not been previously rep…

Current management of choroidal neovascularization.

Age-related macular degeneration is the leading cause of blindness in Americans over age 65. In 1982, the Macular Photocoagulation Study (MPS) established that argon laser photocoagulation reduces the risk of severe visual loss in eyes with extrafoveal choroidal neovascularization (CNV) associated with macular degeneration. Subsequently, similar results were found for histoplasmic and idiopathic choroidal neovascularization. There are also encouraging data on photocoagulation of CNV in other diseases and with different wavelengths. The MPS Group has found the krypton laser to be effective for …

Acute intraoperative choroidal effusion.

We studied five cases of acute choroidal effusion that occurred during intraocular surgery and one case of a localized choroidal hemorrhage. Acute intraoperative choroidal effusion is a poorly recognized surgical complication and may precede expulsive hemorrhage in many cases. The treatment is immediate closure of the wound, after which the intraocular pressure may rise to 80 mm Hg or more. However, the pressure will return to normal after 15 to 30 minutes and the area of effusion will remain localized. Scleral puncture to drain fluid is not indicated if the wound can be closed before there is…

Thirty six cases of toxoplasma chorioretinitis with fundus scars were studied. Subretinal neovascularization occurred in 7 cases out of 36. … nal acuity (0.18 after a follow-up period from 5 to 42 months). The visual outcome of toxoplasmosis choroiditis without subretinal new vessels is usually around 0.5. In 3 eyes photocoagulation of new vessels was performed with improvement in the mean final acuity (0.3). In our cases the occurrence of subretinal new vessels happened only when the healing p …

Computed tomography in choroidal detachment.

Computed tomography proved valuable in localizing and differentiating serous or hemorrhagic choroidal detachment and scleral infolding. The mean attenuation values (CT numbers) in the region of fresh hemorrhagic choroidal detachment were 74 Hounsfield units (HU). The 90% confirmation limit of the mean was 55 to 84 HU. For serosanguineous choroidal detachment, the mean was 50 HU, with a 90% confirmation limit of 46 to 54 HU. For serous choroidal detachment the mean was 46 HU, with a 90% confirmation limit of 39 to 53 HU. Inflammatory choroidal detachment showed uveoscleral enhancement, which in…

[Echographic findings in choroidal detachment].

A- and B-scan sonography was performed in 83 patients with choroidal detachment; a serous choroidal detachment is characterized by high, bifurcated echospikes and an echo-free subchoroidal space in A-scan sonography. The B-scan presents a typical arc shape. In hemorrhagic choroidal detachment multiple echoes with low to medium reflectivity can be demonstrated in the subchoroidal space. The echographic differential diagnosis of other intraocular diseases is discussed.

Choroidal lacunae and Aicardi's syndrome.

Three children with flexion spasms and an ophthalmologic picture characterised by choroidal lacunae are described. Aicardi's syndrome was diagnosed in two cases. A diagnosis of Aicardi's syndrome should include CT scan and/or pneumoencephalographic findings of partial or total agenesis of the corpus callosum and/or other anomalies of the interhemispheric structures. Choroidal lacunae are not entirely pathognomonic.

Prognosis in metastatic choroidal melanoma.

We reviewed the records of 73 patients with primary melanoma of the choroid and ciliary body with metastasis treated at M. D. Anderson Hospital and Tumor Institute between 1973 and 1979. At time of diagnosis of primary melanoma 71 of 73 patients had tumor localized to the eye and were treated with enucleation of the affected eye. The interval from resection of primary tumor to detection of systemic metastases in the 71 patients ranged from one to 201 months (median 43.5 months). Weight loss and abdominal pain due to hepatomegaly were the most common symptoms, and hepatomegaly was the most comm…
