On-premise medical AI agents for reliable clinical decision-making.

Autonomous clinical artificial intelligence (AI) agents powered by large language models (LLMs), meaning systems that can complete a diagnostic workflow without continuous human input, are increasingly capable of supporting complex reasoning and decision-making. Clinical translation, however, remains limited by two unmet requirements: institutionally governed deployment and reliable decision-time uncertainty estimation. Here we developed and evaluated a fully on-premise clinical agent that couples local operational control with a multi-perspective reliability framework to support selective aut

[The transdiagnostic approach of psychological distress in clinical practice].

BACKGROUND: Mental healthcare is undergoing a paradigm shift from traditional disorder-specific diagnostics toward a transdiagnostic framework for conceptualizing psychological distress. This framework facilitates an integrated understanding of underlying mechanisms that cut across conventional diagnostic boundaries. AIM: To present a clinically applicable model of transdiagnostic clinical reasoning, elucidating its added value in routine practice and delineating its distinctions from disorder-focused approaches. METHOD: A narrative synthesis of the literature was conducted to integrate curr…

Engineering safety into AI-assisted music medicine for headache management: a transparent guardrail framework.

While generative artificial intelligence enables large-scale, individualized, and digitally scalable interventions, its deployment introduces model instability and algorithmic reasoning errors. To ensure patient safety, the next frontier for AI-assisted music medicine therapeutics is not merely tool optimization, but the implementation of a transparent, rule-based clinical safety guardrail.

From authoring to editing: a competency framework for trainee use of ambient AI clinical documentation.

Ambient AI has spread through clinical settings faster than curricula can adapt, shifting trainee documentation from authoring to editing notes. Authoring historically honed clinical reasoning for trainees, yet medical education is not prepared to develop the editing task now replacing it. We define this task as a teachable competency with four domains - error detection, clinical reconciliation, independent generation, and trust calibration - and describe how simulation can teach them.

Seeing the patient: professional vision in clinical ward rounds (and beyond).

Clinical ward rounds are complex, high‑stakes encounters in which physicians must integrate clinical reasoning, team coordination, and patient‑centred communication. While ward rounds are routinely described as occasions on which physicians "see" their patients, ironically, the visual dimension of ward round communication has rarely been examined conceptually. In this paper, we argue that communication during ward rounds and other complex clinical encounters crucially relies on physicians' professional vision: learned, task-specific ways of noticing, interpreting, and using visual information …

Developing and evaluating a rubric to assess medical students' analytic thinking and predict academic risk.

PROBLEM: Analytic reasoning requires learners to integrate knowledge, demonstrate analytical skills, and employ metacognition to avoid cognitive biases. Mechanism Maps (MMs), student-generated concept maps, support this process by requiring students to link clinical findings with pathophysiologic mechanisms. However, broader adoption of MMs has been limited by challenges in assessment: existing scoring approaches emphasize structural features rather than accuracy of reasoning, show variable interrater reliability and are time-intensive. Importantly, it remains unclear whether MM-based assessme

Effect of AI-driven simulation integrated with virtual case-based training on diagnostic and therapeutic reasoning homogenization among junior ophthalmologists: a prospective cohort study.

BACKGROUND: Conventional training models for diabetic retinopathy (DR) inadequately address the dynamic decision-making demands required for diagnostic reasoning standardization among junior ophthalmologists. OBJECTIVE: To evaluate whether AI-driven simulation integrated with virtual DR case training is associated with greater homogenization of diagnostic and therapeutic reasoning compared with conventional case-based instruction. METHODS: This single-center prospective cohort study enrolled 100 junior ophthalmologists (1-5 years' clinical experience) assigned to an AI-integrated cohort (n =

A qualitative thematic comparative analysis of independent and AI-supported clinical reflections among medical interns.

