Brucella endocarditis in a non-endemic area presenting as pyrexia of unknown origin.

A 67-year-old man with type 2 diabetes mellitus and hypertension since 7 years presented with a 3-month history of low-grade fever and malaise. Cardiac auscultation revealed the presence of an ejection systolic murmur in the primary aortic area. Most of the investigations for febrile illness were reported normal. His two-dimensional (2D) echocardiogram revealed a calcified aortic valve with mild aortic stenosis. In view of the prolonged fever and calcified aortic valve with mild aortic stenosis, a transoesophageal echocardiogram was performed, which showed small vegetation noted on right coron

The endocardial binary appearance ('binary sign') is an unreliable marker for echocardiographic detection of Fabry disease in patients with left ventricular hypertrophy.

AIMS: The binary sign, a binary appearance of the left ventricular endocardial border, was suggested to be an echocardiographic hallmark in diagnosing Fabry disease, a hereditary, lysosomal storage disorder. The aim of the present study was to examine the reliability of the binary sign as a screening tool to identify patients with Fabry disease. METHODS AND RESULTS: In total 309 subjects with an interventricular septum (IVS) thickness of ≥12 mm were investigated, of which 14 had a confirmed diagnosis of Fabry disease. Urinary globotriaosylceramide testing was used to rule out Fabry disease in

[Classification of cardiomyopathies and indication for endomyocardial biopsy revisited].

The first classifications of cardiomyopathies from 1980 and 1996 described them as heart muscle diseases, with dilated (DCM), hypertrophic (HCM), restrictive (RCM), arrhythmogenic right ventricular (ARVC), and nonclassifiable cardiomyopathies. Furthermore, the World Health Organization/International Society and Federation of Cardiology (WHO/ISFC) classification from 1996 listed among the specific cardiomyopathies inflammatory cardiomyopathy as a new and distinct entity, which was defined histologically as myocarditis in association with cardiac dysfunction. Infectious and autoimmune forms of i

Tampa Bay catheter: a new guiding catheter for endomyocardial biopsy via femoral approach.

Initially, endomyocardial biopsies were obtained almost exclusively using the jugular vein approach. Lately, the femoral vein route has gained popularity and in many centers, including ours, it is preferred. Despite this, guiding catheters specifically designed for endomyocardial biopsy via femoral vein approach are not available. Here, the experience with the Tampa Bay catheter, designed for endomyocardial biopsy using the femoral vein is described. From 1-1-89 to 1-31-90, a total of 486 endomyocardial biopsies were performed in 78 post-heart transplant patients (1-17, mean 6 per patient); 10

The use of evoked endocardial response for assessment of antiarrhythmic drug effects on myocardium.

The technique "asymmetric biphasic stimulation" which paces the heart and neutralizes the post-stimulus polarization at the electrode-tissue interface allows for the recording of the entire evoked endocardial response via a single electrode for both pacing and recording. Using this system the effects of antiarrhythmic drugs, procainamide and N-acetylprocainamide, on the myocardium were studied in 20 dogs. Before and during the five-step drug infusion, the evoked endocardial responses were recorded during bipolar and unipolar at the rates of 120, 150 and 200/min. The plasma concentration of the

Right heart catheter lesions: any significance?

In order to determine whether endocardial lesions that developed in relation to an indwelling catheter in the right heart had any significan … ve replacement if any patients were to survive the underlying disease. Traumatic perforation of the endocardium was uncommon. With the aid of careful indications and strict adherence to guidelines, the risk of fatality from these lesions is extremely unlikely.

[Endocardial fibroelastosis. Pathologic and clinical findings in 20 cases (author's transl)].

Pathologic and clinical findings in 20 infants with Endocardial Fibroelastosis are reported. Ten cases were considered as secondary Fibroelastosis due to left heart structural anomalies. Purpose was to find some clinical criteria to establish differential diagnosis between primary and secondary forms. QRS voltages in electrocardiogram were higher in primary forms and QRS axis in the frontal plane had more rightward deviation in secondary ones.

Cardiac amyloidosis. Diagnosis by transvenous endomyocardial biopsy.

