Contact dermatitis: prognosis, risk factors, and rehabilitation.

This article reviews the prognosis for irritant and allergic contact dermatitis and occupational and nonoccupational contact dermatitis. Factors adversely affecting prognosis are discussed. Rehabilitation of the patient with disabling contact dermatitis is considered. The importance of the various risk factors for contact dermatitis are reviewed.

Photoallergic contact dermatitis due to mineral oil.

A 39-year-old metal polishing mechanic had had an acute-on-chronic eczematous eruption on sun-exposed skin for 2 years. It improved in winter. He had been using an insoluble oil, CRT 20, as a cutting oil for 20 years. The cutting oil itself and mineral oil, which was one of its ingredients, showed positive reactions on photopatch testing. The difference action spectrum with or without mineral oil application, assessed at 48 h, ranged from 300 to 350 nm, with a peak at 320 nm. This is the 1st report of photoallergic contact dermatitis due to mineral oil.

Contact dermatitis from N-(alpha-chlorobenzylidene)phenylhydrazine.

N-(alpha-chlorobenzylidene)phenylhydrazine (AC) is both a strong irritant and a powerful sensitizer. 5 out of 6 people at risk fell ill after only once coming into contact with this halogenated hydrazine derivative. As in the first report concerning this problem the compound was used to train chemistry students.

Consort contact dermatitis due to oak moss.

An allergic contact dermatitis in a woman was found to be due to oak moss in her husband's after-shave lotion. When routine patch testing reveals a positive reaction, the dermatologist should consider exposure to the antigen not only in the patient but also through contact with the patient's consort.

Irritant contact dermatitis and chrome-passivated metal.

An outbreak of hand dermatitis is reported amongst employees working on a new assembly line of an electronics factory. 24 out of 41 employees had signs of irritant contact dermatitis. The fingertips and the dorsal parts of the hands were especially affected. Mechanical factors in combination with zinc-chromium compounds from galvanised steel seemed to be responsible for the dermatitis and the dry skin. The use of an emollient solved the problem.

Contact dermatitis to ethyl-cyanoacrylate-containing glue.

3 patients with contact dermatitis to an ethyl cyanoacrylate glue are presented. Although reactions to cyanoacrylate glues are considered rare, more widespread use of these products by nail salons is likely to be associated with an increased incidence of positive reactions. All 3 of our patients came into contact with the glue during "nail wrapping". In this process, ethyl cyanoacrylate or another "instant glue" is used to adhere glue-impregnated silk or linen to the nail plate which is then filed to shape the nail. This procedure creates fine acrylic-containing dust which may facilitate an al

Industrial airborne irritant or allergic contact dermatitis.

Industrial airborne irritant or allergic contact dermatitis is commonly observed in many factories. Examples of airborne irritants include fibres (such as fibreglass or rockwool), various kinds of dust particles (such as cement, slag, sludge, insulating foam, wood chips), acids and alkalis, gasses and vapours. Airborne contact allergens are unequivocally numerous. The clinical symptoms of both irritant and allergic airborne contact dermatitis are reviewed.

Photoallergic and allergic contact dermatitis from dibenzoylmethanes.

Dibenzoylmethanes are widely used in Europe in UVA-absorbing sunscreens. We found that 2 of these, 4-isopropyl-dibenzoylmethane and 4-tert.butyl-4'-methoxy-dibenzoylmethane caused allergic or photoallergic contact dermatitis. This is the first report on allergic and photoallergic reactions caused by dibenzoylmethanes.

Structure-activity relationships in allergic contact dermatitis.

Skin sensitization by simple chemicals involves reaction with nucleophilic groups on skin proteins or possibly on proteins in cell membranes to form haptenic groups, which act as allergenic determinants. A quantitative model for sensitization by 'alkylating agents', the RAI (Relative Alkylation Index) theory, has been devised and applied to the sensitization observed with three model series of compounds--p-nitrobenzyl halides and saturated and unsaturated sultones. Examination of chemical structures, and predictions of the types of chemical reaction that substances may undergo, can be used to

Clonidine: irritant and allergic contact dermatitis assays.

Clonidine in petrolatum, clonidine transdermal device, and its placebo device were assayed for relative irritancy potential (21-day cumulative irritancy assay) and allergic contact dermatitis potential (Draize repeat insult patch test assay). Both clonidine in petrolatum and the clonidine transdermal device appear to be of minimal irritancy potential. Only the clonidine device demonstrated, in the Draize assay (and in clinical trials), allergic contact dermatitis potential. Its relatively delayed onset of demonstration of allergic contact sensitization in clinical trials requires investigation

Occupational pigmented contact dermatitis from azo-dyes.

A 51-year-old man had been working in a dye factory for 25 years and had noticed itching and pigmentation on the extremities for the past 5 years. Patch testing showed positive reactions to Sudan I and Vacanceine Red, among the azo dyes which he had been handling. After he changed his work, he became free from erythematous lesions with itching. The pigmentation almost disappeared 10 months later.
