Congenital fistula of the palate.

Four cases of congenital fistula of the palate are presented. All four patients had a fistula which was situated in the vault with a bifid uvula, submucous separation of the palatal muscles, deformities of the palatal plates and unilateral cleft lip. Velopharyngeal incompetence appeared in primarily treated children. The aetiology and surgical treatment of the congenital defect are discussed.

Enterocutaneous fistula: a reconstructive dilemma.

Surgical repair of enterocutaneous fistulae in Crohn's disease may result in large skin defects of the anterior abdominal wall. We present a case in which a large defect was managed with reconstruction using a pedicled rectus abdominis myocutaneous flap in a single procedure. The case highlights the technical challenge of such a case and the value of a joint surgical approach between plastic and colorectal services.

Aberrant second branchial cleft fistula.

Second branchial cleft cysts and sinuses rarely present diagnostic problems to the pediatric otolaryngologist as their course is usually predictable based on consistent embryologic development. However, we evaluated two fistula tracts that did not fit the classic description of second branchial tract fistulas. Upon radiographic and intraoperative evaluation, their eventual course ending in the tonsillar fossa was identified. Realizing the potential for aberrancy and using preoperative radiographic evaluation will assist the surgeon in the excision of these developmental anomalies with little r…

[Cholecysto-colic fistula: laparoscopic treatment].

Cholecystocolic fistula is an unusual complication of biliary tract disease and it may defeat laparoscopic treatment. We recently was a patient who, while undergoing laparoscopic cholecystectomy, was found to have a fistula between the gallbladder and the transverse colon. The fistula was transected with a 3 cm endoscopic linear stapling device and uneventful laparoscopic cholecystectomy was performed. This report shows that, with increasing experience, no absolute contraindications exist to starting laparoscopic cholecystectomy by introducing the laparoscope.

Ketoprofen and carotico-cavernous fistula.

Whilst treating a patient with traumatic direct carotico-cavernous fistula, pain relief was difficult to achieve. Adequate doses of ibuprofen and codeine were ineffective, but single doses of ketoprofen alleviated pain in a reproducible manner. Although similar in analgesic efficacy to codeine, and a potent inhibitor of cyclooxygenase like ibuprofen, ketoprofen has other actions which may account for the differential response seen. This observation may help elucidate the nature of local mediators involved in the pathogenesis of vascular headache pain.

Post-cholecystectomy external biliary fistula.

BACKGROUND: External biliary fistula (EBF) following bile duct injury is a serious complication of cholecystectomy. METHODS: From January 1989 to December 1994, 37 patients with post-cholecystectomy external biliary fistula were seen at this centre. There were 14 partial, 22 complete and one sub-vesical duct of Luschka injury. Sixteen patients had a controlled EBF at presentation; 10 patients had intra-abdominal collections and seven patients presented with peritonitis. RESULTS: A staged approach to the management of these patients was adopted in which the initial management aimed at creatin…

Enterovesical fistula: 10 years experience.

BACKGROUND: Enterovesical fistula is a relatively uncommon complication of pelvic malignancies, diverticulitis of the colon, postoperative irradiation or trauma. Early diagnosis of enterovesical fistula is difficult and its management, complicated. The main purpose of this study is to investigate the most appropriate diagnostic method and to discuss the choice among different surgical managements. METHODS: From 1986 to 1995, 30 patients with enterovesical fistula were diagnosed and treated at Veterans General Hospital-Taipei. The records were reviewed retrospectively with regard to symptoms, …

Salpingocolonic fistula secondary to diverticulitis.

A rare diverticular fistula is reported in a 37-year-old woman. The case history and literature were reviewed for clinical presentation, radiologic investigations, and treatment options. Internal fistulas may complicate diverticular disease. The least common fistula is between the colon and the fallopian tube. Successful management of this complication is directed at removing the diseased colon. The woman had a salpingocolonic fistula secondary to diverticulitis. To our knowledge, she represents the youngest patient with this complication of diverticular disease in the literature. Diagnosis of…

[Carotid cavernous fistula: endovascular therapy].

Abnormal communications between the carotidal artery and the cavernous sinus are known as carotid-cavernous fistulas. We can however distinguish between these fistulas two different evolutive and etiological entities: on the one hand there are the direct types with high flux and normally related to hard traumatisms and on the other hand there are the indirect types lacking clear etiological factors, with slower flux and which correspond to dural malformations with different arterial nutrition. In the last two years in our service we have treated eight carotid-cavernous fistulas, four direct an…

Primary aortoduodenal fistula after radiotherapy.

Primary aortoduodenal fistula is an uncommon cause of massive upper gastrointestinal hemorrhage; it is most commonly caused by the erosion of an abdominal aortic aneurysm into the third portion of the duodenum. This report describes a 73-yr-old man who developed uncontrollable hematemesis due to a primary aortoduodenal fistula in the fourth portion of the duodenum approximately 20 yr after radiotherapy and para-aortic lymph node dissection for seminoma. Surgical and postmortem examination revealed encasement of a normal-size aorta by dense fibrous tissue, ischemic necrosis of the aortic wall, …

Rectoperineal fistula in newborn boys.

Three newborn boys presented with features suggestive of classic low anorectal abnormality. However, during surgery they were found to have an intermediate anorectal abnormality and a rectoperineal fistula rather than an anocutaneous fistula. The surgical treatment of these infants is discussed.

Octreotide therapy for pancreaticopleural fistula.

Pancreaticopleural fistula is a rare but remediable complication of pancreatitis. Hitherto, treatment by means of total parenteral nutrition and thoracocentesis had resulted in an overall success rate of 40% only. Surgical obliteration of persistent fistulae is required in many cases, as the underlying pancreatic duct lesion often prevents spontaneous closure of the fistula. We report a patient suffering from pancreaticopleural fistula with a tightly strictured pancreatic duct. The fistula was successfully obliterated with the use of octreotide addition to thoracocentesis and total parenteral …

A unique congenital glossocervical fistula.

Lateral cervical fistulae which communicate with the oropharynx are considered to result from incomplete obliteration of the second branchial cleft and pouch. Classically these fistulae have a well-defined pathway through the neck. We present a case, and discuss the aetiology, of a fistula extending from the lateral neck via a previously undescribed course through the neck structures and opening into the posterior aspect of the tongue at the level of the vallate papillae.

[Complications of antecubital arteriovenous fistula].

A serious complication following the insertion of an a-v shunt for dialysis in a 41-year-old man is reported. Three weeks after the operation, arterial and venous circulatory disturbances occurred in the hand, accompanied by cyanosis distally of the shunt and a lymphatic fistula in the operated cubical area, and followed by the symptoms of a compartment syndrome in the forearm with impaired mobility of the hand. In order to restore normal hemodynamic conditions, the shunt was interrupted and the infected area covered by a local flap. Several weeks later a false aneurysm developed and the circu…