Endomyocardial tissue, obtained from two patients presenting with restrictive cardiomyopathies, demonstrated amyloid infiltration. The percutaneous transvenous cardiac biopsy technic, using a modified Konno-Sakakibara cardiac bioptome, was safe and quick. Physical examination and catheterization data may not provide a definite differential diagnosis between restrictive and constrictive myocardial disease. Confirmation by biopsy of the cardiac amyloidosis assisted in providing optimum diagnostic and therapeutic care for these patients.

Ectopic Kidney Mimicking an Inferior Vena Cava Mass in a Patient with Cardiac Myxoma and Atrial Septal Defect.

An ectopic kidney (EK) and cardiac myxoma are two distinct conditions with no known direct association. Cardiac myxoma represents the most common benign primary cardiac tumor, arising from the endocardium. In contrast, an EK is a congenital anomaly characterized by one or both kidneys being located outside their normal position within the renal fossa. We present the case of a 27-year-old female patient who was evaluated for ischemic stroke and was found to have a left atrial myxoma originating from the interatrial septum (IAS) and prolapsing into the right atrium through an atrial septal defec…

Infective aortitis caused by Corynebacterium striatum in a patient with twice orthotopic heart transplantation.

BACKGROUND: Infective aortitis (IA) with vegetation formation is a rare but potentially life-threatening complication in patients with heart … l arterial embolisms. While transthoracic echocardiography (TTE) revealed normal cardiac valves and endocardium, transesophageal echocardiography (TEE) confirmed an intraluminal mass in the anastomotic site of ascending aortic. The patient declined surgical intervention but responded well to medical therapy. He got resolution of infection following 6 weeks of …

A Narrative Review on Endomyocardial Fibrosis: Unraveling an Under-Recognized Tropical Heart Disease.

Endomyocardial fibrosis (EMF) is a neglected tropical cardiomyopathy, primarily affecting children and young adults in low-income equatorial regions. Characterized by fibrotic thickening of the ventricular endocardium, EMF leads to restrictive physiology, atrioventricular valve dysfunction, and progressive heart failure. Despite being recognized for over half a century, its etiology remains poorly understood, with proposed contributors including malnutrition, parasitic infections, hypereosinophilia, and genetic predisposition. Diagnosis is often delayed due to overlap with other causes of hear…

Infective endocarditis presenting as meningitis: a case report.

UNLABELLED: Infective endocarditis (IE) primarily affects the endocardium of heart valves. Neurologic manifestations include strokes, intracerebral hemorrhages, meningitis, cerebral and spinal abscess, and mycotic aneurysms. Although rare, meningitis is a potentially life-threatening complication of IE, so physicians should be aware of this rare and fatal complication of IE. CASE PRESENTATION: Here, the authors present a case of a 53-year-old male who presented with bacterial meningitis secondary to IE. His blood culture was positive for methicillin-sensitive staphylococcal aureus. Echocardio…

A Rarely Reported Case of Enterococcus faecalis Bacteremia Causing Infective Endocarditis and Osteomyelitis.

Infective endocarditis (IE) is an infection of the heart valves or endocardium, usually due to the spread of infection through the blood. It can cause a varied range of symptoms, from being asymptomatic to reduced heart function, valvular abnormalities, embolization, or death. Enterococci are usually present as normal gut flora but can also cause bacteremia, urinary tract infections, or IE, especially in the elderly population. The source of enterococcal spread in most of the cases is unidentifiable and sometimes associated with the genitourinary tract or damage to the gut mucosa due to trauma…

E. coli endocarditis of the tricuspid valve.

Infective endocarditis (IE) is a focus of infection which effects the endocardium, specifically the heart valves or intra-cardiac devices. A 64-year-old male with gastric carcinoma and no prior cardiac history presented to the emergency room with altered mental status. Initial investigations showed the patient had a leukocytosis with a left shift. Blood cultures taken upon arrival eventually grew Esherichia coli, thought to be from the urinary tract, although initial urinalysis was delayed until after initiation of antibiotics. Electrocardiogram showed sinus bradycardia with frequent premature…
