Confusion after spine injury: cerebral fat embolism after traumatic rupture of a Tarlov cyst: case report.

BACKGROUND: Acute low back pain is a very common symptom and reason for many medical consultations. In some unusual circumstances it could be linked to a rare aetiology. CASE PRESENTATION: We report a 70-year-old man with an 8-month history of left posterior thigh and leg pain who had sudden confusion after a fall from standing. It was due to cerebral fat embolism suspected by computed tomography scan, later confirmed by brain magnetic resonance imaging (MRI). A spinal MRI scan was then performed and revealed a sacral fracture which drained into an unknown perineurial cyst (Tarlov cyst). Unde…

Partial atrioventricular septal defect causing confusion with rheumatic heart disease in children in Harare--case reports.

Four cases of partial atrioventricular septal defect (AVSD) [corrected] which had been confused with rheumatic heart disease are presented. The need for a full clinical and echocardiographic assessment to reduce this confusion and avoid potentially harmful therapy is highlighted in the report.

Detection and diagnosis of delirium in the elderly: psychiatrist diagnosis, confusion assessment method, or consensus diagnosis?

The clinical diagnosis of delirium has traditionally been based on an assessment by one or more physicians. Because of the transient, ubiquitous, and fluctuating nature of the symptoms of delirium, however, this approach may be flawed. Therefore, we decided to compare diagnosis based on one assessment by a psychiatrist, diagnosis by a nurse clinician (using the Confusion Assessment Method [CAM] and multiple observation points), and diagnosis by consensus. The study subjects were 87 patients aged 65 and over who were admitted consecutively from the emergency department to the medical wards, and…

Giant forms of Blastomyces dermatitidis in the pulmonary lesions of blastomycosis. Potential confusion with Coccidioides immitis.

Typical yeast-phase cells of Blastomyces dermatitidis have a characteristic appearance in tissue sections. Fungal morphologic variation occurs infrequently in the lesions of blastomycosis, yet it can complicate the differential diagnosis, particularly if fresh tissue is not available for microbiologic culture. The authors report a case of pulmonary blastomycosis, confirmed by culture and direct immunofluorescence, in which some of the yeast-like cells were abnormally large. These giant yeast-like cells exceeded the size range accepted for the tissue forms of B. dermatitidis; therefore, coccidi

Cognitive mechanisms of change in OCD: Inferential confusion, obsessive beliefs, and mindfulness across three randomized treatments.

Cognitive-behavioral therapy (CBT) with exposure and response prevention (ERP) is widely regarded as the gold-standard psychological treatment for obsessive-compulsive disorder (OCD). Inference-based cognitive behavioral therapy (I-CBT) represents a distinct cognitive approach that targets inferential confusion, defined as a reasoning process whereby imagined possibilities are treated as reflecting reality, without relying on deliberate exposure. The present study reports further mechanism-focused analyses from a previously published multicenter randomized controlled trial (Aardema et al., 202…

AdaptEEG: A Deep Subdomain Adaptation Network With Class Confusion Loss for Cross-Subject Mental Workload Classification.

EEG signals exhibit non-stationary characteristics, particularly across different subjects, which presents significant challenges in the pre … cation boundaries. We proposed a novel approach called deep subdomain adaptation network with class confusion loss (DSAN-CCL) to enhance the performance of cross-subject mental workload classification. DSAN-CCL utilizes the local maximum mean discrepancy to align the feature distributions between the source domain and the target domain for each mental workl …

Snack Confusion: Parents perceive baby, child and adult snacks as more similar than they actually are.

The consumption of commercial snacks for infants and toddlers has risen significantly, yet limited research has explored how parents evaluate these products across age groups. This study examined parental perceptions of snacks marketed for babies (<36 months), children (commercial snacks with child-appealing packaging), and adults, focusing on attributes such as healthiness, appropriateness, and parental willingness to buy. It also assessed the relationship between parents' knowledge of the European Union (EU) baby food regulations and their purchasing intentions. An online survey was conducte

Differences in consonant confusion patterns between bimodal cochlear implant users with greater and less bimodal advantage.

