Ankle lesions are problematic because of their frequency its anatomy and physiology are complex, disturbances of its functions are caused by the smallest c change of the anatomic relations. It is not able to put the fracture into line by the settled therapy; so these lesions are very frequent in our operative programme. the most important are the lateral maleolus -- its ligamentary part and the tibiofibular syndesmosis. The special emphasis is given to physiology and pathology of the ankle, the Weber's pathoanatomic classification of the ankle lesions is also given in this article.

Ankle fracture is common in active young males. Treating ankle fractures can be straightforward or much more complicated; treatment options include nonoperative management or open anatomical reduction with rigid internal fixation. Successful treatment will allow early mobilization to avoid complications. Inadequate treatment, either nonoperative or operative management, may result in malunited ankle fractures. However, malunited ankle fractures due to the delayed presentation are very rare. An 18-year-old male presented to the clinic with a history of twisting injury to his right ankle two yea

INTRODUCTION: Ankle syndesmosis injuries can be associated with a spiral mechanism on tibial shaft fractures. We are not aware if there is evidence of the incidence of ipsilateral syndesmotic lesson associated to high-energy tibial shaft fracture mechanisms. The aim of this study was to analyze the incidence of syndesmotic injuries associated with high-energy tibial shaft fractures.
METHODS: 233 diaphyseal tibial fractures operated between January 2007 and January 2017, the fracture patterns were analyzed, they were classified according to AO, the talocrural angle was measured, they analyzed

Functional treatment of fractures is a nonoperative technique used in intraarticular knee fractures and multifragment and multilevel fractures of the shaft of femurs since 1974. The basic principle of this technique is placement of the fractured extremity in the specially designed rehabilitation splint, with skeletal traction fixed above the ankle joint through both tibia and fibula. Traction reduces the fracture while stretching of muscles provides conditions favorable for callus formation. Exercises are first isometric and then isotonic. During exercises, muscle tone and moments of force in

BACKGROUND/AIM: Vascularized (VFG) and non-vascularized fibula grafts (NVFG) are used in reconstruction of bone defects after tumour resection. This study compared both autografts and their results, risk factors, and complications.
PATIENTS AND METHODS: Tumour resection and reconstruction by using VFG (n=17) and NVFG (n=36) were performed in 53 patients at our institute (range=3-65 years of age, mean: 21.2 ± 13.2 years) of which 24 were female. Malignant tumours were diagnosed in 26 patients (VFG=16 patients-94%). The mean follow-up was 14.9 years (range=1.5-43 years). Factors like consolidat

INTRODUCTION: Using non-vascularized fibula as autogenous graft has gained much success in reconstruction after tumor resection owing to its simplicity. Donor-site morbidity related to fibula deficiency includes valgus ankle, chronic pain and stress fractures of the ipsilateral tibia.
HYPOTHESIS: Growth potential before skeletal maturity is the most important factor promoting fibular regeneration after non- vascularized harvesting, and its decline with skeletal maturity causes failure of fibular regeneration.
PATIENTS AND METHODS: This is a retrospective study of the patients with benign bon

BACKGROUND: Pediatric lower extremity physeal fractures carry a risk of developing deformities. Most epidemiological evidence is over 25 years old, single institution, and lacks follow-up, while recent studies report variable results. Understanding their epidemiology and deformity risk is important for patient counseling and follow-up.
METHODS: The National Trauma Data Bank (NTDB) from 2016 was queried to describe the modern epidemiology of physeal fractures. This was contrasted with our 10-year experience of surgically treated deformities. Basic descriptive statistics, Chi-square analysis, p

OBJECTIVES: The posterolateral approach to the distal tibia is commonly used for stabilisation of ankle fractures as it allows good visualisation and direct reduction of the posterior distal tibia and malleolar fragments. This approach can also be used for internal fixation of an associated lateral malleolus fracture. The aim of our study is to describe the surgical anatomy of the peroneal artery (PA) in relation to the tibial plafond and the distal fibula; thereby suggesting a safe zone during proximal dissection of posterolateral approach.
METHODS: Using Computed Tomography Angiographic (CT

Background and objectives: In this study, the accuracy of point-of-care ultrasonography (POCUS) was compared to radiography (XR) in the diagnosis of fractures, the determination of characteristics of the fractures, and treatment selection of fractures in patients admitted to the emergency department (ED) due to trauma and suspected long bone (LB) fractures. Materials and Methods: The patients were included in the study, who were admitted to ED due to trauma, and had physical examination findings suggesting the presence of fractures in LB (humerus, radius, ulna, femur, tibia, and fibula). Tibialis posterior tendon rupture in the setting of pronation-type ankle fractures can lead to long-term debility as a result of chronic tendon dysfunction. This rare injury pattern presents a diagnostic challenge because thorough preoperative examination of the function of the tendon is limited by pain, swelling, and inherent instability of the fracture. As such, a high index of suspicion is necessary in ankle fractures with radiographs showing a medial malleolus fracture with an associated suprasyndesmotic fibula fracture. This report describes 3 cases of tibialis posterior tendon rupture as

CASE: A 56-year-old morbidly obese woman sustained a high energy motor vehicle collision resulting in an isolated Gustilo-Anderson type IIIA open pilon fracture of the right ankle, with massive distal tibial bone loss. Limb salvage was achieved through the novel use of an off-the-shelf axial titanium spine cage combined with a nonvascularized fibula autograft and hindfoot intramedullary nail arthrodesis.
CONCLUSION: This technique may offer a practical and durable alternative for limb salvage in cases of massive distal tibial bone loss, particularly in resource-constrained settings when acces

Fibular fractures are the third most common stress fractures in children and adolescents. Proximal fibular location is a very rare finding, with few reports in the literature and, frequently, careful investigations before a definitive diagnosis could be necessary. The authors report a case of an adolescent 13 years old soccer player with a proximal fibular fracture that was initially underestimated and misdiagnosed and ultimately confirmed as a stress lesion by MRI.

INTRODUCTION: Posttraumatic pediatric distal tibiofibular synostosis is a rare complication of fractures at this level, unreported at this time. Implications include limb length discrepancy, angular malalignment, and gait and biomechanical abnormalities.
MATERIALS AND METHODS: The authors present a case report of this complication in a 7-year-old child and review the relevant English literature. The patient was treated for a closed distal tibia-fibula fracture and developed a synostosis at that level, with valgus malalignment of the ankle joint. The synostosis was resected and latest follow-u

Bone deformities and fractures are common consequences of renal osteodystrophy in the dialysis population. Persistent hypophosphatemia may be observed with more frequent home hemodialysis regimens, but the specific effects on the skeleton are unknown. We present a patient with end-stage renal disease treated with frequent home hemodialysis who developed severe bone pain and multiple fractures, including a hip fracture and a tibia-fibula fracture complicated by nonunion, rendering her nonambulatory and wheelchair bound for more than a year. A bone biopsy revealed severe osteomalacia, likely sec
