PURPOSE: To describe a hybrid technique of reversed frozen elephant trunk to treat thoracoabdominal aortic aneurysms (TAAA) through an abdominal only approach.
TECHNIQUE: The technique is demonstrated in a 29-year-old Marfan patient with a chronic type B aortic dissection previously treated with a thoracic stent-graft who presented with a thoracoabdominal false lumen aneurysm. Through an open distal retroperitoneal approach to the abdominal aorta, a frozen elephant trunk graft was implanted over a super-stiff wire upside down with the stent-graft component in the thoracic aorta. Following dep

A patent false lumen with persistent flow after endovascular repair of type B aortic dissection (TBAD) is associated with an ongoing risk of aortic dilation and rupture. We describe the case of a 64-year-old man who initially underwent thoracic endovascular aortic repair for acute complicated TBAD, but continued to have symptomatic retrograde aneurysm filling and dilatation because of a patent false lumen. Coil embolization of the patent false lumen successfully excluded the aneurysmal thoracic aorta from further perfusion, and led to a decease in aneurysm size on follow-up. Our case report is

BACKGROUND: The aim of this study was to determine morbidity and 30-day mortality rates of thoracic endovascular aortic repair (TEVAR) for Stanford B dissection over a 16-year period and determine if these rates have improved with better stent-graft technology and surgical technique.
METHODS: Electronic databases were searched in all languages and a systematic review conducted. A comparison of the early (1998-2007 = 787 patients) and later (2007-2013 = 787 patients) halves of the patient population was done. Studies were chosen based on availability of details regarding morbidity and mortalit

BACKGROUND: Because of their fragile and thin wall, ruptured blood blister-like aneurysms (BBAs) at the anterior wall of the internal carotid artery (ICA) are difficult to manage, both surgically, as well as endovascularly. BBA is usually a tiny and broad-necked aneurysm, but it occasionally demonstrates a relatively saccular-like shape. In addition, the pseudoaneurysm sac often assumes a saccular shape. In this paper, the authors present their experience in treating these saccular-shaped BBAs endovascularly with coil packing.
METHOD: Nine saccular-shaped ruptured BBAs in nine patients (one m

OBJECTIVE AND IMPORTANCE: Dissecting aneurysms of the intracranial vertebral artery are increasingly recognized as a cause of subarachnoid hemorrhage. We present a case involving technical success of the stent-supported coil embolization but with recurrence of the dissecting pseudoaneurysm of the intracranial vertebral artery. The implications for the endovascular management of ruptured dissecting pseudoaneurysms of the intracranial vertebral artery are discussed.
CLINICAL PRESENTATION: A 36-year-old man with a remote history of head injury had recovered functionally to the point of independe

BACKGROUND: Isolated renal artery involvement, whether inflammatory or dissecting, is an uncommon and challenging presentation in clinical practice. Its nonspecific symptoms and overlap with non-inflammatory vasculopathies complicate the distinction between localized vasculitis and isolated renal artery dissection (IRAD), two entities requiring fundamentally different therapeutic approaches.
CASE PRESENTATION: We report two cases of unilateral renal artery pathology initially suspected to represent vasculitis. Case 1: A 47-year-old man presented with acute flank pain. CT and MRA revealed peri

Limited sensitivity of conventional MRI and the invasiveness of TFCA have restricted the diagnosis of arterial dissection in lateral medullary infarction (LMI), leading to its underestimation and obscuring its clinical significance. However, high-resolution vessel wall MRI (HR-VWMRI) now allows more accessible and accurate diagnosis of dissection, prompting reevaluation of its prevalence. This single-center retrospective study included the LMI patients, among whom a subset underwent advanced imaging as HR-VWMRI or TFCA to confirm definite arterial dissection, and identified clinical factors as

An 85-year-old man with a history of aortic dissection suddenly fainted, underwent cardiac heart arrest, and died. An autopsy was performed, but the cause of death was not grossly identified. Congo red staining detected amyloid deposits in systemic organs, including the heart, lungs, liver, and kidneys. Immunohistochemical (IHC) analysis revealed immunoglobulin (Ig) λ light chain (-λ) in systemic blood vessels and transthyretin (TTR) in the heart and lungs. Ig-λ was predominantly positive in the blood vessels of the lungs, while TTR was detected in the alveolar septum. In the heart, Ig-λ was p

OBJECTIVE: To elucidate the working principles of three-dimensional dynamic contrast-enhanced magnetic resonance angiography(3D DCE-MRA)and assess its value in the diagnosis of lesions in the major abdominal blood vessels.
METHODS: The imaging data of 3D DCE-MRA and conventional MR examination in 32 cases of suspected major abdominal blood vessel diseases were obtained and analyzed retrospectively.
RESULTS: The peak flow of contrast agents through the abdominal aorta at the level of renal artery occurred 17 to 23 s after the injections. Normal presentation was found in 3 cases, with clear im

