BACKGROUND: Takayasu arteritis is a rare, chronic vasculitis affecting large arteries, primarily the aorta and its principal branches. The illness frequently presents with non-specific systemic symptoms, leading to a postponed diagnosis. Dysphagia resulting from aortic involvement is a rare manifestation and may resemble other cardiovascular or gastrointestinal disorders.
CASE PRESENTATION: We report a 45-year-old Iranian woman with a one-year history of exertional dyspnoea, intermittent dysphagia for solid food, and diffuse musculoskeletal pain with morning stiffness. Initial evaluation show

OBJECTIVE: Although anecdotal findings of spiraling aortic dissections have been reported, the mechanisms of dissection propagation are unclear; hence, we categorized type B dissection helicity and mechanistically explain distinct propagation patterns.
METHODS: From computed tomography of 102 patients, type B dissections were geometrically modeled from entry tear to renovisceral aorta and helical properties were quantified.
RESULTS: There were 4 distinct helical groups: right-hand chiral, left-hand chiral, mixed-chiral, and nonhelical. Right- and left-hand chiral cases exhibit true lumen blo

Postoperative chylothorax after lung resection and mediastinal lymph node dissection is a rare complication that may be difficult to manage when conservative treatment is ineffective, and the thoracic duct or leak point cannot be identified intraoperatively. Chylothorax is thought to result from thoracic duct injury associated with mediastinal lymph node dissection. Although surgical thoracic duct ligation is recommended in refractory cases, identification of the leakage site or thoracic duct itself is often challenging, particularly in patients with intrathoracic adhesions or in facilities wh

OBJECTIVES: Long-term therapeutic effects of thoracic endovascular aortic repair (TEVAR) for chronic type B aortic dissection remain controversial. This study aimed to evaluate the possible predictors of late aortic rupture and re-interventions after TEVAR.
METHODS: We retrospectively reviewed the operative outcomes of 40 patients who underwent TEVAR for chronic type B aortic dissection at Kyushu University Hospital. During a mean follow-up period of 39.2 months, we assessed aortic morphology via computed tomography and then employed a multivariable Cox regression analysis in an attempt to id

BACKGROUND: The present study aimed to evaluate the effect of two-stage hybrid aortic repair at the distal aorta of Stanford A dissection with malperfusion.
METHODS: This retrospective case series included 20 patients with Stanford A dissection administered two-stage thoracic endovascular aortic repair (TEVAR) about 1 month after central repair because of visceral or limb malperfusion. The patients were examined by computed tomography (CT) angiography at 3, 6, 12 and 24 months after operation. Recovery of malperfusion and true lumen index were evaluated during follow-up.
RESULTS: Twenty pati

BACKGROUND AND AIM OF THE STUDY: A bovine pericardial conduit processed in glutaraldehyde was designed, incorporating the principle of crimping used for synthetic vascular prostheses. The crimping process did not affect the integrity of collagen fibers and tissue structure. This conduit, designed for aortic reconstruction, is available in different sizes, with or without a biological valve.
METHODS: Between October 1989 and May 1997, 40 patients with aortic dissection, aortic aneurysm, aortic coarctation or aortoiliac occlusive disease underwent aortic reconstruction using this vascular subst

The purpose of this study was to evaluate clinical outcomes of combined endovascular and open techniques to eradicate false lumen dilatation in the visceral aortic segment after type B aortic dissection associated with aortic aneurysm. We reviewed eight patients with distal thoracic and abdominal false lumen dilatation treated with a staged procedure. These included arch debranching as needed, proximal thoracic endovascular repair, and open surgical correction with abdominal aortic replacement of the visceral and infrarenal aorta. False lumen eradication was successful in all patients. There w

Acute aortic pathologies include acute aortic syndrome (aortic dissection, intramural hematoma, penetrating aortic ulcer), impending rupture, aortic aneurysm rupture and aortic trauma. Acute aortic syndrome, aortic aneurysm rupture and aortic trauma are life-threatening conditions requiring prompt diagnosis and treatment. The basic imaging modality for "acute aorta" is CT angiography with typical findings for these aortic pathologies. Based on the CT, it is possible to classify aortic diseases and anatomical classifications are essential for the planning of treatment. Currently, endovascular t

