[A rare case of bone metastasis from gastro-intestinal stromal tumour: place of radiotherapy].

Gastro-intestinal stromal tumours are the most common mesenchymal neoplasms of the gastrointestinal tract. Their usual metastatic sites are the liver and the peritoneum, but gastro-intestinal stromal tumours rarely metastasize to the bones. We report the case of a 56-year-old male presenting with bone lesions six years after initial surgical resection. We discuss through this paper the possibilities of management of these lesions and the place of radiotherapy.

Malignant peritoneal mesothelioma with lower extremity metastasis: PET/CT imaging.

Malignant peritoneal mesothelioma is usually confined to the abdomen and pelvis. Extra-abdominal and extrapelvic dissemination is very uncommon mostly at advanced stages of the disease. This rare clinical event may be related to contiguous spread through connecting pathways linking the abdomen and pelvis to the thorax and proximal lower extremity. The authors present a case of advanced malignant peritoneal mesothelioma with contiguous dissemination to the right thigh and proximal leg documented and monitored by F-FDG PET/CT imaging.

Clinical outcome of central conventional chondrosarcoma.

INTRODUCTION: Aim of this study was to analyze (1) survival, local recurrence (LR), and metastasis rates between the three histological tumor grades; (2) whether type of treatment and tumor site influenced prognosis for each histologic grade. METHODS: We retrospectively studied 296 patients with central conventional chondrosarcomas (CS) (87 grade 1, 162 grade 2, and 47 grade 3). The femur was the most common site (91 cases), followed by the pelvis (82) and other less frequent sites. Type of surgery was related with histologic grade. Margins were wide in 222 cases, marginal in 23, and intrales

Combination of radiofrequency ablation and intramedullary nailing for the treatment of femoral metastases: single-center, retrospective observational study.

BACKGROUND: Radiofrequency ablation (RFA) has gained attention as a palliative treatment for bone metastases, providing pain relief and local tumor control. While its use for axial lesions is well documented, its application in long bones remains limited due to concerns about post-ablation fractures. These risks may be mitigated through the combination of RFA with prophylactic intramedullary nailing (IMN). METHODS: Five consecutive patients with femoral metastases who underwent combined RFA and IMN, performed either as a single-stage procedure under intraoperative fluoroscopic guidance or as

Extraskeletal osteosarcoma misdiagnosed as heterotopic ossification after periprosthetic femoral fracture: A case report.

Extraskeletal osteosarcoma is a malignant tumor of soft tissue characterized by osteoid production and has a very low prevalence, comprising approximately 4% of all osteosarcomas and about 1% of all soft tissue sarcomas, and a total of about 350 cases have been reported until now. Heterotopic ossification is a pathological finding of bony tissue in soft tissue regions such as muscle, skin and subcutaneous tissue. We report a case of an 86-year-old woman with a history of total hip arthroplasty (THA), in which open reduction and internal fixation were done for periprosthetic femoral Fracture. T

Atypical inguinal malignant peripheral nerve sheath tumour with arteriovenous fistula of the left femoral nerve in a child.

We report a 9-year-old girl who developed a malignant peripheral nerve sheath tumour (MPNST) with an arteriovenous fistula arising from the left femoral nerve and adjacent to the iliofemoral vessels in the ipsilateral groin, but without infiltrating them. We describe the MRI and MRA findings. Although MPNST is relatively well known and widely studied, the location of this mass is unique in a child. The mass was surgically removed.

[Treatment of malignant or aggressive bone tumors with microwave induced hyperthermia].

Limb-sparing procedures have been well established for dealing with malignant bone tumors. Unfortunately, these procedures have different problems. We used an alternative operation combined with microwave-induced hyperthemia to modify the surgical methods. Thermotherapy with microwave intracorporeal irradiation was used to treat 112 patients with bone tumors. In this series, 79 had malignant tumors and 33 aggressive benigh tumors. Postoperatively, immune therapy was carried out regularly. The patients immunologic functions were monitored by assay of the subpopulation of T cells, IL-2 and sIL-2

Surgical management of metastatic disease of bone at the hip.

