Eating in the adult world: the rise of dieting in childhood and adolescence.

Dieting is a common practice among adolescent girls. Recent research suggests that adolescent dieting may not be benign and that it is becoming more frequent in younger age groups. The present study investigated dieting motivation and the underlying attitudes to body weight and shape in two age groups. Eighty-four nine-year-old and 86 14-year-old girls from the same school completed assessments of dietary restraint, body esteem, body satisfaction and body figure preferences. The girls' body weight and height were also measured. The distribution of restraint scores revealed individuals from bot

Psychological and behavioral differences among females classified as bulimic, obligatory exerciser and normal control.

The compulsion to maintain a lean body shape while living in a society obsessed with food may encourage the use of dieting, binge eating and purging behaviors in young women. The body of literature on bulimia nervosa (Boskind-Lodahl and Sirlin 1977; Fairburn and Cooper 1984; Katzman and Wolchik 1984; Scott 1988) clearly establishes its relationship to the current American fashion for thinness and the value placed on physical attractiveness for self-esteem. This notion promotes a multitude of weight control strategies ranging from self-starvation (Humphrey 1983; Brownell and Foreyth 1986) to ci

Average total energy expenditure in anorexia nervosa, bulimia nervosa, and healthy young women.

The double-labeled water method was used to measure average daily total energy expenditure (EE) in 11 healthy normal-weight women classified as unrestrained eaters, in 8 patients with anorexia nervosa, and in 8 patients with bulimia nervosa. The body mass index was 20.0 +/- 1.3 kg/m2 in the control group, 15.2 +/- 5.6 kg/m2 in the anorectic groups, and 19.7 +/- 1.9 kg/m2 in the bulimic group. EE was measured over a 2-week period during which weight remained constant in all groups and was 2357 +/- 504 kcal/day for the controls, 2510 +/- 920 kcal/day for the bulimics, and 2899 +/- 656 kcal/day f

Dieting and pain sensitivity: a validation of clinical findings.

To validate findings of a reduced pain sensitivity in anorexia and bulimia nervosa, the effects of dieting on somatosensation (especially pain sensitivity) were investigated in healthy young women. One group of subjects (n = 11) received a calorically reduced balanced diet for 21 days, while the other group (n = 14) continued to eat normally. The fasting state induced in the dieting subjects was comparable to that of eating disorder patients, since the dieters showed a reduction of the body mass index, a decrease in triiodothyronine and an increase in beta-hydroxybutyric acid plasma levels. Psychiatric residents' educational experiences and attitudes toward eating disorders.

Our study of 180 psychiatric residents from all 16 Canadian psychiatric training centres showed that only 22.2% found university teaching on eating disorders adequate. Eighty-nine percent believed formal instruction on anorexia nervosa (AN) and bulimia nervosa (BN) should be made mandatory. Significantly more female than male residents were interested in AN and BN patients. The authors suggest special attention be given to residents' curriculum on the subject.

Childhood obesity: an update.

Childhood obesity is an increasing health problem in the United States, and it is positively correlated with eventual adult obesity. Childhood obesity is related to many of the same risk factors as adult obesity, most notably cardiovascular and psychosocial factors. Prevention or detection and management of the problem in childhood is warranted. This article describes assessment of obesity, predictors for its development, and components of treatment programs that are successful in children. Information is based on review of the literature and the author's experience working with obese children

Ephedrine as an anorectic: the story of the 'Elsinore pill'.

Obese patients, age 18-60 years, overweight 20-80 per cent, entered a controlled, clinical study comparing the effects of two anorectic drugs, ie a prescription containing ephedrine and caffein ('Elsinore pills') and diethylpropion, with placebo. All 132 patients were instructed in a 1200-kcal diet, and 108 patients completed 12 weeks' treatment. There was a significantly better effect on body weight of diethylpropion (39 patients, median weight loss 8.4 kg, P less than 0.01) as well as of 'Elsinore pills' (38 patients, median weight loss 8.1 kg, P less than 0.01) compared to the effect of pla

Effects of Low-Carbohydrate versus Mediterranean Diets on Weight Loss, Glucose Metabolism, Insulin Kinetics and β-Cell Function in Morbidly Obese Individuals.

Low-calorie Mediterranean-style or low-carbohydrate dietary regimens are widely used nutritional strategies against obesity and associated metabolic diseases, including type 2 diabetes. The aim of this study was to compare the effectiveness of a balanced Mediterranean diet with a low-carbohydrate diet on weight loss and glucose homeostasis in morbidly obese individuals at high risk to develop diabetes. Insulin secretion, insulin clearance, and different β-cell function components were estimated by modeling plasma glucose, insulin and C-peptide profiles during 75-g oral glucose tolerance tests

[Improvement of clinical parameters in a patient with metabolic syndrome through intervention in nutrition education and improvement of lifestyle].

