Clinical outcome of Niigata-Senami-Kyocera modular unconstrained total elbow arthroplasty for destructive elbow in patients with rheumatoid arthritis.

BACKGROUND: Total elbow arthroplasty (TEA) is a treatment option for destructive and painful unstable elbows in rheumatoid arthritis (RA). We evaluated the clinical outcomes of unconstrained TEA (Niigata-Senami-Kyocera modular system). METHODS: Seventy-five unconstrained TEAs were performed in patients with RA (mean age, 64 years; age range, 41-79 years; follow-up rate, 97%). Outcome measures included the Japanese Orthopaedic Association (JOA) functional evaluation score for the elbow joint (JOA score), range of motion, and arc. Bone ingrowth of the humeral component, the incidence of stress …

Total elbow arthroplasty: a prospective clinical outcome study of Discovery Elbow System with a 4-year mean follow-up.

BACKGROUND: Total elbow arthroplasty (TEA) is increasingly used for the treatment of advanced elbow conditions to reduce pain and improve function. However, TEA is still associated with a higher complication rate than total hip and knee arthroplasty despite advances in the design and surgical techniques. This prospective clinical study reports the outcome of the Discovery Elbow System (Biomet, Warsaw IN, USA), which has been in clinical use in the United Kingdom since 2003. METHODS: The study included a total of 100 Discovery Elbows (April 2003 to January 2010) with a minimum 2-year follow-up…

Functional latissimus dorsi island pedicle musculocutaneous flap to restore elbow flexion in replantation or revascularisation of above-elbow amputations.

Two total and one subtotal above-elbow amputations had replantation or revascularization for their severely damaged upper extremities followed by functional latissimus dorsi island pedicle musculocutaneous flap to restore elbow flexion. The mean follow-up was 68 months (range: 14 to 121 months). At final follow-up examinations, the patients had sufficient range-of-motion of their elbows with good strength. Restoring elbow function eliminates one of the most important limiting factors for above-elbow replantations. Functional latissimus dorsi island pedicle musculocutaneous flap is very reliabl…

Risk factor for elbow symptom manifestation in young baseball players with asymptomatic medial elbow abnormalities: a prospective cohort study.

Asymptomatic elbow abnormalities are relatively common in young baseball players, but the factors responsible are unclear. To prospectively identify risk factors related to symptom manifestation in asymptomatic elbow abnormalities, we recruited 573 baseball players (age: 7-14 years) at a pre-participation medical/physical examination in the preseason who were right-handed and had asymptomatic medial elbow abnormalities on ultrasound (US). Baseline preseason and postseason participant characteristics were assessed. A "symptomatic" elbow was defined as an elbow with medial elbow joint problems t…

What constitutes a clinically important change in Mayo Elbow Performance Index and range of movement after open elbow arthrolysis?

AIMS: This study aimed to determine the minimal detectable change (MDC), minimal clinically important difference (MCID), and substantial clinical benefit (SCB) under distribution- and anchor-based methods for the Mayo Elbow Performance Index (MEPI) and range of movement (ROM) after open elbow arthrolysis (OEA). We also assessed the proportion of patients who achieved MCID and SCB; and identified the factors associated with achieving MCID. METHODS: A cohort of 265 patients treated by OEA were included. The MEPI and ROM were evaluated at baseline and at two-year follow-up. Distribution-based MD…

Shoulder elevation and arm extension influence elbow joint loading during door-opening in total elbow arthroplasty: a musculoskeletal modelling study.

Total elbow arthroplasty (TEA) is used to treat end -stage elbow pathologies, yet implant failure is common due to excessive joint loading. This study compared elbow-joint loading, kinematics, and kinetics in TEA patients and healthy controls, and assessed the influence of elbow flexion-extension (FE) and shoulder elevation (SE). Seventeen participants (6 TEA, 11 controls) performed a door-opening task. Musculoskeletal modeling was used to estimate elbow kinematics, net joint moments FE and varus-valgus (VV)), and humeroulnar joint reaction forces (HUJRF). Statistical parametric mapping (SPM) …

Analysis of Factors Affecting Range of Motion Loss after Elbow Joint Release Surgery in Elderly Patients with Traumatic Elbow Stiffness.

