Pollicization of the Second Metacarpal Based on Dorsal Metacarpal Arteries.

Blood supply to the index finger is maintained through volar (palmar arch) and dorsal (intermetacarpal arteries) vascular networks. In traditional index finger pollicization, blood supply is maintained on the volar palmar arch. In case of index finger loss at the metacarpophalangeal joint, remaining length of the second metacarpal is often not used for pollicization because the arc of rotation is limited on digital arteries. In this report, we present a surgical technique for the transfer of the index metacarpal to the thumb on the dorsal vascular network. This method adds no further morbidity

Linburg-Comstock syndrome involving four fingers: a case report and review of the literature.

We report a case of a 14-year-old boy who presented with the inability to independently move fingers on either hand. On physical examination, simultaneous flexion of the index, middle and ring fingers was noted when he actively flexed the distal joint of the thumb. A surgical exploration was performed and anomalous tendinous interconnections between the flexor pollicis longus-index flexor digitorum profundus (FDP) and index FDP-middle FDP were observed and removed. The middle- and ring-finger FDP were adhered by thickened tenosynovium and divided. The patient was able to flex the thumb and fin

The embryonic origins of site-specific arthritis.

The cellular basis for site-specific inflammation remains unclear. In human fingers, proximal interphalangeal (PIP) joints are preferentially affected by inflammatory arthritis, whereas distal interphalangeal joints are spared, providing a model to investigate the predilection of inflammation to distinct sites. Here we combine single-cell RNA sequencing, imaging and X-ray tomography to examine cellular composition, spatial organization and structure of finger joints during fetal development. PIP joints had a larger synovial volume and were enriched for PI16+ 'universal' fibroblasts. These cell…

New classification for septic arthritis of the hand.

The severity of septic arthritis of the hand and the prospects for restoration of joint function are determined by a complex of factors. Among them, the leading role belongs to local changes in tissue structures. This includes the destruction of articular cartilage and bone tissue with the development of osteomyelitis, the involvement of paraarticular soft tissues in the purulent process, and the destruction of the flexor/extensor tendons of the fingers. The currently missing specialized classification of septic arthritis could help in systematizing the diseases, determining treatment tactics,

Etiology, Evaluation, and Management Options for the Stiff Digit.

The stiff digit may be a consequence of trauma or surgery to the hand and fingers and can markedly affect a patient's level of function and quality of life. Stiffness and contractures may be caused by one or a combination of factors including joint, intrinsic, extensor, and flexor tendon pathology, and the patient's individual biology. A thorough understanding of the anatomy, function, and relationship of these structures on finger joint range of motion is crucial for interpreting physical examination findings and preoperative planning. For most cases, nonsurgical management is the initial ste…

Sensory electrical stimulation for suppression of postural tremor in patients with essential tremor.

Essential tremor is an involuntary trembling of body limbs in people without tremor-related disease. In previous study, suppression of tremor by sensory electrical stimulation was confirmed on the index finger. This study investigates the effect of sensory stimulation on multiple segments and joints of the upper limb. It denotes the observation regarding the effect's continuity after halting the stimulation. 18 patients with essential tremor (8 men and 10 women) participated in this study. The task, "arms stretched forward", was performed and sensory electrical stimulation was applied on four

Long-term evolution of cartilage abnormalities and osteophytes in the fingers of elite sport climbers: A cross-sectional 10-year follow-up study.

The sequelae of high mechanical stress to the proximal interphalangeal (PIP) and distal interphalangeal (DIP) joints of the fingers in elite sport climbers and its contribution to the development of osteoarthritis are still relatively unknown. The purpose of this study was to investigate the evolution of cartilage abnormalities of the PIP and DIP joints, as well as the progress of osteophytes, in the fingers of elite sport climbers with a minimum of 25 years of climbing history over the time period of the last 10 years. Moreover, their actual cartilage abnormalities and osteophyte occurrence w

The functional morphology of the accessory interosseous muscle in the gibbon hand: determination of locomotor and manipulatory compromises.

The evidence for two functional roles of M. accessorius interosseus can be adduced as follows: (1) abduction of the whole finger is clearly required to resist the force of the thumb against the index during pinch grasp (Fig. 4) and when greater resistance is applied to the food, activity increases in the muscle. (2) The muscle also flexes the metacarpophalangeal joint at the onset of grasp on the ladder rung. In hanging from the finger tips or from the cage top, with the metacarpophalangeal joints extended, the muscle goes silent. From the functional point of view, the name given by Huxley (18

Treating Proximal Interphalangeal Joint Dislocations.

