Advanced Care for Management of Microtia Using a Two-Stage Autologous Costal Cartilage Technique for Auricular Reconstruction: Surgical Method, Outcomes, and Future Directions.

Auricular reconstruction for microtia remains one of the most technically demanding procedures in craniofacial surgery. The authors present a refined, two-stage approach using autologous costal cartilage. This method, building on the work of Tanzer, Brent, Nagata, and Firmin, demonstrates high aesthetic fidelity and improved patient quality of life. The authors' technique incorporates innovations in framework symmetry, postoperative care, a standardised splinting protocol following the second stage of ear reconstruction, and outcome evaluation using validated health-related quality of life (HR

Relapsing polychondritis, induced in mice with matrilin 1, is an antibody- and complement-dependent disease.

Relapsing polychondritis is an autoimmune disease that affects cartilage in the ear, nose, and respiratory tract. A pathogenic immune response has been proposed and antibodies to several cartilage proteins are detected in sera from these patients. To investigate the role of the humoral immune response in relapsing polychondritis, we used the matrilin-1-induced relapsing polychondritis model. Mice deficient of B cells (muMT) and mice congenic at the complement factor 5, were immunized with matrilin-1, a cartilage-specific protein mainly detected in the tracheal cartilage. To investigate the bin

Tarsoconjunctival flap supplementation: an approach to the reconstruction of large lower eyelid defects.

In the presence of a full-thickness lower eyelid defect too large for reconstruction with a tarsoconjunctival flap alone, the posterior lamella is often reconstructed with a free graft of nasal septal cartilage and mucosa, ear cartilage, or hard palate mucosa. In this series, an alternative approach was taken: a tarsoconjunctival flap of maximum horizontal dimension was created to reconstruct the majority of the posterior lamella defect. A second reconstruction technique, such as a periosteal flap or a Tenzel semicircular flap, was then used to supplement the tarsoconjunctival flap and reconst…

Human immunodeficiency virus--associated non-Hodgkin's lymphoma presenting as an auricular perichondritis.

AIDS-related NHL is an aggressive neoplasm, usually of high or intermediate grade, frequently extranodal at initial treatment, and often the first manifestation of AIDS. Although complete remissions have been reported, they occur in only a minority of patients. We describe a patient with NHL of the external ear that masqueraded as an auricular perichondritis. This is the first case reported in which AIDS-related NHL first appeared in the ear, and this should alert physicians who treat patient with AIDS to be aware of the protean manifestations of this disease.

A proof-of-concept study of an albumin-based bilayered scaffold for cartilage regeneration.

Cartilage, particularly hyaline cartilage, is essential for structural and functional integrity in otorhinolaryngological region (nose, ears, larynx, and trachea) but exhibits limited regenerative capacity due to its avascular nature. Current clinical strategies, including microfracture, autologous chondrocyte implantation, and cartilage grafting remain limited by poor integration, donor site morbidity, and fibrocartilage formation rather than hyaline cartilage. In parallel, most clinically investigated scaffolds rely on xenogeneic collagen, raising concerns regarding batch-to-batch variabilit

Earfold: A New Technique for Correction of the Shape of the Antihelix.

An absent or poorly defined antihelix often plays a central role in the perception of the prominent ear. A wide variety of otoplasty techniques have been described over the last 50 years that aim to reshape, create, or enhance the definition of the antihelix, which can, in turn, help to reduce the prominence of an ear. In addition to conventional suture and cartilage-scoring techniques, a permanent implantable clip system (Earfold® ) has recently become available that is placed using a minimally invasive approach performed under local anesthesia. In this review, we summarize conventional otopl

[Ossicular reconstruction of the loss of the superstructure of the stapes].

At 43 patients the full or almost full reconstruction of the sound conductive system of the middle ear has been performed. The colemella from autogenous cartilage was positioned on the footplate of the stapes or the mallens was rotated on the footplate. In the 4 years after operation the negative functional results were found. The applied operations techniques have been criticised.

The addition of a conjunctival flap to a posterior lamella auricular cartilage graft: A technique to avoid corneal complications.

PURPOSE: To present the benefits of the addition of a conjunctival flap when correcting lower eyelid retraction using an auricular cartilage graft. METHODS: An auricular cartilage graft was obtained either from the concha o the scaphoid fossa. When preparing the receiving bed, the conjunctival incision was made 2 mm below the inferior margin of the lower eyelid tarsus, therefore, a conjunctival flap arising from the inferior border of the tarsus was obtained. The cartilage graft was placed in the lower eyelid. The inferior border of the graft was sutured to the retractors and conjunctiva usin

Salvage Eustachian Tube Cartilage Chip Insertion After Multiple Transnasal Shim Operations in Intractable Patulous Eustachian Tube.

This article presents 2 cases of extremely intractable patulous Eustachian tube following multiple transnasal shim insertion. These cases highlight the disadvantages of repeat transnasal shim operations, including enlargement of the Eustachian tube lumen, frequent dislocation, repeat surgery, recurrent middle ear infection, and shim misswallowing. The patients in these cases were successfully treated with Eustachian tube cartilage chip insertion through a postauricular approach. We describe the surgical technique and advantages of this promising management method.

Rolled auricular cartilage grafts for dorsal augmentation rhinoplasty.

We present a technique, which has not been previously described in the literature, of using rolled autogenous conchal cartilage for dorsal augmentation via an endonasal or external rhinoplasty approach. This technique gives greater dorsal height compared with the more common layering techniques. It is most appropriate in the minimally or moderately saddled nose.

Neonatal molding in deformational auricolar anomalies.

INTRODUCTION: Congenital auricular anomalies can be categorized either as malformational or deformational. The first are characterized by a partial absence of the skin or cartilage resulting in a constricted or underdeveloped pinna and require surgical correction. Deformations are characterized by a misshaped but fully developed pinna and are best treated by auricular molding. AIM: Authors want to present their case load in treatment of infants affected by deformational auricolar anomalies and describe their techniques using early splinting for congenital auricular deformities, like prominent

Endoscopic myringoplasty of chronic suppurative otitis media: comparison in patients with dry ears to ones with wet ears.

BACKGROUND: Chronic suppurative otitis media (CSOM) is characterized by recurrent otorrhea and tympanic membrane perforation. The selection of surgical technique and suitable time for myringoplasty remain variable and unclear. OBJECTIVES: This study aimed to compare the feasibility and effectiveness of the CSOM patients with dry and wet ears using endoscopic inlay and underlay myringoplasty. METHODS: Sixty-four patients with CSOM who underwent endoscopic underlay or inlay myringoplasty were studied retrospectively. Cartilage-perichondrium grafts were used to repair tympanic membrane perforat

Comparative Analysis of Cartilage Conduction Hearing Aid Users and Non-Users: An Investigative Study.

Clinical findings on cartilage conduction hearing aids (CCHAs) have gradually become clear; however, few reports include a large number of cases. This study included 91 ears from 69 patients who underwent CCHA fitting in our hospital. Their ears were divided into six groups (i.e., bilateral aural atresia or severe canal stenosis, unilateral aural atresia or severe canal stenosis, chronic otitis media or chronic otitis externa with otorrhea, sensorineural hearing loss, mixed hearing loss, and conductive hearing loss) according to their clinical diagnosis and type of hearing loss. Most clinical
