Addressing the specificity of affective startle modulation: fear versus disgust.

Previous research is equivocal regarding whether startle reflex eyeblink is specifically sensitive to the emotion of fear, or is more generally influenced by the valence dimension of emotion. Thirty-five undergraduate participants (17 male) viewed 60 pictures from five affective categories (pleasant, neutral, fear, disgust-blood, and disgust-other). Bilateral eyeblink EMG responses to acoustic startle probes were assessed during 2/3 of picture presentations, and 1/3 of intertrial intervals. Left corrugator and levator EMG responses to no-startle pictures were also examined. Supporting an emoti…

Scopolamine and Pavlovian fear conditioning in rats: dose-effect analysis.

Muscarinic-cholinergic antagonism produces learning and memory deficits in a wide variety of hippocampal-dependent tasks. Hippocampal lesions produce both acquisition deficits and retrograde amnesia of contextual fear (fear of the place of conditioning), but do not impact fear conditioning to discrete cues (such as a tone). In order to examine the effects of muscarinic antagonism in this paradigm, rats were given 0.01 to 100 mg/kg of scopolamine (or methylscopolamine) either before or after a fear conditioning session in which tones were paired with aversive footshocks. Fear to the context and…

Bihippocampal damage with emotional dysfunction: impaired auditory recognition of fear.

A right-handed man developed a sudden transient, amnestic syndrome associated with bilateral hemorrhage of the hippocampi, probably due to Urbach-Wiethe disease. In the 3rd month, despite significant hippocampal structural damage on imaging, only a milder degree of retrograde and anterograde amnesia persisted on detailed neuropsychological examination. On systematic testing of recognition of facial and vocal expression of emotion, we found an impairment of the vocal perception of fear, but not that of other emotions, such as joy, sadness and anger. Such selective impairment of fear perception …

Reply to Ley's "dyspneic-fear theory explains hyperventilatory panic attacks".

In a critique of the study by Carr, Lehrer and Hochron (1992), Ley (Behaviour Research and Therapy, 32, 109-111, 1994) attributes the lack of confirmation for his dyspneic-fear theory among panic disorder patients to a basic flaw of that study: Ss were selected according to DSM-IIIR criteria. Ley argues that dyspneic-fear theory pertains exclusively to hyperventilatory (characterized by intense dyspnea) panic attacks not to all attacks that come under the classification of DSM-IIIR. We address the premise concerning the selection of Ss and argue that cognitive theory continues to offer a more …

Fear of hypoglycemia in type 1 (insulin-dependent) diabetic patients.

A clinical-psychological study was carried out in 224 insulin-dependent diabetic patients, distributed into two groups according to the frequency of hypoglycemic episodes. Group. A included 124 patients, 65 men and 59 women (mean age 35 +/- 18 years), who reported frequent episodes of hypoglycemia (HG). In the 100 patients of group B (46 men and 54 women with a mean age of 37 +/- 10 years), the HG episodes were only occasional. The degree and type of anxiety were evaluated using: (1) a semistructured interview to determine the HG frequency and intensity; (2) the anxiety scale for HG, with two

Emotional imagery in simple and social phobia: fear versus anxiety.

In a first study, phobic volunteer subjects (N = 60) reacted psychophysiologically with greater vigor to imagery of their own phobic content than to other fearful or nonaffective images. Imagery heart rate responses were largest in subjects with multiple phobias. For simple (dental) phobics, cardiac reactivity was positively correlated with reports of imagery vividness and concordant with reports of affective distress; these relationships were not observed for social (speech) phobics. In a second study, these phobic volunteers were shown to be similar on most measures to an outpatient clinical…

Measuring fear of cancer recurrence in survivors of childhood cancer: Development and preliminary validation of the Fear of Cancer Recurrence Inventory (FCRI)-Child and Parent versions.

OBJECTIVE: Fear of cancer recurrence (FCR) is a common and distressing psychosocial concern for adult cancer survivors. Data on this construct in child survivors is limited and there are no validated measures for this population. This study aimed to adapt the Fear of Cancer Recurrence Inventory-Short Form (FCRI-SF) for survivors of childhood cancer aged 8-18 years (Fear of Cancer Recurrence Inventory-Child version [FCRI-C]) and their parents (Fear of Cancer Recurrence Inventory-Parent version [FCRI-P]) to self-report on their own FCR and to examine the initial psychometric properties. METHODS…

Development and validation of the multidimensional version of the fear of self questionnaire: Corrupted, culpable and malformed feared possible selves in obsessive-compulsive and body-dysmorphic symptoms.

In recent years, cognitive-behavioural models of OCD have increasingly recognized the potential role of feared possible selves in the development and maintenance of OCD, while simultaneously re-examining factors that have historically been linked to self-perceptions in OCD. The current article describes the development and validation of a multidimensional version of the Fear of Self Questionnaire (FSQ-EV) in a non-clinical (N = 626) and clinical OCD sample (N = 79). Principal component analyses in the non-clinical sample revealed three conceptually and factorially distinct components revolving…

Fears of receiving compassion from others predict safety behaviour use in social anxiety disorder over and above fears of negative self-portrayal.

Why do people with social anxiety disorder (SAD) engage in the use of safety behaviours? While past research has established that fears of negative self-portrayal are strongly associated with safety behaviour use in SAD, no research to date has investigated the potential role of fears of receiving compassion. Both types of fears could motivate those with SAD to engage in safety behaviours in order to keep others at a distance. In the present study, 150 participants with a clinical diagnosis of SAD completed measures of fears of negative self-portrayal, fears of receiving compassion, and safety…

Are Fear Learning Processes Altered in Obsessive-Compulsive Disorder, Social Anxiety, and Specific Phobia? Insights From the Late Positive Potential, Fear-Potentiated Startle, and Ratings.

