Prevalence and morphology of hand eczema in patients with atopic dermatitis.

BACKGROUND: Patients with hand eczema frequently have a history of atopic dermatitis or atopy. No specific morphologic pattern of hand eczema helps distinguish atopic hand eczema from other etiologies. There are few studies of hand eczema prevalence and morphology in a well-defined population of patients with atopic dermatitis. METHODS: We evaluated 777 consecutive patients with atopic dermatitis (diagnosed by standard criteria) for hand involvement. An additional 100 patients had further evaluations, including evaluation of the historical and morphologic characteristics of their hand eczema.

[Safety of topical tacrolimus and pimecrolimus in children with atopic dermatitis].

The use of topical immunomodulators in pediatric patients is an important topic in the clinical practice. Their prescription for chronic diseases suggests the necessity of evaluating their efficacy and safety profile in a long term period. In children they can develop systemic adverse events after their application, though sometimes they are useful to reduce the long consumption of other drugs, as topical steroids, or to have influence in the critical aspects of immunomodulation. Pimecrolimus and tacrolimus are two topical calcineurin inhibitors, from which there are several reports that suppo

The price of pruritus: sleep disturbance and cosleeping in atopic dermatitis.

BACKGROUND: Atopic dermatitis is a common skin disorder that most often begins in infancy. Sleep disturbances in children with atopic dermatitis are likely due to itching and scratching and not only impact the afflicted child but may also affect the entire family. Sleep characteristics in young children with atopic dermatitis and their families have not been thoroughly investigated. OBJECTIVE: To evaluate sleep disturbance and cosleeping in young children with atopic dermatitis and evaluate the association between sleep characteristics and features of the disease. DESIGN AND METHODS: Parents

Food sensitization in infants and young children with atopic dermatitis.

Atopic dermatitis (AD) is a chronic, relapsing, inflammatory skin disease. Children with AD tend to have a higher prevalence of food allergies. This study investigated the clinical significance of food sensitization in AD patients. A total of 266 AD patients participated in this study. The prevalence of food sensitization and clinically relevant sensitization were compared in the subjects according to their age and AD severity. Sera from all patients were analyzed for food-specific IgE levels using the Pharmacia CAP System FEIA. The serum specific IgE levels for egg, milk, peanut and soybean w

[Research on infants with atopic dermatitis caused by food allergies].

We have examined 370 infants less than 12 months of age with atopic dermatitis about food allergies, especially by egg white antigens. Since the results of the skin scratch test and serum specific IgE antibody test often correspond to each other, only the serum specific IgE antibody test was given to infants over 6 months of age. An oral provocation test using boiled egg white was given to 176 patients, and 52 of them showed immediate positive reaction. The number of patients with a positive reaction was much fewer than expected. This may be due to lowered levels of egg white antigens from bei

Intravenous immunoglobulin therapy for the treatment of severe atopic dermatitis.

Intravenous immunoglobulin (IVIg) is standard therapy for primary immunodeficiencies, Kawasaki Disease and idiopathic thrombocytopenic purpura. More recently, the use of high dose IVIg (2 g/kg in divided doses) has widened to include a number of inflammatory diseases, including atopic eczema. The mechanism of IVIg's anti-inflammatory action has yet to be fully understood. Proposed mechanisms include modulation of IgE responses and a reduction in inflammatory cytokines with a reduction in T-cell proliferation. Antibacterial and antitoxin effects may also play a role.

Adherence by Staphylococcus intermedius to canine keratinocytes in atopic dermatitis.

The adherence of Staphylococcus intermedius to canine keratinocytes in normal dogs was compared to that in dogs suffering from atopic dermatitis, primary seborrhoea and bacterial pyoderma. Statistically significant greater adherence by S. intermedius to keratinocytes occurred in atopic dogs and dogs suffering from pyoderma when compared with the normal group (P < 0.01) and dogs suffering from primary seborrhoea (P < 0.05). This is similar to the results of a study of human atopic dermatitis by Cole and Silverberg (1986) who demonstrated increased adherence by S. aureus to keratinocytes from at

Diagnostic tests in children with atopic dermatitis and food allergy.

BACKGROUND: Skin testing is a common diagnostic procedure in food allergy. The skin prick test is the test of first choice for investigating the immediate IgE-mediated reaction. The skin application food test (SAFT) has been developed on the basis of the mechanism of the contact urticaria syndrome (CUS). METHODS: We studied the relevance of the SAFT in children younger than 4 years with atopic dermatitis and (suspected) food allergy as compared with the prick-prick test, the radioallergosorbent test (RAST), and the oral challenge. In the skin tests, we used fresh food, in the same state as it

Characterization of the cutaneous inflammatory infiltrate in canine atopic dermatitis.

Sections from lesional atopic, clinically normal atopic, and normal canine skin were investigated by light microscopy and an immunoperoxidase method using monoclonal antibodies specific for canine leukocyte antigens. We confirmed that skin-infiltrating cells of canine atopic dermatitis are constituted of mast cells, dendritic antigen-presenting cells, memory helper T-lymphocytes, low numbers of eosinophils and neutrophils, and rare B-lymphocytes. The presence of epidermal eosinophil microaggregates and clustered Langerhans' cells supports the hypothesis of epidermal allergen contact. The hyper

Induction of atopic dermatitis by inhalation of house dust mite.