Concordant advances in cancer chemotherapy, radiotherapy, and surgical technique have been of apparent benefit to many cancer patients suffering from skeletal involvement at the hip. Consideration of the effects of underlying disease and prior treatment must be evaluated prior to developing a surgical plan. Acetabular involvement can be treated by either conventional or augmented total hip arthroplasty using protrusio rings and acetabular meshes, depending upon minor or major degrees of tumor destruction. Girdlestone resection arthroplasty for massive degrees of acetabular involvement yields o

Knee arthrography for the preoperative evaluation of juxta-articular masses.

Knee arthrography was performed on twenty-one patients with osseous and extraosseous masses about the knee. Twelve patients had arthrographic abnormalities. Joint involvement was accurately delineated in nine of the ten cases where it was present. Three other patients had small suprapatellar bursae with superior flattening, characteristic of noncommunicating suprapatellar synovial cysts. Fluoroscopic spot radiographs and conventional radiographs after injection of contrast material adequately defined the synovial recesses. Tomograms were needed whenever articular cartilage invasion was a possi

Popliteal artery occlusion secondary to exostosis of the femur.

Osteochondromas are the most common benign tumors of the bone and are usually asymptomatic. In rare cases, they can present as a cause of lower extremity vascular injury in young patients. We report a case of a 24-year-old man who presented with an acute onset of exercise-induced lower extremity claudication and was found to have a popliteal artery occlusion secondary to a femoral exostosis. The patient underwent an excision of the exostosis and resection of the occluded segment with primary reanastomosis of the popliteal and superficial femoral arteries. Successful treatment of patients with

Surgical treatment of metastatic tumors of the femur.

OBJECTIVE: To evaluate the patients functional outcome and pain control after resection of metastatic femoral tumors. MATERIAL AND METHODS: A prospective randomized clinical study was conducted, which included 26 cases of metastatic tumors of the femur with an associated pathologic fracture. These selected cases were randomly divided into two groups based upon the using of methylmethacrylate cement in fracture fixation. Group 1 (n=13) included all cases where the fractures were treated with bone cement augmentation. Group 2 (n=13) included all cases where the fractures were treated without bo

Allograft-allograft healing? Salvage of massive allografts after fracture.

Two cases are presented in which an allograft was inserted to salvage a previous allograft bone reconstruction that had followed resection for osteosarcoma. In each instance an allograft-allograft junction was created and, with time, healed. In the first case, a fracture of a distal femoral osteoarticular allograft was salvaged by adding a second allograft and a total knee arthroplasty. This construct preserved the majority of the allograft and allowed rapid rehabilitation. In the second case, an allograft arthrodesis of the knee was salvaged after fracture by replacing the distal portion of t

Long-term follow-up of custom cross-pin fixation of 56 tumour endoprosthesis stems: a single-institution experience.

AIMS: Aseptic loosening is a major cause of failure in cemented endoprosthetic reconstructions. This paper presents the long-term outcomes of a custom-designed cross-pin fixation construct designed to minimize rotational stress and subsequent aseptic loosening in selected patients. The paper will also examine the long-term survivorship and modes of failure when using this technique. PATIENTS AND METHODS: A review of 658 consecutive, prospectively collected cemented endoprosthetic reconstructions for oncological diagnoses at a single centre between 1980 and 2017 was performed. A total of 51 pa

An alternative method for facial resurfacing: supraclavicular skin prefabrication by perforator fascia flap.

Prefabrication of supraclavicular skin provides a useful source for flaps congruent with the face skin. Among various vascular sources that have been used for this purpose, anterolateral thigh fascia seems to represent a greater value because of having a long and strong vascular pedicle and negligible donor-site morbidity. In this regard, we present a technical report on using the lateral circumflex femoral artery perforator flap harvest technique in preparing an anterolateral thigh fascia flap for the prefabrication of the supraclavicular skin. The technique proved successful in resurfacing t

Periprosthetic metastasis: a rare cause for prosthetic loosening.

Malignant periprosthetic metastasis is a very rare yet important cause of painful joint arthroplasty. This study presents a case in which periprosthetic osteolysis following total hip replacement (THR) occurred as a result of metastatic mesothelioma. After a THR and revision THR, histopathology confirmed the diagnosis of metastatic mesothelioma. Primary lesion was identified using CT scan of the chest. Patient is receiving palliative chemotherapy.
