The role of direct oral anticoagulants for prevention of venous thromboembolism (VTE) in bariatric surgery has been unclear. Current clinical guidelines recommend against using direct oral anticoagulants in bariatric surgery but lack the inclusion of recent prospective trials. This article reviews data on the pharmacokinetics, pharmacodynamics, efficacy, and safety of rivaroxaban or apixaban prophylaxis in this setting. Targeted literature reviews identified prospective studies of rivaroxaban (n = 4) or apixaban (n = 2) and retrospective studies (n = 7) on VTE prophylaxis in bariatric surgery.
