Pathology of eosinophilic fasciitis and its relation to polymyositis.

The anatomical substrate of eosinophilic fasciitis (EF) was studied in 15 muscle biopsy specimens of this disease, six of which included the dermis and subcutaneous tissue. As controls, 94 postmortem muscle specimens from patients dying of non-muscular diseases were used. Of these 94 specimens, 22 (23.4%) showed practically no deep fascia and 72 specimens showed a single dense bundle of collagen with no distinction between deep fascia and epimysium. The 15 specimens of EF showed thickening and inflammatory infiltration of varying degrees in the deep fascia, epimysium, perimysium, endomysium an…

[Clinical aspects and diagnosis of diffuse eosinophilic fasciitis].

The authors presented an analysis of 10 cases of eosinophilic fasciitis (EF): 6 men and 4 women aged 18 to 55. In 4 patients the disease followed excessive physical exercise. Pronounced induration of the limb tissues with the development of flexion contractures, hypereosinophilia prevailed in the clinical picture of the disease, morphology showed a picture of fasciitis. A positive time course was noted in the presence of corticosteroid therapy which in 2 patients was combined with azathioprine and carbohemoperfusion. Besides, there was a patient with the combination of the classical signs of s…

Necrotizing fasciitis: early awareness and principles of treatment.

Necrotizing fasciitis is a serious life-threatening disease. A series of 11 patients with this disease is described, with emphasis on clinical diagnosis, initiating factors, associated diseases, etiologic pathogens and treatment. The following conclusions can be drawn from this report: Escherichia coli was the most prominent single pathogen, hyperbaric oxygen did not show any added beneficial effect, and diabetes mellitus did not affect morbidity and mortality. There is no doubt that aggressive and radical surgical excision and repeated debridement, combined with i.v. antibiotics and hyperalim…

Necrotizing fasciitis of vulvar origin in diabetic patients.

Necrotizing fasciitis is a rare, rapidly progressive, and often fatal infection of the superficial fascia and subcutaneous tissues. The integrity of the deep muscle fascia is usually not breeched, thus limiting the depth of involvement. Centrifugal spread within the planes of the superficial fascia and subcutaneous tissues is characteristic. Patients with diabetes mellitus constitute the group most vulnerable to necrotizing fasciitis, and a vulvar or perineal origin is associated with particularly high mortality. The authors report four such patients. Other apparent predisposing factors are ad…

Diffuse (eosinophilic) fasciitis. A series of six cases.

Six patients with diffuse fasciitis are described. Their ages range from 12 to 45 years. Five had a typical clinical illness which responded well to corticosteroid therapy but one patient relapsed a year after discontinuation of corticosteroids. One patient had severe skin ulceration resistant to corticosteroids, cytotoxics and plasma exchange and was further complicated by the development of life threatening local and generalised sepsis. Eosinophilia of the blood was not present in any of the cases and not unduly prominent in biopsies of the affected tissue, but characteristic fascial thicken…

Usefulness of computerized tomography in evaluating necrotizing fasciitis.

A 54-year-old man was seen with what appeared to be cellulitis of the left lower extremity. Roentgenograms showed no evidence of gas in soft tissues. Two days later films revealed gas in soft tissues of the leg, but not in the thigh. Computerized tomography showed pockets of gas in the lateral fascial planes of both leg and thigh. Surgical exploration revealed extensive necrotizing fasciitis. CT scanning provided a much more accurate picture of the extent of infection than did standard radiographs.

[Respiratory distress syndrome in necrotizing fasciitis of the hand].

A case is reported of acute respiratory distress syndrome occurring during septic shock complicating necrotizing fasciitis of the hand. It is emphasized that, although this kind of evolution is largely unknown, it is logical for this complicated infective pathology. As the presence of septic shock contraindicated the use of positive end-expiratory pressure, the evolution of the respiratory failure could only be of a fulminating nature, and the patient died.

Necrotizing fasciitis of the male genitalia (Fournier's gangrene).

Necrotizing fasciitis of the male genitalia was first described 100 years ago. Although the mortality of this condition has generally been high, the last few years have seen dramatic therapeutic improvements. This paper details the treatment of necrotizing fasciitis in two patients and describes a plan of management, which includes early diagnosis, vigorous antibiotic treatment and radical débridement of the necrotic tissues, that should improve the prognosis of the condition.

Generalized morphea with peripheral eosinophilia, fasciitis and myositis.

The distinctions between eosinophilic fascitis (Shulman's syndrome) and scleroderma may sometimes be unclear. We describe a patient with generalized morphea who also had peripheral blood eosinophilia, fibrosis and inflammation of fascia, and hyper-globulinemia, features usually attributed to Shulman's syndrome. The latter diagnosis should be restricted to those patients whose history, clinical and laboratory findings, and response to therapy closely resemble Shulman's original description. Scleroderma and eosinophilic fasciitis may represent slightly differing aspects of the same disease spect…

Eosinophilic fasciitis. Review and report of six cases.

