INTRODUCTION: The management of displaced fractures of the distal clavicle remains controversial, particularly in younger patients where there is no consensus as to which surgical intervention is best. Each surgical method has unique surgical complications and rates of persistent pain and post-traumatic arthritis. We report an innovative surgical technique using a plate fixation augmented with minimally invasive tension slide coracoclavicular fixation using a cortical tenodesis button (8.5 mm).
METHODS: A single-surgeon series, comprising of eleven cases, underwent retrospective review. A low…

Neer type II distal clavicle fractures are inherently unstable. The purpose of this study was to review the outcome of Neer type II distal clavicle fractures arthroscopically treated using a suspensory cortical button technique. Between 2008 and 2012, a total of 17 Neer type IIB fractures were managed operatively at the authors' institution. Functional outcomes were assessed using the pain score, the Disabilities of the Arm, Shoulder and Hand score, the Penn Shoulder Score, and the American Shoulder and Elbow Surgeons score. Radiographic union was also assessed. At a mean of 1 year, the mean p…

BACKGROUND: The treatment of distal clavicle fracture is always a challenge, as it is mostly unstable and has higher rate of delayed union, malunion, non-union and associated acromioclavicular arthritis. So the management of these fractures remains controversial. The purpose of this study is to evaluate the functional results of Type 2 distal end clavicle fractures treated with superior anterior locking plate.
METHODS: From June 2011 to August 2015 a retrospective study of12 male patients (mean age of 41.3 years) 11 with unilateral and 1 with bilateral distal clavicle fractures treated with s…

Middle third clavicle fractures are common injuries that need surgical fixation in a certain subset of patients. The use of pre-contoured clavicle plates facilitates intra-operative fixation and can reduce soft tissue irritation and hardware removal rates. Lag screw fixation is a well-established technique for the treatment of oblique fractures. Empirical application of this method in clavicle fracture fixation and clavicle non-union treatment has been reported previously. A recent biomechanical study supports the use of a lag screw technique in clavicle fracture fixation when possible. We rep…

Review of available English literature suggests that combined injuries involving a clavicle shaft fracture and an acromioclavicular (AC) separation are rare. The force dissipation after the occurrence of either a midshaft clavicle fracture or an AC separation typically renders the injuries mutually exclusive. This article presents a review of literature on this combined injury pattern. A variety of treatment approaches have been put forth, including nonoperative, operative, and hybrid management of the two distinct injuries. The most appropriate treatment rendered depends on the degree of AC j…

Runx2 is an essential transcription factor for osteoblast differentiation and chondrocyte maturation. Heterozygous mutations in RUNX2 cause cleidocranial dysplasia, characterized by an open fontanel, hypoplastic clavicles, and short stature. Runx2 is transcribed from two promoters, P1 and P2, generating Runx2-II and Runx2-I, respectively. However, the function of Runx2-I remains unclear. To clarify its function, we generated Runx2-I-specific knockout mice by systematically deleting the P2 region while retaining the downstream sequence from the branch site for intron 1 splicing. As the 2.5-kb a…

INTRODUCTION: Clavicle fractures are common in children, representing 7-15% of pediatric fractures, typically healing with conservative management. Non-union is exceedingly rare, especially in children under 3 years of age.
CASE REPORT: We report a case of a 3-year-old male with traumatic midshaft clavicle fracture non-union, presenting 2 years post-injury with persistent pain, swelling, deformity, and restricted shoulder function. Radiographs revealed a hypertrophic non-union with significant displacement and 2 cm shortening. The patient was managed surgically with open reduction, internal f…

PURPOSE: The operative treatment of mid-shaft clavicle fractures shows benefit in union rates, return to work, and lower pain scores relative to non-operative treatment. We sought to determine if the surgical treatment of isolated mid-shaft clavicle fractures would result in fewer opioids prescribed as compared to those managed non-operatively.
METHODS: All mid-shaft clavicle fractures treated at a Level 1 trauma center were identified from 2012 to 2016. Demographics, fracture characteristics, surgical complications/outcomes, non-operative outcomes, and all narcotics prescribed for 6 months p…

