Management of stage III pressure ulcers in moderately demented nursing home residents.

PURPOSE: To determine whether the myocutaneous flap procedure, the preferred treatment for young patients with stage III pressure ulcers, is appropriate for moderately demented nursing home residents. SUBJECTS AND METHODS: A model of the treatment decision was created using the decision-tree format. The probabilities of treatment outcomes in the decision tree were derived from the literature. Utilities for treatment outcomes were obtained from five internists, five geriatricians, six nurse practitioners, five plastic surgeons, and six lay people. RESULTS: A majority in each group of responde

The interplay of coronary serial stenoses and their deceptive impact on hemodynamic and surgical planning.

BACKGROUND: Assessing serial stenoses in coronary artery disease (CAD) remains a difficult task, as hemodynamic interactions between lesions can mask individual severity. In this work, we present an automated and non-invasive decision tree-based framework that integrates computational fluid dynamics (CFD)-derived indices with CAD-RADS classification to guide revascularization strategies in cases of serial stenoses. METHODS: A dataset of 400 virtual coronary artery models was generated from 20 patients' anatomies, augmented with serial stenosis severities (CAD-RADS system) and boundary conditi

Prediction of patient's neurological recovery from cervical spinal cord injury through XGBoost learning approach.

Источник пометил публикацию как отозванную

Due to the diversity of patient characteristics, therapeutic approaches, and radiological findings, it can be challenging to predict outcomes based on neurological consequences accurately within cervical spinal cord injury (SCI) entities and based on machine learning (ML) technique. Accurate neurological outcomes prediction in the patients suffering with cervical spinal cord injury is challenging due to heterogeneity existing in patient characteristics and treatment strategies. Machine learning algorithms are proven technology for achieving greater prediction outcomes. Thus, the research emplo

A machine learning framework to adjust for learning effects in medical device safety evaluation.

OBJECTIVES: Traditional methods for medical device post-market surveillance often fail to accurately account for operator learning effects, leading to biased assessments of device safety. These methods struggle with non-linearity, complex learning curves, and time-varying covariates, such as physician experience. To address these limitations, we sought to develop a machine learning (ML) framework to detect and adjust for operator learning effects. MATERIALS AND METHODS: A gradient-boosted decision tree ML method was used to analyze synthetic datasets that replicate the complexity of clinical

Quantitative assessment of Public Health and Social Measures implementation and relaxation on influenza transmission during COVID-19 in China: SEIABR and GBDT models.

BACKGROUND: Since 2019, China has implemented Public Health and Social Measures (PHSMs) to manage the coronavirus disease 2019 (COVID-19) outbreak. As the threat from SARS-CoV-2 diminished, these measures were relaxed, leading to increased respiratory infections and strained health care resources by mid-2023. METHODS: The study utilised WHO's FluNet and Oxford's COVID-19 Government Response Tracker to assess how policy shifts have affected influenza. It examined changes in influenza incidence, subtype prevalence, and epidemic cycles over three periods: pre-COVID-19 and pre-PHSMs, during COVID

Behavioral red flags for optimizing castration-induced acute pain diagnosis in cattle.

Unesp-Botucatu Cattle Pain Scale (UCAPS) is widely used in experimental settings, however the high number of UCAPS behaviors might represent a barrier to its implementation in the farm's or hospital's routine. We aimed to identify a smaller combination of UCAPS behaviors that could be used as behavioral red flags for optimizing the acute pain diagnosis in cattle. We hypothesize that a specific set of UCAPS behaviors might be used as behavioral red flags for pain. This would represent a quick and simple pain evaluation and might optimize the acute pain assessment in large-scale systems. Data fr

Enhanced heart sound classification using Mel frequency cepstral coefficients and comparative analysis of single vs. ensemble classifier strategies.

This paper seeks to enhance the performance of Mel Frequency Cepstral Coefficients (MFCCs) for detecting abnormal heart sounds. Heart sounds are first pre-processed to remove noise and then segmented into S1, systole, S2, and diastole intervals, with thirteen MFCCs estimated from each segment, yielding 52 MFCCs per beat. Finally, MFCCs are used for heart sound classification. For that purpose, a single classifier and an innovative ensemble classifier strategy are presented and compared. In the single classifier strategy, the MFCCs from nine consecutive beats are averaged to classify heart soun

A hybrid sampling algorithm combining synthetic minority over-sampling technique and edited nearest neighbor for missed abortion diagnosis.

BACKGROUND: Clinical diagnosis based on machine learning usually uses case samples as training samples, and uses machine learning to construct disease prediction models characterized by descriptive texts of clinical manifestations. However, the problem of sample imbalance often exists in the medical field, which leads to a decrease in classification performance of the machine learning. METHODS: To solve the problem of sample imbalance in medical dataset, we propose a hybrid sampling algorithm combining synthetic minority over-sampling technique (SMOTE) and edited nearest neighbor (ENN). Lifetime cardiovascular management of patients with previous Kawasaki disease.

Kawasaki disease (KD) is an inflammatory disorder of young children, associated with vasculitis of the coronary arteries with subsequent aneurysm formation in up to one-third of untreated patients. Those who develop aneurysms are at life-long risk of coronary thrombosis or the development of stenotic lesions, which may lead to myocardial ischaemia, infarction or death. The incidence of KD is increasing worldwide, and in more economically developed countries, KD is now the most common cause of acquired heart disease in children. However, many clinicians in the UK are unaware of the disorder and
