[The Freeman-Sheldon syndrome].

The authors report two cases of Freeman-Sheldon syndrome, characterised by recessive autosomic transmission, affecting a brother and a sister born of parents who were not blood relations. The syndrome, for which 50 cases are described in the literature, takes its name from the authors who were the first to describe it in 1938. The typical picture is based on congenital deformity involving the cranium, the face and the locomotor apparatus, with a particular aspect of the subject affected related to his or her expression, resembling that of someone whistling ("whistling face", Burian, 1963). In

[Freeman-sheldon's syndrome (cranio-carpo-tarsal dysplasia) (author's transl)].

A case with typical features of Freeman-Sheldon's syndrome is reported. The proband, a five years old girl had not any other affected members in her family, and showed every other associated malformations previously described: soft tissue swelling of supraorbital area, deep set eyes, epicantus, convergent strabismus, small nose, long philtrum area, H-Defect of chin, craneal alterations, kyphoscoliosis, finger contractures, thickening of the skin over the dorsum of the fingers, talipes equinovarus, etc. A cylindrical morphology of the three last metacarpals of both hands is remarkable.

Axenfeld-Rieger Syndrome with Negative Chromosomal Microarray and Whole-Exome Sequencing: A Case Report.

Axenfeld-Rieger Syndrome (ARS) is a rare genetic disorder defined by ophthalmological abnormalities, frequently accompanied by craniofacial and dental anomalies. Although mutations in the PITX2 and FOXC1 genes are commonly identified, some patients do not receive molecular confirmation. This report describes a clinically compelling case of ARS with a typical phenotype but negative genetic testing. A 10-year-old male demonstrated physical abnormalities from early childhood and underwent multiple medical evaluations without a definitive diagnosis. He experienced difficulty and discomfort during

Clinical and Neurodevelopmental Course in a Case of EFNB1-Related Craniofrontonasal Syndrome With Unrepaired Craniosynostosis.

INTRODUCTION: Craniofrontonasal syndrome displays an unusual X-linked dominant inheritance pattern due to pathogenic variants in EFNB1, which encodes a membrane-bound ligand for the ephrin receptor. Females exhibit a more severe phenotype than males and can have craniosynostosis, hypertelorism, craniofacial asymmetry, thoracic skeleton abnormalities, and an assortment of other features. Males typically only develop a milder hypertelorism, or they have no discernable features at all. METHODS: Exome sequencing was used to identify a likely pathogenic EFNB1 c.129-2A>G splice site variant in an i

Specialty-Specific Citation-Enabled AI Clinical Decision Support System for Craniofacial Surgery: Development of CASPER.

BACKGROUND: Craniofacial surgery requires synthesis of complex, multidisciplinary knowledge, yet specialty-specific decision support tools are lacking. Retrieval-augmented generation (RAG) offers an opportunity to create transparent, evidence-based artificial intelligence (AI) systems tailored to surgical practice. METHODS: The authors developed CASPER, a domain-specific, multimodal RAG system with text and image analysis capabilities, built with RAPTOR hierarchical architecture and a knowledge base of 8561 open-access craniofacial surgery articles (2000-2025). The system retrieved and synthe

Modeling the long-range effect of an inversion downstream of EFNB1 concludes a 43-year molecular diagnostic odyssey for craniofrontonasal syndrome.

Craniofrontonasal syndrome (CFNS; MIM #304110) is a rare craniofacial disorder characterized by hypertelorism, a broad nasal root with a bifid nasal tip, orofacial clefting, and genital malformations caused by pathogenic variants in the X-linked gene EFNB1 (MIM *300035). CFNS exhibits sex-specific heterogeneity, with increased severity in females likely secondary to cellular interference related to random X-inactivation, resulting in mosaic EFNB1 expression. Previous studies have identified over 140 variants in EFNB1, but approximately 20% of CFNS have negative molecular testing, either due to

RNA Polymerase I Dysfunction Underlying Craniofacial Syndromes: Integrated Genetic Analysis Reveals Parallels to 22q11.2 Deletion Syndrome.

BACKGROUND/OBJECTIVE: POLR1A and related gene variants cause craniofacial and developmental syndromes, including Acrofacial Dysostosis-Cincinnati, Treacher-Collins types 2-4, and TWIST1-associated disorders. Using a patient case integrated with molecular analyses, we aimed to clarify shared pathogenic mechanisms and propose these conditions as part of a spectrum of RNA polymerase I (Pol I)-related ribosomopathies. METHODS: A patient with a heterozygous POLR1A variant underwent clinical evaluation. Findings were integrated with a literature review of craniofacial syndromes to identify overlapp

The Sesquialter (First and a Halfth) Branchial Cleft Anomaly.

A 6-month-old presented with a sinus in the neck coursing from below the body of the mandible, between the carotid and parotid spaces, and terminating posterior to the stylomastoid foramen on MRI. Facial nerve monitoring was used, a lacrimal probe was placed in the sinus, an elliptical incision was made in the skin, and the tract was followed to the parapharyngeal space. Pathology demonstrated a keratinous squamous epithelial lining, skin adnexae, and elastic cartilage. Because there were clinical, radiographic, and pathological features intermediate to first and second branchial anomalies, we

Systemic Factors Affecting Orthodontic Treatment Outcomes and Prognosis-Part 2.

This article explores the various challenges systemic conditions can pose before and during orthodontic treatment. Cardiovascular conditions like infective endocarditis require antibiotic prophylaxis before certain orthodontic procedures are started. Patients with bleeding disorders require special considerations in regards to viral infection risk and maintenance of excellent atraumatic oral hygiene. Orthodontists play an important role in early identification of signs and symptoms of eating disorders and should deal with these patients sensitively. Congenital disorders, craniofacial anomalies