The clavicle has various anatomic shapes unique to each individual. Additionally, with the increase in high-energy traumas such as sports injuries and traffic accidents, the patterns of fractures become complex and complicated. Thus, there is a need for a variety of shapes of locking compression plates (LCP) to accommodate different types of fractures and facilitate quicker rehabilitation. The aim of this study is to present different types of LCP that secure fracture fragments and distribute stress evenly, in comparison to typical anatomical LCPs, for reinforcing clavicle fractures. Three mod…

CASE: An 18-year-old woman with a history of congenital pseudarthrosis of the clavicle (CPC) presented with episodes of right upper extremity ischemia. Vascular studies demonstrated an extensive thrombus with complete occlusion of the brachial artery. She underwent urgent thrombectomy. Subsequently, she underwent first rib resection and scalenectomy as well as pseudarthrosis takedown and fixation. Postoperatively, she returned to Division I collegiate soccer with complete symptomatic resolution.
CONCLUSION: We report a case of arterial thoracic outlet syndrome secondary to CPC.

Clavicle fractures combined with sternoclavicular joint dislocations are very rare injuries that need to be addressed quickly and treated effectively due to the altered biomechanics of the shoulder girdle as well as to the potential damages to the surrounding "noble" anatomical structures. A diagnostic and therapeutic algorithm for such injuries has not yet been established. Computer Tomography with 3D reconstruction should be the diagnostic gold standard. In case of a highly displaced fracture and/or dislocation in an active patient, surgical treatment is advised in order to obtain proper bon…

CASE: A 77-year-old man with an acromioclavicular joint ganglion cyst with cuff tear arthropathy had a large mass in the left shoulder. Arthroscopic distal clavicle resection successfully relieved his symptoms without loss of shoulder function by enlargement of the bypass between the cyst and subacromial space through the acromioclavicular joint.
CONCLUSION: Arthroscopic distal clavicle resection can remove a 1-way check valve in a minimally invasive manner. This case indicated that arthroscopic distal clavicle resection could be a useful treatment option for patients with acromioclavicular j…

CASE: Although congenital nonunion of the clavicle is a well-known pathology in children, posttraumatic nonunion is a rare entity. Nonunion after open reduction and internal fixation of a clavicle fracture is exceptional in the pediatric population. We report on a patient presenting posttraumatic pseudarthrosis of the clavicle after surgical treatment. Pseudarthrosis was resected, and the defect was bridged with an iliac crest autograft, supplemented with bone morphogenetic protein, and fixed with an elastic stable intramedullary nail.
CONCLUSION: This procedure offered enough stability for a…

BACKGROUND: Fracture of distal-end accounts for 20% of all clavicle fracture. In the situation of impaired bone healing environment such as chronic kidney disease (CKD), nonunion rates after surgery might increase. In the case where bone healing is unexpected, biological healing with tendon graft could be an alternative method to maintain bone reduction.
PRESENTATION OF CASE: A 62-year old male with a history of end-stage CKD presented to the hospital with pain, wound, and deformity on the right shoulder. The patient have had surgery in the past 4 months and resulted in a non-union fracture o…

Synovitis-acne-pustulosis-hyperostosis-osteitis (SAPHO) syndrome is considered after exclusion of infection and arthritis; however, microbial infection may be present in osteoarticular lesions of these patients. Chronic osteomyelitis and associated bacterial infection were detected in a recurrent osteoarticular lesion in an adolescent patient with a history of clavicle pain, who complained of recurrent swelling in the left clavicle. Most pediatric case reports of SAPHO syndrome describe patients with associated skin conditions. This case report describes a patient diagnosed with SAPHO syndrome…

BACKGROUND: The Constant score (CS) has been the primary endpoint in most studies on clavicle fractures. However, the CS was not developed to assess patients with clavicle fractures. Our aim was to examine inter-rater reliability and agreement of the CS in patients with clavicle fractures. The secondary aim was to estimate the correlation between the CS and the Disabilities of the Arm, Shoulder and Hand score and the internal consistency of the 2 scores.
METHODS: On the basis of sample sizing, 36 patients (31 male and 5 female patients; mean age, 41.3 years) with clavicle fractures underwent …