Fear learning processes are often considered underlying mechanisms in the development and maintenance of anxiety- and stress-related disorders. However, limited attention has been paid to whether these changes are shared across disorders or certain symptoms. In this context, transdiagnostic research on symptom dimensions is especially relevant, as it addresses the significant symptom overlap and heterogeneity observed in these disorders. In the current study, we investigated the late positive potential, fear-potentiated startle, and subjective ratings (US-expectancy) in a transdiagnostic sampl…

Reductions in movement-associated fear are dependent upon graded exposure in chronic low back pain: An exploratory analysis of a modified 3-item fear hierarchy.

OBJECTIVE: To explore the effectiveness of a modified fear hierarchy on measuring improvements in movement-associated fear in chronic low back pain. METHODS: A modified 3-item fear hierarchy was created and implemented based on principles of graded exposure. This study was an exploratory analysis of the modified 3-item fear hierarchy from a larger clinical trial data set. Both groups received pain education and exercise, either bodyweight or strength training. Both groups performed item one on the hierarchy, the squat. Only the strength training group performed item 2, the deadlift. Neither g…

Effect of D-cycloserine and valproic acid on the extinction of reinstated fear-conditioned responses and habituation of fear conditioning in healthy humans: a randomized controlled trial.

RATIONALE: Although the effects of D: -cycloserine (DCS) and valproic acid (VPA) on the facilitation of the extinction of fear-conditioned memory have been elucidated in animals, these effects have not been clearly confirmed in humans. OBJECTIVE: This study aimed to determine the effect of DCS (100 mg) and VPA (400 mg) on the facilitation of the extinction and acquisition of fear-conditioned memory in humans. METHODS: We performed a randomized, blind, placebo-controlled, four-arm clinical trial in 60 healthy adults. Visual cues and electric shocks were used as the conditioned stimulus (CS) a…

Behavioural avoidance to imagined conditioned stimuli in differential fear conditioning.

The underlying mechanisms of how a fear is born in our imagination is presently unclear. The current study used differential fear conditioning to assess how a fear is created in one's mind and how behavioural avoidance interacts with imagined fear. This study contained an Acquisition, Behavioural Avoidance, and Transfer phase. During the Acquisition phase, participants combined instructions such that whether they heard "imagine yellow" (colour instruction) or "imagine horizontal" (orientation instruction) they were to imagine yellow horizontal lines (CS+) and likewise for the "imagine blue" an…

Model-Free and Model-Based Learning in Human Fear Conditioning.

Learning to predict threat and to adapt behavior accordingly is critical to human survival. This type of learning is governed by distinct systems in the brain. Model-free learning is reflexive and computationally efficient, allowing for rapid responses based on past experience, but is also resistant to environmental change. In contrast, model-based learning is more flexible and relies on an internal model of the environment that can be updated without direct experience. To enhance the translational value of human fear-conditioning research and facilitate the interpretation of fear-reduction in…

Dissociable Thalamocortical Circuit Disruptions During Contextual Fear Renewal in PTSD.

OBJECTIVE: Post-traumatic stress disorder (PTSD) is marked by impaired contextual modulation of fear, leaving patients vulnerable to symptom return after extinction-based therapy. The thalamus is theorized to coordinate hippocampal-prefrontal circuits during contextual updating. Yet, its role in PTSD remains uncharacterized. We examined the contribution of the medial mediodorsal thalamus (MDm) to extinction-recall and fear renewal and its association with symptom severity. METHODS: 425 participants completed threat renewal and 524 extinction-recall paradigms during fMRI (threat renewal: 189 h…

Intrinsic connectivity networks during fear and safety learning in youth.

BACKGROUND: Recent studies have focused on characterising the brain's intrinsic connectivity networks during fear learning, going beyond traditional brain activation analyses, to study the acquisition of fearful associations as a model for clinical anxiety onset. AIMS: Given the sharp increase in prevalence of affective disorders during puberty, a period marked by heightened affective sensitivity and fronto-amygdala circuitry development, the current study compares the connectivity within and between the salience network, executive control network and default mode network throughout fear lear…

Late sleep enhances fear extinction recall in posttraumatic stress disorder.

Sleep disturbances are a key feature of posttraumatic stress disorder (PTSD) and is implicated in extinction (i.e., previously threatening stimuli are now safe) and safety (i.e., stimuli signalling absence of threat) memory consolidation. This study examined whether early sleep versus late sleep impacts extinction and safety recall, indexed by skin conductance response (SCR) and threat expectancy ratings, in trauma-exposed adults with and without likely PTSD. Seventy adults (M age 25.78 ± 6.71 years; 32.86% with likely PTSD) completed a two-day fear conditioning paradigm. Following extinction …

Addressing fear of breast cancer recurrence through process-oriented hypnosis.

Fear of cancer recurrence is a common and understandable reaction for a woman with a history of breast cancer. A certain amount of fear about cancer recurring - in other words, a healthy protective concern for the body - may help a woman take the initiative to get continuing care and drive her to make healthy lifestyle changes. All too often though, a woman's fears and her reactions to them can dominate her life long after the cancer itself has been successfully treated. This article briefly describes clinically relevant "fear of cancer recurrence" (FCR) and then discusses how process-oriented…
