Healthcare Providers' Perspectives on Antidepressant Discontinuation: A Focus Group Study.

Antidepressant (AD) discontinuation in long-term users can be challenging for both patients and healthcare providers (HCPs). Better understanding of how HCPs handle this challenge is needed to improve discontinuation care. Therefore, we identified AD discontinuation barriers and facilitators from the viewpoint of community pharmacists (CPs), general practitioners (GPs), psychiatrists and nurse practitioners and explored their views on their specific roles in AD discontinuation. Two focus group discussions involving four GPs, six pharmacists and three psychiatrists and one interview with a nurs

The role of the TSC Alliance in advancing therapy development: a patient organization perspective.

Tuberous sclerosis complex (TSC) is a genetic disease leading to malformations, or tubers, in the cerebral cortex and growth of tumors, most frequently in the brain, heart, kidneys, skin, and lungs. Changes in the brain caused by TSC usually have the biggest negative impact on quality of life. Approximately 85% of individuals with TSC have epilepsy, and TSC-associated neuropsychiatric disorders (TAND) affect nearly all individuals with TSC in some way. TSC Alliance's research strategy is built upon both funding and catalyzing research. Through grants, the organization provides funding directly

Challenges and Progress in Building a Comprehensive Statewide System for Fetal Alcohol Spectrum Disorders Prevention in Hawai'i.

Introduction Fetal Alcohol Spectrum Disorders (FASD) has been a maternal and child public health concern in Hawai'i for over the past decade. Methods A historical assessment of FASD related activities was conducted to map the challenges and progress made in building a comprehensive statewide system for FASD prevention and intervention in an island state. Results Progress has primarily been reflected in increasing the number of individuals receiving FASD education and training, as well as some initiatives in public awareness. The creation of a State FASD Coordinator position was significant in

Community control in cellular protein production: consequences for amino acid starvation.

Deprivation of essential nutrients can have stark consequences for many processes in a cell. We consider amino acid starvation, which can result in bottlenecks in mRNA translation when ribosomes stall due to lack of resources, i.e. tRNAs charged with the missing amino acid. Recent experiments also show less obvious effects such as increased charging of other (non-starved) tRNA species and selective charging of isoaccepting tRNAs. We present a mechanism which accounts for these observations and shows that production of some proteins can actually increase under starvation. One might assume that

The feasibility of task-sharing the identification, emergency treatment, and referral for women with pre-eclampsia by community health workers in India.

BACKGROUND: Hypertensive disorders are the second highest direct obstetric cause of maternal death after haemorrhage, accounting for 14% of maternal deaths globally. Pregnancy hypertension contributes to maternal deaths, particularly in low- and middle-income countries, due to a scarcity of doctors providing evidence-based emergency obstetric care. Task-sharing some obstetric responsibilities may help to reduce the mortality rates. This study was conducted to assess acceptability by the community and other healthcare providers, for task-sharing by community health workers (CHW) in the identifi

Tobacco Use, Experiences and Knowledge Among Indigenous Mexican Agricultural Workers.

The unique culture and languages of Indigenous Mexican immigrants to the United States, a substantial portion of the nation’s agricultural workforce, has limited participation of this population in tobacco and other health related research. Our established community-academic team [Mixteco Indigena Community Organizing Project (MICOP) and UCLA] developed and conducted a community survey (n = 200) from February – October 2024, in Spanish (n = 145), English (n = 10) or Mixteco (n = 45), aimed to understand tobacco use and knowledge among adults in this population. Participants (mean age 26 years;

Orchestral Musicians' Community Engagement as a Means for Flourishing: A Phenomenological Study.

Recent decades have witnessed a rapid expansion of community music projects, and findings have highlighted their potential to build important psychological resources such as positive emotions, a sense of accomplishment, engagement, purpose and social skills. Despite the collaborative nature of this type of work, the experiences of the musicians who facilitate it remain largely undocumented. This study emerged from two innovative initiatives that brought together primary school children and musicians from two of London's major orchestras (London Philharmonic Orchestra and London Contemporary Or

Mental Hospitals in India in the 21st century: transformation and relevance.

Mental hospitals in India, as elsewhere in the world, have played an important role in the care of persons with mental illness. Since their inception, they have both been decried for gross violations of human rights and dignity as well as lauded as places of refuge and care for persons turned away by the communities. In a country where community interventions for mental health care are still fragmentary, the mental hospital still continues to be a relevant and legitimate locus of care along with other limited resources available for the care of persons with mental illness outside of the family

Subinhibitory concentrations of antibiotics mediate primary metabolism and shape diverse species interactions among coevolved soil Streptomyces.

Antibiotics produced by soil microbes have provided critical advances in human health, yet their roles within natural soil ecosystems remain poorly understood. Though traditionally viewed as inhibitory weapons, antibiotics at subinhibitory concentrations (SICA) can alter transcription across a wide range of gene targets, with documented impacts on microbial metabolism and hypothesized consequences for community resource competition. However, work to date has focused primarily on SICA outside of the context of coevolved microbial populations. As interest in leveraging the power of microbiomes g

Healthcare providers' awareness, perceptions and experiences in integrating palliative care for women with HIV and cancer in Nigeria: a qualitative study on barriers and facilitators.

BACKGROUND: Women living with HIV face an increased risk of cancer due to compromised immune systems, necessitating integrated care approaches that include palliative care (PC). However, in resource-limited settings such as Nigeria, significant gaps in integrating PC into community healthcare persist, resulting in suboptimal outcomes for women managing HIV and cancer conditions. Understanding the barriers to and facilitators of PC integration from the perspective of healthcare providers is essential for improving care delivery. METHODS: A qualitative study was conducted across Nigeria’s six ge

Divergence of environmental filtering: rock exposure rate filters reproductive traits whereas slope shape community richness.

INTRODUCTION: Environmental filtering is a core mechanism driving plant functional trait reassembly and community succession. However, it is often treated as a uniform process, and whether distinct abiotic factors act through divergent mechanisms remains poorly understood. METHODS: We quantified reproductive traits (fruit types and seed dispersal modes), environmental gradients, and community attributes across plots in a typical resource-limited ecosystem characterized by intensified environmental filtering. Using an integrated framework that combined correlation screening, mediation analysis

Leveraging spatial structure to design spatially-targeted malaria interventions at the community-scale.

Progress in malaria control has stagnated since the early 21st century in many countries, requiring new approaches such as the use of spatially-targeted interventions. However, evidence on the effectiveness of spatially-targeted interventions is mixed, depending on both the setting and intervention. In addition, the potential role of community health programs in implementing spatial targeting is underexplored, with models focusing on coarser spatial scales than those needed to guide programmatic activities. We developed a two-age-class, spatially-explicit model of malaria at the community-scal

Cost-effectiveness of a community-based dementia support model: evidence from a real-world economic evaluation.

BACKGROUND: The prevalence of dementia is currently set to rise to 1.7 million by 2040, with associated costs estimated at £90 billion. Given its substantial impact on quality of life (QoL) and the growing societal and financial burden, identifying efficient approaches to dementia support is a key policy priority. Current policy direction emphasises a shift toward community-based models of care; however, economic evidence is required to determine whether such approaches can improve outcomes while representing cost-effective use of limited healthcare resources. This study presents a cost-effect