Six patients with eosinophilic fasciitis are presented. This syndrome is characterized by indurative swellings of arms and legs, with rapidly progressing difficulties in extending elbows, wrists, and fingers, and often limited motion of shoulders and ankle joints. Pain when contracting muscles, and weakness of proximal muscles and hand grip are common features. The frequent occurrence of localized skin lesions has presented differential diagnostic difficulties to systemic sclerosis and to polymyositis. Visceral involvement and Raynaud's phenomena, however, are absent or mild. Blood eosinophili…

[Shulman's syndrome: fasciitis with eosinophilia, pseudoscleroderma with eosinophilia].

The authors report two cases of the Shulman's syndrome. Twenty one cases of this new entity are found in the literature. The main signs are the acute painful onset after an unusual physical exertion, the development of fasciitis, and (or) myositis, and (or) scleroderma of limbs, the absence of Raynaud's phenomenon, visceral involvement, constant eosinophilia and hypergammaglobulinemia; the prognosis is good with an improvement sometimes spontaneous or coincident with prednisone therapy. One of the observations is interesting by the association with Gougerot-Sjogren's syndrome, and a familial c…

Necrotizing Fasciitis After Cesarean Section: A Case Report.

Necrotizing fasciitis (NF) is a rapidly progressive, life-threatening soft-tissue infection requiring prompt diagnosis and aggressive surgical intervention. Although approximately 20% of NF cases arise from postoperative infections, its occurrence after cesarean delivery is extremely rare, with an estimated incidence of 0.18%. We report a case of a 35-year-old primigravida with no significant medical history who underwent emergency cesarean section for non-reassuring fetal status. On postoperative day (POD) 6, she developed fever, elevated inflammatory markers, and uterine tenderness consisten…

Eosinophilic fasciitis following postpartum: A rare case report.

Eosinophilic fasciitis is a rare connective tissue disease characterized by increased levels of peripheral blood eosinophils and infiltration in the myofascia. The diagnosis of eosinophilic fasciitis is primarily based on clinical manifestations, laboratory tests, and pathological biopsy, with the exclusion of other possible causes of fasciitis and eosinophil increase. Herein, we report a case of postnatal eosinophilic fasciitis occurring in a woman in her 20s, who developed the condition in her right forearm. The patient, who was postpartum, showed considerable recovery after conservative tre…

Pyocystis Causing Abdominal Wall Abscess and Necrotising Fasciitis.

Pyocystis is a rare but known complication of the defunctioned bladder. We report a case of a female patient with long-term bilateral nephrostomies who was initially thought to have cystitis but later found to have pyocystis causing a vesicocutaneous fistula and an anterior abdominal wall abscess. It progressed to necrotising fasciitis, necessitating emergency drainage and debridement. This yielded 400 ml of pus, and she was found to have a defect in the anterior rectus sheath leading straight to the bladder. This report highlights the importance of maintaining a high index of suspicion for py…

Pyoderma Gangrenosum Mimicking Necrotizing Fasciitis: A Case Report.

Pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis that can mimic other severe conditions, complicating diagnosis. We report a case of PG of the knee initially mistaken for necrotizing fasciitis (NF) in a 68-year-old woman who presented with knee swelling, erythema, papules, and pustules following a fall. Despite antibiotic therapy and surgical debridement for suspected NF, the patient's fever persisted, and laboratory markers (elevated WBC and C-reactive protein) did not improve. Histopathology revealed a dense neutrophilic infiltrate, and negative cultures ruled out infection, leadi…

Bilateral autologous penetrating keratoplasty following periorbital necrotising fasciitis.

The case report describes a rare instance of a man in his 70s with periorbital necrotising fasciitis (NF) who underwent bilateral autologous penetrating keratoplasty. NF is an acute infection that can lead to severe complications, including vision loss. The patient presented with severe facial swelling and necrosis of the right eyelid, treated with surgical debridement and antibiotics. Post-surgery, he lost vision in the right eye due to compressive optic neuropathy and developed exposure keratopathy, which was managed with tarsorrhaphy. His left eye had poor vision due to herpetic corneal sca…

Juvenile eosinophilic fasciitis: a single center case series.

BACKGROUND: Eosinophilic fasciitis (EF) is a rare disease characterized by skin induration and musculoskeletal abnormalities. Diagnostic criteria for EF are based on adult populations. There is a need to expand the literature on EF in children due to limited reported cases and potential differences compared to adults. METHODS: We conducted a retrospective review of medical records for six pediatric patients diagnosed with EF at our institution between November 2011 and April 2023. Inclusion criteria required patients to be under 18 years of age at the time of diagnosis and to have confirmed d…
