Evaluation of cortical bone thickness at the nasomaxillary and zygomaticomaxillary buttresses using cone-beam computed tomography imaging for Le Fort I osteotomy.

BACKGROUND/PURPOSE: Studies have indicated that 50%-55% of the population have malocclusion, and approximately 5%-10% require orthognathic s … re input into medical imaging software to simulate the placement of titanium screws and plates. The cortical bone thickness at these positions was measured to assess the thickness in the nasomaxillary buttress (surrounding the nasal opening) and the zygomaticomaxillary buttress (surrounding the maxillary zygomatic process). Associations of these thicknesses wit …

Comparison of facet fusion rates and clinical outcomes between cortical bone trajectory screw and percutaneous pedicle screw fixation for degenerative lumbar spondylolisthesis.

BACKGROUND CONTEXT: Cortical bone trajectory (CBT) screws have been introduced as an alternative technique for pedicle screw (PS) insertion because they have greater contact with the cortex and a greater uniaxial pullout load than traditional PS. CBT screwing can also minimize muscle dissection. However, CBT screws and traditional PSs have not yet been compared in terms of fusion rates and clinical outcomes for particular operative procedures. PURPOSE: This study aimed to assess the fusion rate and clinical outcomes of facet fusion (FF) fixed with CBT screws (CBT-FF) and to compare them with …

Freehand screw insertion technique without image guidance for the cortical bone trajectory screw in posterior lumbar interbody fusion: what affects screw misplacement?

OBJECTIVE: Cortical bone trajectory (CBT) screw insertion using a freehand technique is considered less feasible than guided techniques, due to the lack of readily identifiable visual landmarks. However, in posterior lumbar interbody fusion (PLIF), after resection of the posterior anatomy, the pedicles themselves, into which implantation is performed, are palpable from the spinal canal and neural foramen. With the help of pedicle wall probing, the authors have placed CBT screws using a freehand technique without image guidance in PLIF. This technique has advantages of no radiation exposure and…

Biomechanical analysis and optimization of screw fixation technique for the cortical bone channel of lower thorax: Study protocol clinical trial (SPIRIT Compliant).

It is well known that the main segments of spinal fracture is thoracolumbar (T11-L11). Therefore, in addition to the lumbar, the lower thoracic vertebra (T9-T12) often has the clinical needs of implantation of cortical bone trajectory (CBT) screws. However, the anatomic parameters of the lower thoracic vertebrae are quite different from those of the lumbar vertebrae, which means that if CBT screws are to be implanted in the lower thoracic vertebrae, the selection of the screw entry point, the length, diameter, angle and path of the screws in each segment need to be redefined. Methods In this p…

Effects of inversion time on inversion recovery prepared ultrashort echo time (IR-UTE) imaging of bound and pore water in cortical bone.

Water is present in cortical bone in different binding states. In this study we aimed to investigate the effects of inversion time (TI) on the signal from bound and pore water in cortical bone using an adiabatic inversion recovery prepared ultrashort echo time (IR-UTE) sequence on a clinical 3 T scanner. In total ten bovine midshaft samples and four human tibial midshaft samples were harvested for this study. Each cortical sample was imaged with the UTE and IR-UTE sequences with a TR of 300 ms and a series of TI values ranging from 10 to 240 ms. Five healthy volunteers were also imaged with th…

Effects of testosterone replacement therapy on cortical and trabecular bone mineral density, vertebral body area and paraspinal muscle area in hypogonadal men.

Loss of bone and muscle mass are major findings of male hypogonadism. In order to determine the long-term effect of testosterone replacement therapy on spinal bone and muscles, the trabecular and cortical bone mineral density, vertebral body area and paraspinal muscle area were assessed by quantitative computed tomography in 32 testosterone-substituted patients, aged 18-74 years, with idiopathic hypogonadotropic hypogonadism (n=6), pituitary insufficiency (n=5), Klinefelter syndrome (n=12) or other forms of primary hypogonadism (n=9). They were followed for a mean period of 3.2+/-1.7 years (me…

Effect of protein supplementation on hip bone mineral density, cortical thickness, and bone strength in older adult participants during a caloric restriction and aerobic exercise weight loss intervention: a randomized controlled trial.

UNLABELLED: Weight loss in older age can cause bone loss. In older adults with overweight/obesity in a weight loss trial, 6-month hip bone strength increased with higher protein intake versus controls consuming the Recommended Dietary Allowance. However, greater weight loss was associated with greater 18-month hip bone mineral density loss. PURPOSE: Weight loss (WL) to treat obesity in older age can exacerbate bone loss. METHODS: This trial assessed the effects of higher protein intake on hip bone outcomes in 187 older adults with overweight/obesity participating in 6 months of active WL (ca

Comparison of Outcomes Between a Fixed-Loop Cortical Suspensory Device and Metal Interference Screw for Femoral Fixation of Bone-Patellar Tendon-Bone Grafts in Anterior Cruciate Ligament Reconstruction: A Scandinavian Registry Study.