OBJECTIVES: The variability in bimodal advantage among bimodal users remains a significant challenge in clinical approaches. This study compared confusion patterns in consonant recognition between bimodal users who received greater bimodal advantage (GBA) and those who received less bimodal advantage (LBA), aiming to clarify these patterns in order to identify underlying causes that can help improve speech perception for all users. METHODS: Confusion matrices were measured monaurally and binaurally in both quiet and noisy conditions. Twenty-one subjects were divided into GBA (n = 8) and LBA (…

Improved Side-Effect Stimulation Thresholds and Postoperative Transient Confusion With Asleep, Image-Guided Deep Brain Stimulation.

BACKGROUND AND OBJECTIVES: Asleep, image-guided deep brain stimulation (DBS) is a modern alternative to awake, microelectrode recording (MER) guidance. Studies demonstrate comparable efficacy and complications between techniques, although some report lower stimulation thresholds for side effects with image guidance. In addition, few studies directly compare the risk of postoperative transient confusion (pTC) across techniques. The purpose of this study was to compare clinical efficacy, stimulation thresholds for side effects, and rates of pTC with MER-guided DBS vs intraoperative 3D-fluoroscop…

An Unusual Case of Acute Confusion in an Elderly Man: Pituitary Apoplexy With Lateral Rectus Palsy.

Often, the provisional diagnosis for an elderly patient who arrives at the hospital with confusion is presumed to be delirium stemming from confusion usually caused by an infectious cause. The famous mnemonic PINCH ME signifies the ruling out of pain, infection (that usually has a urinary cause), constipation, dehydration, medication (particularly narcotics), and the environment (factors triggering confusion in a patient with a background of dementia). However, we report a rare case of sudden confusion in an elderly male with no previous history of cognitive impairment. This is the first ever …

[Chronic granulomatous villitis due to cytomegalovirus. An unusual finding that may cause confusion. A case report].

Chronic cytomegalovirus (CMV) villitis typically causes inflammation with predominance of plasma cells. The granulomatous reaction in the chorionic villi is usually caused by pathogens other than CMV, such as toxoplasma or rubella. We present a case of a pregnant woman presenting with foetal death in the twentieth week of gestation. The study of the placenta revealed chronic CMV villitis with a granulomatous reaction, rather than the more common plasma cell inflammation.

Cross sectional analysis of scoliosis-specific information on the internet: potential for patient confusion and misinformation.

BACKGROUND CONTEXT: Patients and their families are increasingly turning to the internet for medical information. Most of these patients believe the information to be accurate and reliable. However, the quality and accuracy of that information on the internet is variable and unregulated. Accurate and applicable information may align patients' expectations and improve satisfaction and overall outcomes. PURPOSE: This study aimed to evaluate the quality and accuracy of scoliosis-related information available on the internet. STUDY DESIGN: Cross-sectional analysis. METHODS: Independent searches

Abnormal visual representations associated with confusion of perceived facial expression in schizophrenia with social anxiety disorder.

Deficits in social functioning are especially severe amongst schizophrenia individuals with the prevalent comorbidity of social anxiety disorder (SZ&SAD). Yet, the mechanisms underlying the recognition of facial expression of emotions-a hallmark of social cognition-are practically unexplored in SZ&SAD. Here, we aim to reveal the visual representations SZ&SAD (n = 16) and controls (n = 14) rely on for facial expression recognition. We ran a total of 30,000 trials of a facial expression categorization task with Bubbles, a data-driven technique. Results showed that SZ&SAD's ability to categorize

[Confusion, loss of consciousness and unspecific visual hallucinations may be symptoms of dementia with Lewy bodies].

Dementia with Lewy bodies (DLB) is a common type of dementia. The core clinical symptoms are visual hallucinations, fluctuating cognition with pronounced variations in attention, REM sleep behaviour disorder and parkinsonism. Early diagnosis and differentiation from Alzheimer's disease can be challenging, especially when the core clinical symptoms are modest or only partly present. In this case report, we describe two patients, who had unexplained loss of consciousness and visual hallucinations, which were the early signs of DLB in absence of parkinsonism.