PURPOSE: To report a case of operative intervention for an aortic arch aneurysm rupture via a hybrid approach.
METHODS AND RESULTS: Ten years after successful open repair of an acute type A aortic dissection a male presented with a rupture of his distal aortic arch aneurysm. A hybrid procedure was carried out which involved debranching of the innominate and left common carotid arteries and stent-graft deployment landing proximally into the ascending aorta and distally into the descending thoracic aorta.
CONCLUSIONS: Ruptured aortic arch aneurysms present a challenging and complex surgical di

Acute Stanford type A aortic dissection (TAAD) is a surgical emergency characterized by a high mortality rate and numerous complications. In the treatment of TAAD, the timing of surgery and the choice of surgical procedure are of paramount importance. Open total aortic arch reconstruction remains the gold standard for aortic arch surgery and is one of the most challenging procedures. However, this approach is invasive, relatively lengthy, and associated with substantial bleeding, which necessitates high levels of operator skill and carries the risk of multiple complications, particularly neuro

We present the case of a 75-year-old man with repeated lower limb hematoma caused by consumptive coagulopathy from a type B chronic aortic dissection. His abdominal aorta was replaced with a Y-shaped graft 30 years prior to admission. As his previous aortic stent graft treatment failed, he underwent open surgical prosthetic graft replacement of the descending aorta under deep hypothermia. To reduce intra- and postoperative bleeding, we avoided cutting the ribs and intercostal arteries. The aneurysm was approached only through the 8th intercostal space; however, as the proximal descending aorta

A 74-year-old man was diagnosed with Stanford type B early thrombosed aortic dissection. Computed tomography (CT) 2 years later revealed that an aneurysm of the distal arch had enlarged from a maximal diameter of 47 mm to 62 mm and involved the descending aorta. Total arch replacement using a quadrifurcated arch graft proceeded through a median sternotomy under deep hypothermic cardiopulmonary bypass (20 degrees C) and selective brain perfusion. The long elephant trunk of another prosthesis was inserted into the descending aorta and a distal anastomosis was constructed immediately after the le

Acute aortic dissection is a life threatening disease, which is occasionally limited to an ascending aorta only (DeBakey type II). In majority of patients it involves the aortic arch and entire rest of the aorta (DeBakey type I). The standardized cannulation and operation strategy can not be used in cases, when aortic arch branches are involved in dissection (complex aortic arch dissection) or in cases with malperfusion or severely compromised hemodynamics (tamponade or heart failure due to severe aortic valve insufficiency). The aim of this present review is to present the "Essen" treatment c

The objective of our study is to describe the most significant radiological findings that influence and guide the preparation for emergency surgery for acute thoracic aortic dissection in the radiology emergency department, based on our knowledge of the surgical technique used. Acute thoracic aortic dissection is the most common urgent aortic pathology, with high morbidity and mortality rates. Radiology plays a key role in proper diagnosis, with CT angiography being the imaging test of choice, performing a multiphase study with image acquisition without and after contrast administration. BACKGROUND: Thoracic Endovascular Aortic Repair [TEVAR] is used as a gold standard treatment for aortic disease such as Type B dissection, proximal descending thoracic aortic disruption and descending thoracic aortic fistulas. There was never a report, before this one, of TEVAR utilization for uncontrolled bleeding on the aortic arch cannulation site.
CASE PRESENTATION: This case report is of a 72-year-old female patient who presented to our facility with a day history of anterior sharp pain and dyspnea. Clinical examination revealed a frail patient in distress with tachycardia, tachypnea and

INTRODUCTION: Aortoesophageal fistula (AEF) is a fatal complication results in sudden massive hematemesis. Although thoracic endovascular aortic repair (TEVAR) is an established method of treating aortic aneurysms or aortic dissection, the number of AEF after TEVAR is recently increasing due to the spread of TEVAR. However, the therapeutic strategy for AEF remains controversial.
PRESENTATION OF CASE: We describe a 71-year-old man with Stanford B aortic dissection and aortic aneurysm rupture treated by TEVAR who developed AEF between the thoracic aorta and upper thoracic esophagus 20 months th

The authors present a case report of a patient who was diagnosed with aortic dissection during a hemodynamic exam following acute myocardial infarction. With reference to this case, the European Society of Cardiology's latest classification of aortic dissection, published in 2001, is also presented. The patient had the usual risk factors described for aortic dissection, which are simultaneously those for coronary disease. The patient reported an isolated episode in the past of retrosternal pain, radiating to the back, which may have been caused by the aortic dissection. Since then he had been
