Fluid Overload in Small Animal Patients.

Fluid therapy is used daily by veterinary practitioners and is an essential part of treatment of many veterinary patients. However, as with all interventions, there is the potential for negative side effects resulting from fluid therapy. Fluid overload is a key side effect that has been increasingly recognized in human medicine as leading to significant negative sequelae. Evidence related to fluid overload in veterinary medicine is sparse but it is likely that the same types of negative sequelae are seen in our veterinary patients. The goal of this review is to present a definition for fluid o…

[Advances in the research of fluid resuscitation and its monitoring in burn patients].

Shock is the one of the most serious complications during the early stage of burn injury. Early effective fluid resuscitation, enabling the burn patient to pass through the shock stage smoothly and uneventfully, plays a necessary and essential role in the prevention of the subsequent organ complications, reduction of mortality and morbidity, and improvement in life quality. Rapid restoration of blood volume is the fundamental measure to prevent burn shock. In this review, the history and the current status of several important issues related to burn shock resuscitation, including the fluid rep

[Dehydration in geriatric patients. Fluid substitution--also subcutaneous!].

Signs of exsiccation are to be found in almost every fourth acutely ill patient admitted to the geriatric department. The major clinical signs are those associated with reduced general condition, together with somnolence, possibly agitation, oliguria, dry skin, orthostatic hypotension, and a tendency to be bedridden. In such cases, subcutaneous infusion is a simple-to-apply and for the patient stress-free, largely pain-free possibility for fluid replacement. Properly applied and with account being taken of the contraindications--in particular emergency situations, decompensated cardiac insuffi

Principles of Fluid Management.

Fluid therapy is the most common intervention received by acutely ill hospitalized patients; however, important questions on its optimal use remain. Its prescription should be patient and context specific, with clear indications and contradictions, and have the type, dose, and rate specified. Any fluid therapy, if provided inappropriately, can contribute unnecessary harm to patients. The quantitative toxicity of fluid therapy contributes to worse outcomes; this should prompt greater bedside attention to fluid prescription, fluid balance, development of avoidable complications attributable to f…

Intravenous fluid resuscitation: learning from a case study with complex presentation.

Intravenous fluid resuscitation (IFR) is one of the most common interventions administered to patients with complex presentations of acute health deterioration. However, a growing body of evidence details variations in practice surrounding IFR due to differences in clinical guidelines, with the effectiveness of IFR reliant upon the selection of the appropriate rate, type and volume of fluid to be administered. According to governing bodies such as the Nursing and Midwifery Council, it is vital that both the prescriber and administrator of IFR have sound understanding of the physiological impli

[The European Society for Clinical Nutrition and Metabolism (ESPEN) Guideline on Clinical Nutrition and Hydration in Geriatrics].

Malnutrition and dehydration are widespread in older people, and obesity is an increasing problem. In clinical practice, it is often unclear which strategies are suitable and effective in counteracting these key health threats. The objective of this guide was to provide evidence-based recommendations for clinical nutrition and hydration in older persons in order to prevent and/or treat malnutrition and dehydration. Further, to address whether weight-reducing interventions are appropriate for overweight or obese older persons. This guideline was developed according to the standard operating pro

The muscle damage can occur because of direct … valuation for any complications and initiation of appropriate treatment are recommended. Aggressive fluid therapy, correction of metabolic derangement, and, when warranted, renal replacement therapy, form the mainstays of treatment.

Comprehensive Overview of Hypernatremia: Pathophysiology, Diagnosis, and Management.

Hypernatremia, defined as an elevated serum sodium concentration, is a critical electrolyte imbalance with significant neurological and systemic effects. Effective management hinges on understanding the condition's underlying aetiology, such as dehydration, excessive sodium intake, or impaired renal water handling. The approach to treatment varies by onset and severity: acute hypernatremia (<48 hours) necessitates rapid but controlled correction to prevent complications like vascular rupture or cerebral bleeding, while chronic hypernatremia (≥48 hours) requires gradual intervention to mitigate

The First 24 Hours: Burn Shock Resuscitation and Early Complications.

Resuscitation is required for the management of patients with severe thermal injury. Some of the initial pathophysiologic events following burn injury include an exaggerated inflammatory state, injury to the endothelium, and increased capillary permeability, which all culminate in shock. Understanding these processes is critical to the effective management of patients with burn injuries. Formulas predicting fluid requirements during burn resuscitation have evolved over the past century in response to clinical experience and research efforts. Modern resuscitation features individualized fluid t

Perioperative Fluid Management.

The medical complexity of the geriatric patients has been steadily rising. Still, as outcomes of surgical procedures in the elderly are improving, centers are pushing boundaries. There is also a growing appreciation of the importance of perioperative fluid management on postoperative outcomes, especially in the elderly. Optimal fluid management in this cohort is challenging due to the combination of age-related physiological changes in organ function, increased comorbid burden, and larger fluid shifts during more complex surgical procedures. The current state-of-the-art approach to fluid manag

Essence Core: Fluid Management in Acute Kidney Injury.

Acute kidney injury (AKI) is a heterogeneous disorder characterized by sudden decrease in kidney functioning, with increased serum creatinine levels and impairment of vital kidney functions such as fluid, electrolyte, and acid-base homeostasis. The key to perioperative AKI management is accomplishing optimal intravenous fluid therapy, involving guided fluid resuscitation and fluid balance management including proper fluid removal. In the present review, we highlighted the importance of fluid-based management of AKI, which is a critical process, as both reduced and increased levels of body flui…

Current practice and evolving concepts in septic shock resuscitation.

Clinical and pathophysiological understanding of septic shock has progressed exponentially in the previous decades, translating into a steady decrease in septic shock-related morbidity and mortality. Even though large randomized, controlled trials have addressed fundamental aspects of septic shock resuscitation, many questions still exist. In this review, we will describe the current standards of septic shock resuscitation, but the emphasis will be placed on evolving concepts in different domains such as clinical resuscitation targets, adequate use of fluids and vasoactive drugs, refractory sh

[Prescribing intravenous fluids in hospitalized patients : from need to harm].

Prescribing intravenous fluids in a hospital setting is a task that each clinician has to endorse in an everyday practice. This task is universal to the point that it is often undervalued, or even neglected during clinical teaching. However, one shouldn't underestimate the risks related to excessive fluid administration, of which the clinician often doesn't know the exact composition. Literature about persistent fluid overload in hospital is scarce. This complex condition should however be given the attention it deserves. Indeed, it seems to be associated with increased costs and an excess in

Fluid Management in Acute Kidney Injury.

Correction of intravascular hypovolemia is a key component of the prevention and management of acute kidney injury (AKI), but excessive flui … liguria alone should not be viewed as a trigger for fluid administration. If fluid overload occurs, fluid therapy needs to be discontinued, and fluid removal using diuretic agents or extracorporeal therapies should be considered.

Quality and quantity of volume replacement in trauma patients.

An epidemiologic evaluation of trauma-related deaths in trauma centers reveals that the majority of patients die within 6 hours from exsanguination, whereas secondary brain injuries predominate between 6 and 24 hours. Late deaths remain attributable to sepsis and pulmonary embolism,1-3 while early deaths are due in part to multiple bleeding injuries or to a set of complex and untreatable injuries, mainly of the liver and pelvis. Before trauma systems existed, these patients died at the scene of the trauma, whereas since the establishment of the trauma system, they die in emergency or operating
