The diagnostic value of computed tomography (CT) and barium enema (BE) studies was evaluated prospectively in hospitalized patients with the presumptive diagnosis of acute sigmoid diverticulitis based on the presence of left-lower-quadrant pain and tenderness, fever, and leukocytosis. Of 56 patients, 11 had sigmoid diverticulitis confirmed at surgery and 16 by clinical response to medical therapy. CT, performed in all diverticulitis patients, had positive results in 93% (25 of 27). These compared favorably with BE study results, of which 80% (20 of 25) were positive. Neither examination had fa…

IMPORTANCE: Diverticulitis has a tendency to recur and affect quality of life.
OBJECTIVE: To assess whether sigmoid resection is superior to conservative treatment in improving quality of life of patients with recurrent, complicated, or persistent painful diverticulitis.
DESIGN, SETTING, AND PARTICIPANTS: This open-label randomized clinical trial assessed for eligibility 128 patients with recurrent, complicated, or persistent painful diverticulitis in 6 Finnish hospitals from September 29, 2014, to October 10, 2018. Exclusion criteria included age younger than 18 years or older than 75 years…

BACKGROUND: With the growing adoption of nonoperative management of complicated diverticulitis, its postdischarge consequences remain unclear. This study aimed to compare the readmission outcomes between operative and nonoperative management of patients with complicated diverticulitis.
METHODS: This is a retrospective cohort study of the 2020-2021 Nationwide Readmission Database, including adult patients (≥18 years) with acute complicated diverticulitis, defined by the presence of concomitant abscess formation, obstruction, or perforation. Patients were stratified by management approach-nonop…

INTRODUCTION: Robotic-assisted surgery is now the standard for elective colorectal procedures, but its use in emergencies remains limited. This study describes the feasibility of a dedicated emergency robotic programme for acute complicated diverticulitis.
METHODS: We retrospectively analysed a prospectively maintained database (2020-2023). Of 31 patients undergoing robotic surgery for suspected complicated diverticulitis, 19 met inclusion criteria and underwent robotic left colectomy or Hartmann's procedure.
RESULTS: Median total procedure time was 171 minutes (IQR 42) and median total OR t…

BACKGROUND: Acute diverticulitis (AD) is a prevalent gastrointestinal disorder with a significant recurrence rate. Faecal calprotectin (FC) is a non-invasive biomarker of intestinal inflammation, but its role in diagnosing and monitoring diverticulitis remains to be fully established.
OBJECTIVE: This narrative review aims to evaluate the current evidence on the utility of FC in the diagnosis, severity assessment, prediction of recurrence, and monitoring of therapeutic response in patients with diverticular disease (DD) and AD.
METHODS: A structured literature search was conducted using PubMe…

Hepatic abscesses are uncommon and rarely associated with diverticulitis as an indirect source of infection. We report the case of a 60-year-old male with a history of recurrent diverticulitis who presented with fever, nausea, and abdominal pain demonstrating a single multiloculated hepatic lesion measuring 5.8 × 5.1 × 5.1 cm upon admission. Piperacillin-tazobactam was initiated for sepsis management, and metronidazole was initiated for amoebic coverage. Aspiration and drainage of the abscess were performed, and cultures confirmed Streptococcus intermedius, prompting antibiotic therapy to be n…

INTRODUCTION: Non-operative management (NOM) of acute complicated diverticulitis (ACD) has become the preferred approach. A Hospital-at-Home (HaH) program at our institution launched to provide NOM for ACD. This study evaluated the HaH program feasibility for well-selected patients.
METHODS: Prospective single-arm single-institution non-randomized feasibility study conducted from 10/2022 to 10/2023. Patients in the Emergency Department (ED) with ACD who met inclusion criteria (> 18 years, hemodynamically stable, caregiver at home, technology proficiency, within 10 km of service center within …

Perforated diverticulitis is a critical surgical emergency that demands immediate attention and intervention. Damage control surgery (DCS) has emerged as an alternative to traditional approaches for managing physiologically unstable and critically ill patients. This systematic review includes 16 articles published between 2015 and 2025, covering randomized controlled trials, observational cohorts, case reports, and guidelines. The inclusion and exclusion criteria focused on human studies published in English that addressed managing perforated diverticulitis with DCS. The Preferred Reporting It…

Anaerobic Gram-positive rod bacteremia is typically linked to organisms such as Clostridium perfringens or contamination, but can occasionally be caused by less common microorganisms. Here, we present a case of bacteremia caused by Christensenella hongkongensis, identified through 16S rRNA gene sequencing, in a 79-year-old man who developed a fever during hospitalization for diabetes education.

Lemmel syndrome is a rare cause of biliary obstruction resulting from the extrinsic compression of the distal common bile duct by a periampullary duodenal diverticulum in the absence of choledocholithiasis or malignancy. We present the case of a 62-year-old male patient with a history of hepatic steatosis and a recent laparoscopic cholecystectomy who was admitted with right upper quadrant pain and jaundice. Laboratory findings showed leukocytosis, cholestatic liver enzyme elevation, and hyperbilirubinemia consistent with severe cholangitis. Imaging revealed biliary ductal dilation without calc

Colonic diverticula are very common as asymptomatic findings on endoscopic examination. Diverticulitis as a complication occurs in ~4% of these patients with well-known further complications. Colocutaneous fistulas are very rare and are usually caused by percutaneous drainage procedures of abscess formations or as a complication of the natural disease. We present here a case report of a 70-year-old man who initially presented with signs of sepsis and later developed a colocutaneous fistula in the lumbar region.

Jejunal diverticulum perforation is a rare condition and presents diagnostic challenges. A 41-year-old male presented to the emergency room … oration and abscess was identified during the procedure. The pathological diagnosis confirmed acute diverticulitis with abscess formation, and malignancy was ruled out. The patient remained stable throughout his postoperative recovery and was discharged on the sixth postoperative day.

Acute left colonic diverticulitis (ALCD) is a common clinical condition encountered by physicians in the emergency setting. Clinical presentation of ALCD ranges from uncomplicated acute diverticulitis to diffuse fecal peritonitis. ALCD may be diagnosed based on clinical features alone, but imaging is necessary to differentiate uncomplicated from complicated forms. In fact, computed tomography scan of the abdomen and pelvis is the highest accurate radiological examination for diagnosing ALCD. Treatment depends on the clinical picture, the severity of patient's clinical condition and underlying …

Introduction Diverticular disease and anxiety disorders are common in the general population. Prior research on diverticular disease showed that these patients have an increased frequency of anxiety and depression. The objective of this study was to explore the impact of generalized anxiety disorder (GAD) on the outcomes of adult patients admitted with acute diverticulitis. Methods Using the National Inpatient Sample database from the year 2014 and International Classification of Diseases, Ninth Edition Revision, Clinical Modification (ICD-9 CM) codes, acute diverticulitis patients were select…