OBJECTIVE: The main objective is the reduction of early morbidity and mortality associated with metabolic syndrome (MS). METHODS: 24h intakes count for 3 days and clinical-nutritional history. Follow-up of 19 visits to monitor the evolution. RESULTS: Following weight loss, the measurement of capillary blood glucose was 59 mg/dl, discontinuing then treatment with metformin; after 7 days without metformin treatment the value was 64 mg/dl. CONCLUSIONS: Intervention in nutritional education and lifestyle contributed to almost a 10% reduction of body weight; the patient moved from the obesity BM

Geometric and Mechanical Bone Response to a Multidisciplinary Weight Loss Intervention in Adolescents With Obesity: The ADIBOX Study.

The effectiveness of structured multidisciplinary weight loss (WL) programs combining nutrition and physical activity on bone geometry and strength remains uncertain in adolescents with obesity. The study investigated the impact of a structured WL intervention on bone geometry and strength in adolescents with obesity. Thirty-one adolescents with obesity (mean [standard deviation] 13.61 [1.27] yr, body mass index Z-score 2.26 [0.30]) experienced an 8-mo WL program. A group of 23 maturation-matched controls (mean [standard deviation] 15.90 [0.43] yr, body mass index Z-score -0.12 [0.48]) were re

Childhood and adolescent obesity definitions as related to BMI, evaluation and management options.

In North America, 30% of children are overweight or obese. Child obesity is a precursor to adult health risks. Diagnosis of childhood obesity is difficult. The best measurement is BMI percentile charts. Obesity in children and adolescents can be treated by diet and activity changes. Family involvement in healthy lifestyle changes is best for weight loss and prevention.

Total body weight loss of ≥ 10 % is associated with improved hepatic fibrosis in patients with nonalcoholic steatohepatitis.

BACKGROUND: Given the rising epidemics of obesity and metabolic syndrome, nonalcoholic steatohepatitis (NASH) is now the most common cause of liver disease in the developed world. Effective treatment for NASH, either to reverse or prevent the progression of hepatic fibrosis, is currently lacking. AIM: To define the predictors associated with improved hepatic fibrosis in NASH patients undergoing serial liver biopsies at prolonged biopsy interval. METHODS: This is a cohort study of 45 NASH patients undergoing serial liver biopsies for clinical monitoring in a tertiary care setting. Biopsies we

Examining the role of negative urgency in a predictive model of bulimic symptoms.

The etiological dual pathway model of bulimia nervosa suggests that dietary restraint and negative affect are significant predictors of this disorder. Negative urgency, or the tendency to act rashly in response to negative emotionality, is also associated with bulimia nervosa; however, no study has examined the role of negative urgency within the context of the dual pathway model. The purpose of this study was to investigate the relationship between bulimic symptomatology and negative urgency in the context of dietary restraint and depressive affect. Participants (N=166) were college women rec

Dieting in bulimia nervosa is associated with increased food restriction and psychopathology but decreased binge eating.

The cognitive behavioral model of bulimia nervosa (BN) suggests that dieting is central to the maintenance of binge eating. However, correlational and experimental studies suggest that additional clarification is needed about the nature of this relationship. Dieting, weight, eating disorder psychopathology, and depression were assessed at admission among 166 patients with BN presenting for residential treatment. As in past research, a significant fraction (43%) of patients with BN reported not currently dieting. A comparison of weight loss dieters and non-dieters found greater food restriction

Exercise and diet in weight management: updating what works.

The world is facing major problems associated with the rapid increase in levels of overweight and obesity. Solving this problem via appropriate modifications to exercise habits and/or diet appears easy, but in practice it is inordinately difficult and only a small percentage manage to maintain their weight loss over the long term. However, a number of strategies can be used to increase the chances of success. Medical doctors, dieticians and other allied health professionals are potentially well placed to provide guidance to those at risk of overweight/obesity. This review is aimed at supportin

Bilateral parotid enlargement due to malnutrition under the influence of the media in an adolescent in Lithuania.

The elimination of censorship for the media in post-communist countries in transition has contributed to increases in the prevalence of several medical problems. Children and adolescents are particularly vulnerable to the messages conveyed through the media, which influence their perceptions and behaviour. We describe a case of bilateral parotid enlargement due to malnutrition under the influence of self-prescribed diet in an adolescent. A 15-year-old girl reported to our institution under suspicion of Sjögren's syndrome for medical advice. Two months ago she developed persistent bilateral par
