Coronary artery disease in aortic stenosis: importance of coronary arteriography and surgical implications.

In our report, the presence or absence of angina pectoris did not predict the presence of coronary artery disease. A significant number of patients with aortic stenosis and angina pectoris have coronary artery disease but coronary artery disease also exists in asymptomatic form in a significant number of patients with severe aortic stenosis that could not be detected clinically and therefore suggests that the routine use of selective coronary arteriography is indicated in patients over 40 years undergoing cardiac catheterization because of aortic stenosis. This is very important in the preoper

Q-Waves Associated With Postinfarct Chronic Total Occlusion Arteries Predict Non-Viable Myocardium Even in the Presence of Collaterals.

BACKGROUND AND AIM: The presence of grade 3 collaterals is known to be associated with viability in the presence of a chronic total occlusion (CTO). However, it is not clear whether this holds true even in patients who had ST-segment elevation myocardial infarction (STEMI). The aim of our study was to look at the viability of myocardium in patients with CTOs in both infarct-related and non-infarct related occluded vessels with grade 3 collaterals. METHODS: We prospectively collected consecutive patients with CTOs who had good grade 3 collaterals and studied the viability of the myocardium in

Transcoronary chemical ablation of ventricular tachycardia.

After identification of the artery supplying blood to the arrhythmogenic area, transcoronary chemical ablation of ventricular tachycardia was undertaken in three patients with incessant tachycardia in whom the other therapeutic options had failed. Sterile ethanol (96%) was given at a dose of 1.5 ml in two patients and a total of 6 ml in the third. The arrhythmia was cured in two patients and suppressed during a 1-month period in the third until new collateral blood supply to the arrhythmogenic area developed and ventricular tachycardia recurred. The procedure was then repeated successfully. Af

Coronary-artery spasm immediately after myocardial revascularization: recognition and management.

We investigated coronary-artery spasm in six patents who had had unexpected hemodynamic collapse within two hours after cardiopulmonary bypass for myocardial revascularization. All six had profound hypotension and recurrent ST-segment elevation in electrocardiographic Leads II, III, and aVF. All had either normal or noncritical luminal irregularities of dominant right coronary arteries and more than 75 per cent occlusions in the left coronary circulation. Right-coronary-artery spasm, which was reversed after intracoronary nitroglycerin, was demonstrated angiographically in one patient; a paten…

Anomalous Left Coronary Artery from the Pulmonary Artery: Cinematic Volume Rendering Technique for Enhanced Anatomic Visualization.

Anomalous left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital anomaly with exceptional survival into adulthood. We present a 66-year-old woman with chest and back pain in whom ALCAPA was diagnosed using coronary computed tomography angiography (CCTA) with curved planar reformation and cinematic volume rendering technique (cVRT). Photorealistic three-dimensional reconstruction provided complementary three-dimensional visualization that may facilitate anatomic understanding and communication of the anomalous origin. Conservative management was adopted given the patient's

Cardiogenic Shock After Arterial Y-Graft Coronary Bypass Surgery Secondary to Critical Stenoses of the Left Subclavian and Left Main Coronary Arteries.

We present a case of a 62-year-old man who was in cardiogenic shock. He had a history of coronary artery bypass grafting 4 years previously, with left internal mammary radial artery Y-grafting to a left dominant coronary circulation. Critical stenoses of the left main coronary and left subclavian arteries were seen at angiography. An occluded abdominal aorta precluded the use of mechanical circulatory support. The patient underwent high-risk stenting of the left subclavian artery with a successful outcome. The case highlights the unresolved issue of screening for subclavian stenoses in patient…

A different kind of Christmas tree: anomalous origin of the right coronary artery from the pulmonary artery (ARCAPA).

Anomalous right coronary artery from the pulmonary artery (ARCAPA) is a rare congenital coronary anomaly that has an incidence of 0.002%. We report a case of a previously healthy female who presented to our hospital with pneumonia and was incidentally discovered to have ARCAPA. This was initially diagnosed on echocardiography by the unusual echocardiographic finding of multiple color flow Doppler signals around the right ventricular free wall and apex which were subsequently confirmed by angiography to be due to extensive collateral circulation between the left and right coronary arteries. Thi

Coil reduction of the right ventricular cavity in a boy with pulmonary atresia, intact ventricular septum, and an aneurysmal coronary sinusoid.

Coronary sinusoids are often found in patients with pulmonary atresia and intact ventricular septum. We report on a 4-year-old boy with such pathology in whom, after completion of the Fontan-circulation, the pulsatile flow through a major sinusoid significantly increased the size of a coronary artery up to aneurysmal dimensions. Because percutaneous closure of the sinusoid was unsuccessful, the right ventricular cavity was partially obliterated with coils. This resulted in a significant decrease of right ventricular stroke volume, regression of the pulsatility and of the coronary size, with go

Coronary artery bypass grafting in a patient with brainstem ischemia.

We describe a 54-year-old male with severe coronary artery disease and cerebrovascular disease including right cerebellar infarction, total occlusion of the bilateral vertebral arteries, brainstem ischemia, and right cerebral infarction with significant right carotid artery disease. Repeated percutaneous transluminal coronary angioplasty had been performed, however, unstable angina was developed despite maximal medical treatment. Coronary artery bypass grafting was successfully undergone with use of propofol, application of the intra-aortic balloon pumping perioperatively, and mild hypothermic

Anomalous origin of right coronary artery originating from the proximal part of the left anterior descending artery via a collateral.

The incidence of anomalous coronary artery is approximatly 1% in patients who undergo coronary angiography. These anomalies, together with congenital heart disease, may cause myocardial ischemia, decreased life expectancy and sudden death. The case of a patient with a rare anomalous right coronary artery (RCA) from the very proximal part of the left anterior descending (LAD) artery via a collateral, aberrant vessel is described.

Effect of adenosine on coronary blood flow and its use as a diagnostic test for coronary artery disease.

Myocardial perfusion imaging during adenosine induced coronary hyperaemia is a highly sensitive method for diagnosing coronary artery disease. Most perfusion defects reflect heterogeneity in coronary blood flow in the territories of normal and diseased coronary arteries. Myocardial ischaemia can occur, however, due to subendocardial hypoperfusion and coronary artery steal. Evidence for myocardial ischaemia is more difficult to document in clinical than in experimental studies, and includes abnormalities in the ST segment, wall motion, haemodynamic variables, metabolism, and coronary blood flow

Subarachnoid hemorrhage presenting as cardiac arrest mimicking acute myocardial infarction due to multivessel coronary vasospasm: a case report.

A 58-year-old man presented with out-of-hospital cardiac arrest. The initial rhythm was pulseless electrical activity, which deteriorated into ventricular fibrillation during resuscitation. After return of spontaneous circulation, electrocardiography demonstrated diffuse ST-segment depression with ST-segment elevation in lead aVR, strongly suggesting acute coronary syndrome. Emergent coronary angiography revealed simultaneous angiographic occlusion with TIMI 0 flow in the ostial left anterior descending artery and distal right coronary artery. During preparation for percutaneous coronary inter

The authors report a case of fistulae between the right and left coronary arteries and the left ventricle. A 58 year old man was investigate … he diagnosis of coronary-left ventricular fistulae. The dilatation and rapid emptying of the entire coronary circulation were indirect signs of increased flow due to the shunt. There was no coronary alternation or stenosis. The rarity of coronaro-cardiac fistulae, especially involving both main coronary arteries and the left ventricle, is emphasised. The symptoms of a …
