Prenatal Diagnosis and Pregnancy Outcome Analysis of Polyhydramnios.

The aim of the study was to investigate the etiology and pregnancy outcomes in mothers with polyhydramnios through prenatal diagnosis and pregnancy outcome analysis of pregnant women with polyhydramnios. One hundred and thirty women were enrolled. Fifty pregnant women with polyhydramnios were categorized as the case group, and 80 pregnant women with normal amniotic fluid were categorized as the control group. The causes of polyhydramnios and the pregnancy outcomes were analyzed. Two cases had chromosomal abnormalities, seven had severe α-thalassemia, 15 had fetal anomalies, four had maternal-f

How and when to recommend delivery of a growth-restricted fetus: A review.

Clinicians consider a range of variables when formulating decisions regarding the diagnosis, monitoring plan, and ultimately the decision to recommend the delivery of a growth-restricted fetus. The differential diagnosis of a pathological fetal growth pattern is initially considered via the history, a physical and laboratory examination of the pregnant person, as well as a comprehensive fetal ultrasound examination. These factors allow a broad distinction between pre-existing disease in the pregnant person, constitutionally small normal growth, placenta-mediated Fetal Growth Restriction (FGR),

[Not Available].

Due to the responsibility for the mother and the unborn child, fetal surgery represents a challenging task for the anesthesiologist. Maternal changes during pregnancy have to be considered as well as the fetal physiology and the surgeon's needs. Main principles of the anesthesiological management of fetal surgery include the stabilization of the mean arterial pressure in order to preserve a sufficient placental blood flow, sustainment of an adequate oxygenation and ventilation as well as thorough temperature surveillance. Since the mothers are at risk for perioperative pulmonary edema, extende

Prolonged pregnancy. I. Observations concerning the causes of fetal distress.

During a 2-year prospective investigation of prolonged pregnancy in 727 women, 59 (8%) were delivered by cesarean section for fetal distress. This condition was diagnosed by means of electronic fetal heart rate monitoring in 47 of the women, and the patterns were unexpectedly characteristic of umbilical cord compression rather than uteroplacental insufficiency. Blinded sonar examinations were performed in 213 women, and the incidence of cesarean section for fetal distress as now described was significantly increased in those with oligohydramnios (two or fewer 1 cm pockets of amniotic fluid). Współczesne metody monitorowania dobrostanu płodu w ciąży powikłanej wewnątrzmacicznym zahamowaniem wzrastania.

Intrauterine growth restriction (IUGR) is one of the most important problems in current perinatology. The number of complications such as intrauterine fetal hypoxia, preterm and operative labours, intrauterine demises and neonatal deaths are signifcantly higher among pregnant women with IUGR. The proper monitoring and assesement of the fetal well-being are crucial to make the right decision about optimal time and mode of delivery.

Duplex Doppler systems in obstetric ultrasound.

Doppler ultrasound applications in normal and complicated pregnancies are presented in this article. Anatomy and pathophysiology of fetal placental blood flow are described. Potential pitfalls in the evaluation of fetal placental blood flow are explored. Doppler flow velocity waveform analysis combined with other clinical and physiologic information is valuable in monitoring of the high-risk pregnancy. Duplex technology may also be useful in the first trimester.

Paroxysmal tachycardia and cardioversion during pregnancy.

Synchronized direct current (DC) cardioversion was used successfully for treatment of a pharmacologically refractory supraventricular tachycardia during pregnancy. No maternal or fetal complications or ill effects occurred. Although few cases have been reported, a review of the literature supports the use of synchronized DC countershock therapy for the treatment of certain arrhythmias during pregnancy. Thorough knowledge of electrocardioversion techniques and of concurrent fetal monitoring is recommended.

Remote blood pressure monitoring in women at risk of or with hypertensive disorders of pregnancy: A systematic review and meta-analysis.

BACKGROUND: Remote blood pressure monitoring refers to an organized framework that either allows clinicians to review home-based blood pressure readings and institute management, or provide participants with clear instructions for contacting care teams when blood pressure readings are out of prespecified targets. With widespread uptake of telemonitoring and mobile health in recent years, such models of care have been increasingly described in the literature. OBJECTIVES: This study aimed to review remote blood pressure monitoring in pregnant and postpartum women who are at high-risk for or hav

A transducer for detection of fetal breathing movements.

The paper describes a new transducer for detection of fetal movements and sounds from the maternal abdominal wall. This transducer is based on an inductive principle. The compliance of the transducer should match the compliance of the maternal abdominal wall in order to detect the very weak acoustic signal caused by individual fetal breathing movements. The contact area of the transducer is supported on a membrane, the tension of which can be adjusted so as to match the compliances of tissue and transducer. The goal of this paper is to investigate detection of fetal movements in order to monit

The SAVE FGR study: Short term variation Analysis versus Visual Evaluation of cardiotocography in early-onset Fetal Growth Restriction to trigger expedited birth - study protocol for a stepped wedge cluster randomized trial.

BACKGROUND: Early-onset fetal growth restriction (eoFGR) is associated with high risks of adverse outcomes and a key dilemma in clinical obstetrical practice in determining the optimal timing of birth. Cardiotocography (CTG) is an important tool for antepartum fetal surveillance and typically assessed visually. Computerised CTG including short term variation (STV) analysis offers an objective analysis by quantifying the beat-to-beat variation in the fetal heart rate. Currently, there is a lack of randomized studies of sufficient power that examine whether birth intervention based on such quant

A model for fetal surgery through intrauterine endoscopy.

Video-endoscopic technology may offer new hope for the future of fetal surgery. Ten time-dated pregnant ewes (95 to 105 days; term, 145 days) were used to develop a model of fetal surgery through intrauterine endoscopy. Special 5-mm ports were designed with short, flexible, balloon-tipped shafts. The cannulas were inserted through purse-stringed hysterostomies. Amnioinfusion of warmed physiological fluid (Hartmann) was used as the work medium. Fetal heart rate and oxygen saturation (pulse oximetry), temperature, and amniotic pressure were monitored continuously through endoscopically placed se

Characteristics of fetal and maternal heart rate tracings during labor: A prospective observational study.

BACKGROUND: Fetal well-being is assured during labor and delivery with the employment of electronic fetal heart monitoring (EFHM). In uncommon instances, maternal heart rate (MHR) instead of fetal heart rate (FHR) can be the source of signals on monitors (signal ambiguity) leading to erroneous interpretation and management. Information about MHR characteristics are comparatively inadequate. We aim to analyze and compare MHR and FHR characteristics during the first and second stages of labor. METHODS: A prospective cohort study was conducted in a single tertiary care center during a one year p

Fetal QRS extraction from single-channel abdominal ECG using adaptive improved permutation entropy.

Fetal electrocardiogram (fECG) monitoring is crucial for assessing fetal condition during pregnancy. However, current fECG extraction algorithms are not suitable for wearable devices due to their high computational cost and multi-channel signal requirement. The paper introduces a novel and efficient algorithm called Adaptive Improved Permutation Entropy (AIPE), which can extract fetal QRS from a single-channel abdominal ECG (aECG). The proposed algorithm is robust and computationally efficient, making it a reliable and effective solution for wearable devices. To evaluate the performance of the