BACKGROUND: Use of a cortical suspensory device (CSD) is an alternative fixation method for bone-patellar tendon-bone (BPTB) in the femur. To the authors' knowledge, no study has compared revision outcomes and patient-reported outcomes (PROs) with the traditional interference screw (IS) fixation. PURPOSE: To compare the survival rate of grafts fixated on the femoral side with either a fixed-loop CSD or IS for primary anterior cruciate ligament reconstructions (ACLRs) with BPTB autograft 2 and 5 years after index surgery, as well as to compare PROs between the same 2 treatment groups based on

Arthrodesis and Defect Bridging of the Upper Ankle Joint with Allograft Bone Chips and Allograft Cortical Bone Screws (Shark Screw®) after Removal of the Salto-Prosthesis in a Multimorbidity Patient: A Case Report.

The case describes the revision of an upper ankle prosthesis because of loosening. When ankle replacement is the first choice and actual bone quality does not allow a replacement of the prosthesis, arthrodesis is the only way of reducing pain and gaining stability. The amount of missing bone due to the removed prosthesis was severe. Shark Screws®, made of human allograft cortical bone, were used to fix an allograft femoral head and tibia as well as fibula and talus to each other for stabilization. This was performed without any autologous bone graft and without metal screws. The human matrix o…

Romosozumab significantly improves vertebral cortical bone mass and structure compared with teriparatide, whereas both treatments increase vertebral trabecular bone mass similarly: high-resolution quantitative computed tomography analyses of randomized controlled trial results in postmenopausal women with low bone mineral density.

Romosozumab, a sclerostin inhibitor, exerts a dual effect of increasing bone formation while decreasing bone resorption to rapidly increase bone mineral density (BMD) and reduce fracture risk in postmenopausal women. Iterative Convolution OptimizatioN (ICON) allows accurate calculation of deconvolved cortical thickness from high-resolution quantitative computed tomography (HR-QCT) scanning. This study employed HR-QCT to evaluate compartmental (including endosteal and periosteal) changes in the vertebral cortical shell in postmenopausal women who received romosozumab, teriparatide, or placebo.

Evaluation of bone density and microstructure by DXA, TBS and HR-pQCT: an assessment of cortical porosity and its association with vertebral fractures in patients with acromegaly.

PURPOSE: Patients with acromegaly are at increased risk of fractures, regardless of areal bone mineral density (BMD). This study aimed to identify whether vertebral fractures (VFs) are related to decreases in bone volumetric density and microstructure via high-resolution peripheral quantitative computed tomography (HR-pQCT) and the trabecular bone score (TBS). METHODS: VF was evaluated in 49 patients with acromegaly (22 men and 27 women) via spine X-ray. Areal BMD, volumetric BMD, and bone microstructure were analyzed via dual-energy X-ray absorptiometry (DXA) and HR-pQCT. Lumbar spine trabec

An integrated experimental-computational framework to assess the influence of microstructure and material properties on fracture toughness in clinical specimens of human femoral cortical bone.

Microstructural and compositional changes that occur due to aging, pathological conditions, or pharmacological treatments alter cortical bone fracture resistance. However, the relative importance of these changes to the fracture resistance of cortical bone has not been quantified in detail. In this technical note, we developed an integrated experimental-computational framework utilizing human femoral cortical bone biopsies to advance the understanding of how fracture resistance of cortical bone is modulated due to modifications in its microstructure and material properties. Four human biopsy s…

Immediate Weight Bearing of Plated Both-Bone Forearm Fractures Using Eight Cortices Proximal and Distal to the Fracture in the Polytrauma Patient Is Safe.

INTRODUCTION: Rehabilitation of trauma patients is facilitated by surgical stabilization permitting weight bearing (WB) of the fractured extremity. Both-bone forearm fracture (BBFx) plate osteosynthesis is an accepted technique with high union and low complication rates; yet, postoperative WB protocols have not been adequately investigated. There exists concern for increased complications in plated BBFx fractures for patients prescribed immediate WB. We hypothesized that immediate WB of surgically treated BBFxs results in acceptable rates of complications. METHODS: Patients presenting to a Le

Stressed volume estimated by finite element analysis predicts the fatigue life of human cortical bone: The role of vascular canals as stress concentrators.

The fatigue life of cortical bone can vary several orders of magnitude, even in identical loading conditions. A portion of this variability is likely related to intracortical microarchitecture and the role of vascular canals as stress concentrators. The size, spatial distribution, and density of canals determine the peak magnitude and volume of stress concentrations. This study utilized a combination of experimental fatigue testing and image-based finite element (FE) analysis to establish the relationship between the stressed volume (i.e., volume of bone above yield stress) associated with vas…