BACKGROUND: The pathogenetic role of house dust mite in atopic dermatitis remains controversial. Recent studies have shown that intensive epicutaneous contact of house dust mite allergen with premanipulated skin may induce dermatitis. It is, however, uncertain whether such conditions are met during natural contact with house dust mite. In the past, allergen inhalation has been suggested to induce exacerbation of atopic dermatitis. The aim of this study was to investigate whether dermatitis could be induced in patients with atopic dermatitis by inhalation of house dust mite. METHODS: Twenty pa

The precipitation of symptoms by common foods in children with atopic dermatitis.

Atopic dermatitis (AD) is a chronic and disabling condition that has a major impact on financial and social resources of the individual and the community. Its incidence is increasing dramatically, and no cure is available. Pharmacological treatment is only partially effective. The evidence that diet plays a role in children with atopic dermatitis is now irrefutable. Prophylactic measures can prevent or limit the development of AD, and partially restricted diets can modify the disease's course or severity. This study reports the reactions to various foods as perceived by parents of 112 children

Does a patient's occupation influence the course of atopic dermatitis?

The importance of occupational environments in relation to the course of an atopic skin disease was investigated. A series of 1008 adult patients, who had had an atopic disease since their teenage years, were studied. Severe or moderate childhood atopic dermatitis tended to persist or relapse in adulthood independent of occupation. Only dermatitis of the hands showed a correlation with extensive daily exposure to occupational irritation factors. Atopic dermatitis patients did not go on sick leave more often than controls or those with atopic mucosal symptoms, although sick leave caused by skin

Oral sodium cromoglycate in the management of atopic dermatitis in children.

Atopic dermatitis (AD), a common, multifactorial, and extremely complex disorder, should be examined from various points of view, and it requires all the skill a physician can gather because it can be a challenge for pediatricians, dermatologists, and allergists. The role of dietary factors in AD has long been a subject of controversies, and several investigators have demonstrated the effectiveness of elimination diets in the management of AD. The treatment of choice for AD due to food sensitivity (FS) is the elimination of the offending food(s). This can be easily achieved when the child is a

Clinical and histological features of dry skin in atopic dermatitis.

Of 200 atopic dermatitis patients observed during cold months, 44 (22%) had ichthyosis vulgaris. Histologically, the dry skin in atopic dermatitis coexistent with ichthyosis demonstrated ichthyotic features which were frequently superimposed with eczematous changes. The dry skin in pure atopic dermatitis demonstrated the histology of mild eczema. Examinations using monoclonal antibodies showed that the dry skin in pure atopic dermatitis had increased numbers of OKT6-positive cells in epidermis. The lymphocytes in the dermal infiltrates consisted predominantly of LEU-3a-positive cells. It is co

Pharmacokinetics and antipruritic effects of hydroxyzine in children with atopic dermatitis.

We studied the pharmacokinetics and antipruritic effects of hydroxyzine hydrochloride in 12 children, mean age 6.1 +/- 4.6 years, with severe atopic dermatitis. After a single 0.7 mg/kg orally administered dose of the drug, the mean peak serum hydroxyzine concentration of 47.4 +/- 17.3 ng/ml occurred at a mean time of 2.0 +/- 0.9 hours. The mean elimination half-life was 7.1 +/- 2.3 hours, the mean clearance rate was 32.08 +/- 11.05 ml/min/kg, and the mean apparent volume of distribution was 18.5 +/- 8.6 L/kg. The elimination half-life increased with increasing age (r = 0.83). Pruritus was sig

Impairment of the autologous mixed lymphocyte reaction in atopic dermatitis.

The T cell proliferative response to autologous non-T cells is termed the autologous mixed lymphocyte reaction (AMLR). Recent studies have suggested that the AMLR represents an inducer circuit for the activation of T8+ suppressor/cytotoxic effector cells. Since atopic dermatitis (AD) patients are deficient in T8+ cytolytic T cell function, we investigated the AMLR in AD. When sheep erythrocytes were used to separate T cells from non-T cells, the AMLR was found to be significantly decreased (P less than 0.001) in AD patients (n = 11; delta cpm = 1,550 +/- 393) when compared with normal control

Differences in the detection of circulating Hsp90 alpha between patients with atopic dermatitis and dermatitis herpetiformis.

Heat shock protein 90 alpha (Hsp90α) is one of the key intra- and extracellular chaperones responsible for the biological activity of various signaling molecules that are involved in (auto)immune-mediated inflammatory diseases. Recent epidemiologic data suggest that patients with atopic dermatitis (AD) are at risk for several autoimmune diseases, including dermatitis herpetiformis (DH), an extraintestinal manifestation of celiac disease (CD). In addition, pruritic diseases such as AD may be confused clinically with DH. In this study, we aimed to determine the role of circulating Hsp90α in pati
