INTRODUCTION: Chylothorax is a rare complication of surgical neck dissection. This is the first reported pediatric case of bilateral chylothorax following cervical surgery and the first to occur after tracheoplasty. Chylothorax can lead to significant complications, including hypoxemia and shock, and requires timely treatment. Chylothorax occurs in 0.25 to 0.50% of cardiac operations performed through thoracotomy incisions and is more unusual after median sternotomy. A case of chylothorax following coronary artery bypass grafting is presented. Combined treatment with pleural drainage, "nothing per os", total parenteral nutrition and subcutaneous injection of somatostatin was effective and led to rapid cessation of chyle production.

Voriconazole (VRZ) is an effective first-line treatment for invasive pulmonary aspergillosis. This case report aims to describe the unachievable therapeutic VRZ serum levels in a patient with probable invasive pulmonary aspergillosis and idiopathic bilateral chylothorax with chylopericardium requiring intercostal drainage. Moreover, extrahepatic VRZ loss via intercostal drainage was observed by measuring VRZ in the chylous fluid, which was comparable to the serum concentration.

Chylothorax, a known complication of surgery for Congenital Diaphragmatic hernia, can sometimes be resistant to treat. Octeriotide (Somatostatin analogue) can be useful in this situation. However, the dose and schedule of Octeriotide therapy in neonates is not well established. We report two cases of resistant chylothorax following surgery for congenital diaphragmatic hernia which were successfully managed by using an escalating infusion of octeriotide. The literature on the subject is also reviewed.