AIM: Elbow joint release surgery is commonly used to treat elbow joint stiffness. Though it can restore elbow joint mobility, some patients may still experience range of motion (ROM) loss after surgery. Therefore, this study aims to explore the factors influencing ROM loss after elbow joint release surgery in elderly patients with traumatic elbow stiffness. METHODS: This retrospective study included 122 elderly patients with traumatic elbow stiffness who underwent elbow joint release surgery at Hanzhong Central Hospital from January 2023 to April 2024. The patients with range of motion loss w…

Comparing Static Stability of Native Elbow With Static Stability of Novel Bidirectional Ligament Reconstruction at Different Degrees of Elbow Flexion.

PURPOSE: The treatment of bidirectional ligament instability is proposed using a method that simultaneously tensions medial and lateral ligaments. Graft tension is maintained via plates that apply compression between the graft and bone. METHODS: We tested static varus and valgus elbow stability in six cadaver elbows with intact ligaments and capsules at five positions, and then created gross instability by dividing all soft tissue attachments. A ligament reconstruction was subsequently performed with and without nonabsorbable ligament augmentation. Elbow stability was measured and compared wi…

Single Assessment Numeric Evaluation Correlates with American Shoulder and Elbow Surgeons Score for Common Elbow Pathology: A Retrospective Cohort Study.

BACKGROUND: There are currently a variety of patient-reported outcomes (PROs) for elbow pathology, without any established gold standard. The Single Assessment Numeric Evaluation (SANE) is a single question assessment of the patient's perceived overall function compared with normal. The SANE score has been correlated with PROs from the shoulder and knee literature. PURPOSE: To correlate the SANE score for a variety of elbow pathologies with a traditionally reported elbow outcome measure, the American Shoulder and Elbow Surgeons-Elbow score (ASES-E). STUDY DESIGN: Cohort study (diagnosis); Le…

Serial Casting and Hinged Elbow Bracing in the Treatment of Fixed Flexion Deformity of the Elbow Following Obstetric Brachial Plexus Palsy.

BACKGROUND: Fixed flexion deformity (FFD) of the elbow is a recognized sequela of obstetric brachial plexus palsy (OBPP). Physiotherapy, stretching, and extension splinting are commonly used, but established contractures may persist despite these measures. This study evaluated the outcomes and complications of serial casting and hinged elbow bracing for OBPP-related elbow FFD. METHODS: A retrospective case series of 89 treatment courses in 73 patients with OBPP-related elbow FFD treated between 1996 and 2014 was performed. There were 80 serial casting courses, 8 hinged elbow brace courses, an…

Regional distribution prevalence of heterotopic ossification in the elbow joint: a 3D study of patients after surgery for traumatic elbow injury.

BACKGROUND: Heterotopic ossification (HO) is a common complication after elbow fracture surgery and can lead to severe upper extremity disability. The radiographic localization of postoperative HO has been reported previously. However, there is no literature examining the distribution of postoperative HO at the three-dimensional (3D) level. This study aimed to investigate 1) the distribution characteristics of postoperative HO and 2) the possible risk factors affecting the severity of postoperative HO at a 3D level. METHODS: A retrospective review was conducted of patients who presented to ou…

Biomechanical influence of pin placement and elbow angle on joint distraction and hinge alignment for an arthrodiatasis elbow-pin-fixator construct.

Stiffness and contracture of the periarticular tissues are common complications of a post-traumatic elbow. Arthrodiatasis is a surgical technique that uses an external fixator for initial immobilization and subsequent distraction. The two prerequisites for an ideal arthrodiatasis are concentric distraction (avoiding bony contact) and hinge alignment (reducing internal stress). This study used the finite element (FE) method to clarify the relationship between these two prerequisites and the initial conditions (pin placement, elbow angle, and distraction mode). A total of 12 variations of the in…
