Acute abdomen secondary to perforated jejunal diverticulitis: an unusual clinical presentation - case report.

INTRODUCTION: Jejunal diverticulosis is an uncommon condition, usually asymptomatic that could lead to severe complications such as perforation, associated with high mortality, and its presentation can mimic other acute abdominal or even urologic disorders, often requiring urgent surgery for both diagnosis and treatment. CASE REPORT: A 68-year-old woman with no significant medical history presented with sudden-onset severe colicky pain in the left lumbar and iliac regions, initially resembling renal colic. Laboratory tests revealed leukocytosis and elevated CRP. A non-contrast abdominal CT ru

Rare forms of small intestinal diverticulum and diagnostic features. A clinical case report.

INTRODUCTION: Small intestinal diverticulosis is a rare pathology compared with colonic diverticular disease and is often characterized by nonspecific clinical manifestations. Diagnostic difficulties increase significantly in emergency surgery, especially when diverticulitis mimics other acute abdominal conditions. OBJECTIVE: To present a clinical case of jejunal diverticulosis complicated by diverticulitis and intestinal paresis, highlighting the diagnostic challenges encountered in emergency surgical practice. MATERIALS AND METHODS: A retrospective analysis of the clinical course, diagnost

Jejunoileal diverticulosis, a rare cause of ileal perforation - Case report.

INTRODUCTION: Jejunoileal diverticulosis (JID) is a rare condition associated with nonspecific symptoms, consisting of acquired false diverticula. It frequently co-exists with colonic diverticulosis. Diagnosis is usually made incidentally or after complications. These include hemorrhage, obstruction and diverticulitis, with or without perforation. PRESENTATION OF CASE: 81-year-old man presented with a painful abdominal mass in the right lower quadrant (RLQ), diffuse abdominal discomfort and fever. Abdominal examination confirmed a well-defined mass in the RLQ without rebound tenderness. Labor

Acute Diverticulitis as an Atypical Presentation of Alpha-Gal Syndrome.

Alpha-gal syndrome (AGS) is an immunoglobulin E (IgE) mediated hypersensitivity reaction caused by tick bites. The body's immune response to tick bites can lead to the production of anti-alpha-gal IgE antibodies, disrupting the normal oral tolerance to food allergens. AGS typically results in delayed anaphylaxis after consuming red meat or certain medications and immediate anaphylaxis following tick bites; however, isolated gastrointestinal manifestations are increasingly being reported. We present a case of a 38-year-old female patient with a history of AGS who developed acute sigmoid diverti

Laparoscopic Resection of a Colovesical Fistula Secondary to Diverticulitis Using Near-Infrared Fluorescent Ureteral Catheters.

INTRODUCTION: A colovesical fistula is a rare complication most often caused by diverticulitis, typically requiring surgical repair due to the pressure gradient in which the intraluminal pressure within the colon exceeds that of the bladder. CASE PRESENTATION: We report a 49-year-old man with pneumaturia and fecaluria secondary to chronic sigmoid diverticulitis. Imaging demonstrated a colovesical fistula, and cystoscopy confirmed fecal discharge at the bladder dome. Laparoscopic sigmoidectomy with bladder repair was performed after preoperative placement of bilateral near-infrared fluorescent

Patient-centered online health information on diverticulitis: Assessing the readability, content, and overall quality.

BACKGROUND: This study assessed the quality, content, and readability of patient-centered online health information about diverticulitis. METHODS: Five common diverticulitis-related terms were searched on Google. The first 50 websites for each term were reviewed. Popularity, quality, core content, and readability were evaluated using Google Trends, the DISCERN instrument, core content domains based on American Society of Colon and Rectal Surgeons patient education, and an online readability calculator. RESULTS: Of 250 search results, 176 met inclusion criteria, yielding 98 unique websites. T

Primary Epiploic Appendagitis: A Mimicker of Abdominal Pain.

Epiploic appendagitis is a rare cause of acute lower abdominal pain. Epiploic appendices are fat-filled serosal outpouchings of the cecum and sigmoid colon. Primary epiploic appendagitis (PEA) is characterized by epiploic inflammation caused by torsion of the appendage leading to ischemia or thrombosis of the appendage draining vein. Secondary epiploic appendagitis occurs in association with other inflammatory conditions of the abdomen or pelvis, most commonly diverticulitis. PEA is an important clinical mimicker of more severe causes of acute abdominal pain, such as diverticulitis, appendicit

[Diverticular sigmoiditis: surgical treatment].

