Drugs and Nutrients in Epilepsy: Vitamin B6 and the Ketogenic Diet.

Certain foods and specific drugs have been linked to epilepsy in the literature. Here, we query PubMed citations for the co-occurrence of epilepsy with foods and drugs, using a list of 217,776 molecules from the HMDB. Notably, the top associations with epilepsy include approved drugs and drug families, diagnostic markers, inducers, and vitamins. Drugs include fosphenytoin (40%), topiramate (37%), valproic acid (34%), hydantoin (20%), phenytoin (31%), carbamazepine (33%), carbamazepine-10,11-epoxide (40%), trimethadione (31%), gabapentin (14%), pregabalin (11%), flunarizine (7%), fenfluramine (…

Effects of monoamine releasers with varying selectivity for releasing dopamine/norepinephrine versus serotonin on choice between cocaine and food in rhesus monkeys.

Monoamine releasers constitute one class of candidate medications for the treatment of cocaine abuse, and concurrent cocaine-versus-food choice procedures are potentially valuable as experimental tools to evaluate the efficacy and safety of candidate medications. This study assessed the choice between cocaine and food by rhesus monkeys during treatment with five monoamine releasers that varied in selectivity to promote the release of dopamine and norepinephrine versus serotonin (5HT) [m-fluoroamphetamine, (+)-phenmetrazine, (+)-methamphetamine, napthylisopropylamine and (±)-fenfluramine]. Rhes…

The development of tolerance to drugs that suppress food intake.

Appetite suppressants have been available as weight-reducing aids for over 50 years. The first discovered was amphetamine, which was potent, but possessed undesirable side effects (it is a stimulant and elevates blood pressure). Subsequently, a variety of appetite drugs was developed, all structurally related to amphetamine, but mostly lacking unwanted side effects. Until recently, fenfluramine (FEN) was the most widely used; presently, sibutramine is the most commonly used appetite suppressant. While these appetite suppressants are effective at reducing hunger and food intake when given as a …

Antiobesity pharmacotherapy in the management of type 2 diabetes.

Obesity is a well-known risk factor for the development of Type 2 diabetes mellitus. The management of the obese diabetic patient remains a challenge for the clinician but, in any case, weight reduction should be considered as a key objective. In this respect, several antiobesity drugs have demonstrated potential. However, while fenfluramine and dexfenfluramine have been shown to promote weight loss and to directly improve insulin sensitivity, being two mechanisms contributing to better blood glucose control in obese Type 2 diabetic patients, they were recently withdrawn due to safety problems…

Obesity.

Obesity is a chronic disease, which similar to diabetes and hypertension, requires long-term treatment. The patient must be willing to make major changes in eating habits, lifestyle, and physical activity to achieve long-lasting results.

Effects of chronic benfluorex treatment on the activities of key enzymes of hepatic carbohydrate metabolism in old Sprague-Dawley rats.

Chronic effects of benfluorex on some parameters of carbohydrate metabolism have been studied in 24-month-old Sprague-Dawley rats. Treatment once a day for 14 days with 25 mg benfluorex per kg body weight lowered body weight, decreased circulating insulin and resulted in an increase in hepatic glycogen. Measurement of the activities of several important regulatory enzymes of hepatic carbohydrate metabolism showed a significant decrease in the activities of phosphoenolpyruvate carboxykinase and glycogen phosphorylase. The activity of glucose-6-phosphatase, on the other hand, was slightly increa

[A new therapeutic approach of associated metabolic diseases. 1 000 patients treated with benfluorex (author's transl)].

Multiple metabolic factors of atherosclerosis frequently associated in the same patient proves the interest of a multifactorial treatment of atherosclerosis. In the paper the authors studied the results of the treatment with benfluorex of 1 000 patients suffering one or more metabolic risk factors. After a 3 month treatment, a highly significant and simultaneous reduction of hyperlipidemia, hyperglycemia, hyperuricemia is observed. The treatment is the more efficient that initial values are higher and that the diet is better observed. The results remain significant even in patients who have no

A role for potassium channels in smooth muscle cells and platelets in the etiology of primary pulmonary hypertension.

Plasma serotonin levels are markedly elevated in patients with primary pulmonary hypertension (PPH) and platelet levels of serotonin are low. Furthermore, plasma serotonin levels remain elevated after bilateral lung transplantation, in the absence of any pulmonary hypertension. Dexfenfluramine can cause the anorexigen-induced form of PPH that is clinically and histologically indistinguishable from PPH. We find that dexfenfluramine releases serotonin from platelets and inhibits its reuptake. These observations suggest that serotonin might be involved in, or be a marker for, the mechanism respon…

Effect of dexfenfluramine on body weight set-point: study in the rat with hoarding behaviour.

The food hoarding behaviour paradigm (HB) was used to evaluate the modification of the set-point of body weight (BW) regulation induced by dexfenfluramine (D-FF) in rats fed with chow or previously made obese by a cafeteria diet for 51 days. Daily injections of D-FF at a dose of 7 mg/kg (i.p.) induced a 58.5 g shift-down of the BW set-point in a group of chow fed rats submitted to HB measurement, and induced an equivalent weight reducing effect in a control-BW group always fed with chow ad libitum. In the rats fed ad libitum with the cafeteria diet, the BW reducing effect of D-FF(7 mg/kg/day) …

Effects of benfluorex and fenofibrate treatment on mitochondrial and peroxisomal marker enzymes in rat liver.

Our results demonstrate that benfluorex at doses that are strongly hypotriglyceridemic does not increase hepatic peroxisomal enzyme activities, whereas fenofibrate at doses that are only slightly hypolipidemic induces a dramatic increase in the activity of these enzymes. Thus, the biochemical approach used in this study reveals that the hypolipidemic drug benfluorex does not belong to the class of hypolipidemic compounds known to induce hepatomegaly, hepatic peroxisome proliferation and hepatocarcinoma in rodents. Morphological studies should confirm the absence of peroxisomal induction.

[Cross-over study of benfluorex and a hypolipemic agent in diet-resistant types IV and II b hyperlipemia (author's transl)].

The effects of benfluorex and a hypolipemic agent were compared in 24 patients with types IV or II b hyperlipoproteinemia unimproved after 4 months diet, by means of a cross-over study with two periods of two months treatment separated by two months without treatment. Results showed similar efficacy for each treatment against lipid parameters; statistically significant variations in body weight (-0,8%; p < 0,0001), fasting blood sugar (-11,7%; p < 0.001), and of uric acid (-8,3%; P greater than or equal to 0,001) with benfluorex VS., respectively (-0,1%; P: NS), (+ 3,4%; P < 0.05), and (+ 1,3%