307 patients with complete resection for non-small cell lung cancer between 1989 and 1996 were examined to evaluate the clinical features of local recurrence. Postoperative recurrence was observed in 104 of the 307 patients. At the first recurrence, local recurrences occurred in 21 (20%) of the 104 patients, distant metastases in 72 (69%), and combined recurrences in 11 (11%). 32 patients had local recurrences in 47 localizations, bronchial stump recurrences occurred in 11 (34%) of 47 localizations, hilarmediastinal lymph node metastases in 18 (56%), supraclavicular lymph node metastases in 10

Blunt carotid artery injury (BCI) poses a rare yet severe threat following vascular trauma, often leading to significant morbidity and mortality. We present a case of a 33-year-old male who suffered complete thrombotic occlusion of the right common carotid artery (CCA) following a workplace accident. Clinical evaluation revealed profound neurological deficits, prompting multidisciplinary surgical intervention guided by the Denver criteria (Grade I - disruption inside the vessel that results in a narrowing of the lumen by less than 25%; Grade II - dissection or intramural hematoma causing great

BACKGROUND: Perforation of the carotid artery is a rare, life-threatening emergency. This entity is usually caused by failed puncture of jugular veins, external trauma, or infection of the vascular wall. The existence of spontaneous rupture as a cause of vessel rupture is discussed in the literature.
CASE REPORT: The case of a 57-year-old woman who suffered painful cervical swelling on the left side for 2 days is described. Six weeks prior to this, she had received transjugular intrahepatic shunt implantation (TIPS) via the jugular vein because of liver cirrhosis. Further signs were vocal cor

IMPORTANCE: Spontaneous coronary artery dissection (SCAD) is an increasingly recognized cause of myocardial infarction predominantly affecting women. There is a paucity of reproductive health data surrounding pregnancy-associated SCAD (P-SCAD).
OBJECTIVE: To examine detailed reproductive variables as well as demographics, psychosocial, and SCAD-event factors among women with a history of at least 1 prior pregnancy in a large, multicenter registry (iSCAD Registry).
DESIGN, SETTING, AND PARTICIPANTS: This cohort study compared data of women with P-SCAD vs women with non-pregnancy-associated SC

BACKGROUND: This systematic review evaluated patient characteristics and outcomes of patients presenting with concomitant spontaneous coronary artery dissection (SCAD) and cardiogenic shock (CS) compared with those without CS.
METHODS: Data from four studies were analyzed. A comprehensive electronic literature search was performed by two authors using PubMed and Embase databases from inspection to February 1st, 2025. MeSH and broad terms including "Coronary Artery Dissection, Spontaneous" OR "SCAD" OR "Coronary Artery Dissection" were used with and without "Clinical Trials" as a search limit.

A healthy man in his 40s presented with acute anterolateral ST-segment elevation myocardial infarction after sudden-onset chest pain during a coughing episode. Coronary angiography and intravascular ultrasound revealed extensive spontaneous coronary artery dissection (SCAD) of the left anterior descending artery with Thrombolysis in Myocardial Infarction II distal flow. He was managed conservatively, but his course was complicated by severe left ventricular systolic dysfunction, apical thrombus and a ventricular tachycardia storm leading to cardiogenic shock. He required repeated emergency dir

We report a 55-year-old man, the recipient of a cardiac allograft for ischemic cardiomyopathy 9 years earlier, who presented with progressive aortic root dilation, worsening aortic insufficiency, and an incidentally discovered chronic type A aortic dissection limited to the donor aorta. The patient was taken to the operating room, and the aortic dissection successfully repaired using standard reoperative techniques. This is the sixth case reported in the literature, and only the fourth survivor. To our knowledge, this case represents the first successful repair, of a limited aortic dissection

To reduce the incidence of false aneurysm formation at the suture lines, a known complication with the inclusion technique such as Bentall's procedure or Cabrol's procedure, modified Bentall's procedure (Carrel patch technique), which is identical to Inberg's procedure, has been selected as treatment for annulo-aortic ectasia since 1991. This operation was carried out in 6 consecutive patients with annulo-aortic ectasia from July 1991 to August 1992. The aortic valve and the aneurysm were resected, the coronary ostia were dissected free, mobilized, and then implanted to the composite graft. It

A rare case of acute aortic dissection and aortic regurgitation in a patient with Turner's syndrome is reported. A 25-year-old woman with a history of Turner's syndrome presented to our hospital with complaints of chest pain and dyspnea. Emergent surgery was performed after chest roentgenography and two-dimensional echocardiography. Intraoperatively, the patient was found to have a single coronary artery and a bicuspid aortic valve. The patient was treated successfully with a composite graft consisting of a 24 mm woven dacron graft, a 21 mm St. Jude Medical aortic prosthetic valve, and an equi