From 1966 to 1990, 226 consecutive patients were operated on electively for diverticular disease of the sigmoid colon. The indications for surgery were colovesical fistula or suspicion of residual abscess, existence of two or more previous attacks of acute inflammation, existence of chronic symptoms and suspicion of colonic carcinoma. Colonic resection with primary anastomosis was performed in 217 patients with a covering colostomy in 1 case only. The Hartmann procedure was performed in 9 patients with extension of the lesions to the rectum and/or high operative risk. One or more abscesses wer

[Actinomycosis of the sigmoid as obstructive space-occupying lesion of the pelvis. A case report].

Abdominal actinomycosis is a rare chronic infectious disease, which may mimic abdominal cancer, inflammatory bowel disease or diverticulitis. We report the case of a 46-year-old women with a large bowel obstruction caused by extensive abdominal actinomycosis. Colon contrast examination revealed a stenosis in the sigmoid colon, while abdominal ultrasound showed a stenosis of the left ureter with left hydronephrosis. Preoperative presumptive diagnosis was a carcinoma of the sigmoid colon. She required emergency surgery, which involved both resection and colostomy. As in most cases reported in th

Sigmoid Perforation Secondary to Accidental Ingestion of a Chicken Bone.

Presentation We describe an unusual presentation of sigmoid colon perforation secondary to an ingested chicken bone. Diagnosis The patient presented with a 4 day history of abdominal pain and distension. On examination there were signs of peritonism. Inflammatory markers were raised. Computed tomography revealed a linear density projecting through the wall of the colon. Treatment The patient underwent emergency laparotomy and a Hartmann's procedure. A chicken bone was found to be the causative foreign object. Conclusion Foreign body ingestion is an uncommon cause of sigmoid perforation which m

[Laparoscopic "fast-track" sigmoidectomy for diverticulitis disease in Germany. Results of a prospective quality assurance program].

BACKGROUND AND OBJECTIVE: The natural course of sigmoid colon diverticulitis during conservative therapy and the assessment of the perioperative morbidity after sigmoid colon resection are differently evaluated by surgeons and gastroenterologists. The "fast-track" rehabilitation accelerates the reconvalescence and reduces the rate of postoperative general complications after colorectal surgery. The results of the laparoscopic "Fast-track" sigmoidectomy should be examined within a quality assurance program to better evaluate the perioperative risks following surgical management of diverticuliti

Ileal Diverticulitis Leading to Small Bowel Obstruction: A Rare Case of Ileal diverticular Disease.

While small bowel diverticulosis, particularly, ileal diverticulitis is rare and often asymptomatic, it can lead to chronic vague symptoms or acute complications. Acute presentations may mimic appendicitis, small bowel obstruction, or colonic diverticulitis but can also appear with atypical symptoms, making diagnosis challenging. In many cases, definitive diagnosis relies on imaging such as abdominal CT, which can identify features like extraluminal air or mass lesions, though surgical exploration may be required in complex cases. Small bowel diverticula are structural outpouchings of the inte

Rectal prolapse: rational therapy without foreign material.

A number of operations are available for the correction of rectal prolapse. Rational judgement is required for the selection of the most appropriate procedure for each patient. Perineal rectosigmoidectomy offers a reasonable alternative to an abdominal procedure in patients who are elderly and debilitated. Simple suture rectopexy is our procedure of choice for an abdominal prolapse repair. We believe that simple rectopexy offers comparable results with less risk than procedures utilizing resection or foreign material. Colon resection should be reserved for those patients who require surveillan

Diverticular Pylephlebitis and Polymicrobial Septicemia.

Diverticulitis primarily affects the sigmoid colon and is often complicated by intra-abdominal abscesses and fistulas. Rarely, however, mesenteric venous thrombosis has been known to occur. Optimal management is still unclear. We report the first case of polymicrobial sepsis resulting from diverticular pylephlebitis, managed successfully with bowel rest, antibiotics, and anticoagulation.