BACKGROUND: Reflection is fundamental to medical education, supporting clinical reasoning, professional identity formation, and lifelong learning. Although generative artificial intelligence (AI) chatbots have emerged as tools to facilitate reflection, little is known about how AI-supported reflection differs from independent reflective writing. This study compared the characteristics and educational contributions of independent reflective writing and AI-supported reflection among medical interns. METHODS: A comparative qualitative quasi-experimental study was conducted with medical interns d…

[An abnormal chest X-ray; difference between 'community-acquired' pneumonia and non-infectious lung conditions].

Distinguishing between infectious pneumonia and other causes of abnormal X-rays can be challenging. This commentary describes how this distinction can be made by means of sound clinical reasoning, making judicious use of laboratory tests and especially by using HRCT scanning to confirm certain diagnoses on the basis of well-known radiology patterns.

The role of neuropsychology in distinguishing the posterior cortical atrophy syndrome and Alzheimer's disease.

This study investigated the neuropsychological hallmarks of posterior cortical atrophy (PCA). Seventeen patients with PCA, 17 patients with probable Alzheimer's disease (PAD), and 17 healthy age-matched subjects underwent neuropsychological testing for abstract reasoning, visuospatial abilities, memory, language, executive functions, praxes, and attention. The PCA patients were significantly more impaired in visual perception, spatial memory, visual attention, and visuospatial reasoning compared to the PAD patients who were relatively more impaired in episodic memory. In the PCA group, no test

Symbolic and non-symbolic geometric abilities across development and in individuals with dyscalculia and dyslexia.

Geometry is fundamental to mathematical thinking, yet its foundations and difficulties in learning disorders remain poorly understood. Here, we investigated the development of geometric abilities and their relationship with developmental dyscalculia and dyslexia. Seventy-nine children and adults, including individuals with and without learning difficulties, completed behavioral measures of non-symbolic geometry and a paper-and-pencil assessment of symbolic geometry. Geometric abilities continued to develop from early adolescence into young adulthood and non-symbolic geometry predicted symbolic

Neuropsychological aspects of the Moyamoya disease: A case report.

Moyamoya syndrome is characterized by endothelial hyperplasia fibrosis of the carotid artery and its adjacent pathways, and its main clinical feature is the occurrence of strokes. It has a prevalence of 3 cases per 100,000 children in Asian countries, where it is more common. This study sought to identify the aspects and comorbidities associated with the syndrome based on a previously conducted neuropsychological assessment. Perception, attention, language, social cognition, memory, executive functions, reasoning, and processing speed were assessed, in addition to an inventory for Obsessive-Co

A systems-level framework for drug discovery identifies Csf1R as an anti-epileptic drug target.

The identification of drug targets is highly challenging, particularly for diseases of the brain. To address this problem, we developed and experimentally validated a general computational framework for drug target discovery that combines gene regulatory information with causal reasoning ("Causal Reasoning Analytical Framework for Target discovery"-CRAFT). Using a systems genetics approach and starting from gene expression data from the target tissue, CRAFT provides a predictive framework for identifying cell membrane receptors with a direction-specified influence over disease-related gene exp

A case of seizures induced by abstract reasoning.

We describe a case of reflex seizures induced by abstract reasoning but not other cognitive processes. The patient, a 46-year-old man, experienced myoclonic seizures whenever he played shogi (Japanese chess). To identify the critical thought processes responsible for inducing his seizures, we monitored his clinical seizures and epileptiform discharges while he performed comprehensive neuropsychological tests, including the Wechsler Adult Intelligence Scale-Revised (WAIS-R), spatial working memory, mental rotation, and Wisconsin Card Sorting Test (WCST) tasks. A myoclonic seizure occurred only

Does Artificial Intelligence Make Dermatologists Less Intelligent?

Artificial intelligence is transforming dermatology, but may introduce risks of cognitive offloading and automation bias. Excessive reliance on AI could weaken independent clinical reasoning, particularly among trainees. Rather than replacing physician judgment, AI should function as a cognitive partner. Responsible integration and AI literacy can enhance dermatologists' efficiency, knowledge, and decision-making.