Proximal interphalangeal (PIP) joint dislocation a common injury. Usually, concentric stable reduction can be achieved with closed reduction. Occasionally, PIP joint dislocations are irreducible and open reduction is necessary. Complications include prolonged splinting and delay in presentation with subluxation or persistent dislocation. Surgery is often recommended for contracture or joint reduction. Surgical techniques focus on contracture release, joint reduction, and range of motion. Techniques have evolved from primary repair to tenodesis and suture anchor reconstruction. Most studies on

[Thumb reconstruction with cryopreserved autogenous thumb skeleton and microneurovascular flap].

This paper describes a procedure for thumb reconstruction using autogenous cryo-conserved bone from the thumb itself. In a case of severe explosion injury, the thumb and index finger were amputated at the carpometacarpal joint level and the remaining fingers at the distal interphalangeal joint level. Severe soft tissue damage and lesion of both the superficial and deep palmar arterial arches rendered customary surgical procedures for thumb reconstruction impossible. The cryo-conserved bone was replanted following soft tissue healing and then covered with a free radial forearm flap. This proced

Hand reconstruction using heterotopic replantation of amputated index and little fingers.

In cases of severe segmental injury across the hand and wrist, but one or other fingers are still in peak condition, the fingers can be selected for replantation at the forearm bones to restore pinch function. Here we reported an unusual case with a severe crush-avulsion amputated injury to the right hand caused by a machine accident. We conducted hand reconstruction using heterotopic replantation of the amputated index and little fingers. During 19 months follow-up, the bone union healed well with satisfactory outcome. The interphalangeal and metacarpophalangeal joint of the fingers after the

Fractional Dolphin Tenotomy for Boutonniere Deformity: A Cadaveric Study.

PURPOSE: Terminal extensor tenotomy or Dolphin tenotomy, is a described treatment for the management of distal interphalangeal (DIP) joint hyperextension in chronic boutonniere deformity. The purpose of this study was to investigate the effects of incremental partial Dolphin tenotomy in correcting boutonniere deformity, with a focus on evaluating the improvement in DIP joint hyperextension deformity and documenting the development of iatrogenic mallet finger. METHODS: Thirty-eight fingers from 10 cadaveric hands were used. We created a boutonniere deformity in each digit by transecting the ce

A Pragmatic and Evidence-Based Approach to Mallet Finger.

A mallet finger is a common injury that results from a sudden flexion force on an extended distal phalanx or rarely, from hyperextension of the distal interphalangeal joint. Mallet finger can be purely tendinous or bony when associated with an avulsion fracture. The management of this injury is largely conservative with the use of a splint, although surgery may be indicated for select patients. There is little consensus on the indications for surgery or the suitable surgical technique. The aim of this review article is to provide a pragmatic and evidence-based approach to mallet finger that wi

Corrective Step-cut Osteotomy at the Affected Bone for Correction of Rotational Deformity Due to Fracture of the Middle Phalanx.

Malunion at the shaft of the middle phalanx yields less functional problems compared with malunion at the shaft of the proximal phalanx and metacarpal bones. In the present report, the patient sustained a minimally displaced fracture at the distal portion of the distal middle phalanx of the ring finger spraining the finger during playing flag football. Fracture was treated conservatively and fracture union was completed. However, the patient complained of functional problems in activities of daily living due to the malrotational deformity of the finger. We treated the malrotational deformity c

[Palmar luxation of the metacarpophalangeal joint. Report of a case and review of the literature].

A palmar dislocation of the metacarpophalangeal (MCP) joint of a little finger occurred in a 25-year-old man and was successfully treated by an open reduction. Only seven other cases were reported in the literature. The mechanism of the palmar MCP joint dislocation was assumed to occur by hyperflexion injury. The dorsal capsule of the MCP joint was thought the essential pathologic feature that prevented successful closed reduction. The mechanism and pathologic anatomy of the palmar dislocation of the MCP joint is discussed.

Traumatic divergent dislocation of the extensor tendons over the metacarpophalangeal joint.

Dislocation of the extensor tendon over the metacarpophalangeal joint (MCP) is rare. Involvement of the fifth ray is even more exceptional. The authors of the present study describe such a case that occurred in a professional musician. We report a divergent dislocation of both extensor tendons over the metacarpophalangeal joint of the fifth finger.

Outcomes of Small Joint Arthroplasty for the Rheumatoid Hand.

The number of patients presenting to the hand surgeon with severe rheumatoid disease has decreased with the introduction of more effective medical management. However some rheumatoid patients will undergo disease progression despite this. MCPJ and PIPJ replacement can be effective surgical treatments in these patients but patient selection is crucial. Patients undergo surgery hoping to achieve improvements in pain, hand function and appearance. Multiple types of implant have been introduced over the years; however, silicone remains the most reliable option in terms of survivorship, complicatio
