Lynch syndrome (LS) is the most common inherited cancer syndrome. It is inherited via a monoallelic germline variant in one of the DNA mismatch repair (MMR) genes. LS carriers have a broad 30% to 80% risk of developing various malignancies, and more precise, individual risk estimations would be of high clinical value, allowing tailored cancer prevention and surveillance. Due to MMR deficiency, LS cancers are characterized by the accumulation of frameshift mutations leading to highly immunogenic frameshift peptides (FSPs). Thus, immune surveillance is proposed to inhibit the outgrowth of MMR-de

BACKGROUND: Estimating risk for advanced proximal neoplasia (APN) based on distal colon findings can help identify asymptomatic persons who should undergo examination of the proximal colon after flexible sigmoidoscopy (FS) screening.
OBJECTIVE: We aimed to determine the risk of APN by most advanced distal finding among an average-risk screening population.
DESIGN: Prospective, cross-sectional study.
SETTING: Teaching hospital and colorectal cancer screening center.
PATIENTS: A total of 4651 asymptomatic persons at average risk for colorectal cancer aged 50 to 74 years (54.4% women [n = 252

The treatment model for colorectal cancer has undergone a significant evolution, progressing from single surgical resection to multidisciplinary team (MDT)-based comprehensive therapy. As a revolutionary strategy in the field of advanced colorectal cancer treatment, conversion therapy aims to transform initially technically or oncologically unresectable advanced tumors into a state of no evidence of disease (NED) through efficient and precise systemic treatments, followed by local interventions such as surgery. This approach offers patients potential curative opportunities and significantly im

Colorectal cancer (CRC) is one of the leading forms of cancer. In 2017, over 72,000 of Russian citizens have been diagnosed with CRC. Cancer stage IV was diagnosed in 18 149 of them. Peritoneal carcinomatosis (PC) is one of the forms of metastatic dissemination throughout the peritoneum. There no any unified and standardized approaches to the treatment or prevention of PC associated with CRC. Therefore, it is advisable to identify PC predictors in patients with colon cancer and prevention measures.

In this issue of Cancer Cell, Manca et al. investigate the genomic evolution and metastatic tropism of over 7,000 colorectal cancers. Integrating primary tumor location, genomic alterations, and clinical variables, they reveal how distinct patterns of tumor evolution shape metastatic competence and impact organ tropism, offering a framework to refine surveillance.

The incidence of thyroid metastases among patients suffering from primary colorectal cancer is rare. As these metastases are usually asymptomatic, they present as incidentalomas on follow-up imaging. Hereby, we present and discuss a case of metastatic sigmoid adenocarcinoma to the thyroid gland, diagnosed and treated at our institution.

Gastric cancer is common with well-established routes of spread. Metastasis to the colon or rectum is rare; however, we have recently managed two patients with this clinical picture. We present these cases together with a literature review of current practice. A systematic review in PubMed using the terms 'gastric cancer' and 'colorectal metastasis' was performed. The identified papers were screened for relevance and the reference lists of relevant papers were also reviewed to ensure capture of all relevant reports. Twenty-four papers containing 26 cases of gastric cancer with metastasis to th

BACKGROUND: Management of cutaneous metastases in colorectal cancer is crucial because it can significantly impact patient survival.
OBJECTIVE: To assess the global prognosis of skin metastases among patients treated for colorectal cancer.
METHODS: A systematic review of the PubMed database only was conducted for English articles or reports published between 1 January 1990 and 1 November 2023. Reports concerning clinical outcomes of patients with cutaneous metastases of colorectal cancer and systematic reviews of the literature were included. Study characteristics and results of eligible stu

The development of surgical techniques, chemotherapy, biological agents, and multidisciplinary approaches have made patients with unresectable colorectal liver metastases eligible for surgery. Many strategies have been developed to allow patients for surgical resection (percutaneous portal vein embolization, liver venous deprivation, parenchyma-sparing liver surgery, reverse strategy, associating liver partition and portal vein ligation for staged hepatectomy, and liver transplantation), the only form of disease control and curative treatment.

Peritoneal metastasis of colorectal cancer is the second most common metastatic pattern after liver metastasis, clinically common and associated with a poor prognosis. Refractory subtypes such as mucinous adenocarcinoma, signet-ring cell carcinoma, and BRAF V600E-mutated colorectal cancers account for a relatively high proportion in peritoneal metastasis. While previous diagnosis and treatment faced significant challenges, recent advances in new technologies and evolving therapeutic concepts have achieved progress in management. Many patients with colorectal cancer peritoneal metastasis have o

BACKGROUND: The global incidence of secondary liver cancer is rising due to multiple risk factors, presenting significant challenges in public health. Similarly, colorectal cancer (CRC) remains a leading cause of cancer-related mortality with the development of frequent liver metastases. Surgical resection of CRC liver metastases is only suitable for a limited subset of patients, necessitating alternative nonsurgical treatments such as electrochemotherapy (ECT); Methods: This review adhered to the S.P.I.D.E.R.
FRAMEWORK: Systematic searches of PubMed, Cochrane, and Scopus databases were condu

BACKGROUND: It is necessary to engage in an appropriate level of physical activity to improve the prognoses of colorectal cancer patients, yet no guidelines currently exist. The goals of this systematic review are to determine the impact of levels of physical activity on the prognoses of colorectal cancer patients and to suggest recommended guidelines for levels of physical activity.
METHODS: This systematic review was conducted along PRISMA guidelines. Per the inclusion criteria, papers published in academic journals in English from 2010 to 2019 were selected. A literature search was perform
