BACKGROUND: Nutritional deficiency in pregnant women is a confirmed cause of neural tube defects (NTDs). Iron, folate, and vitamin B-12 status was found to be poor in a substantial proportion of outpatients with generally inactive Crohn's disease. Diet counseling was associated with a normalization of TIBC and serum folate over a 6-month period, but no other consistent benefits were noted despite moderate improvements in intake. The outpatients appeared to be at low risk of developing a zinc or copper deficiency.

Dietary folacin deficiency in the presence and absence of dietary sulfonamides was studied in the guinea pig for the purpose of relating the growth and hematologic effects of folacin deficiency to tissue and biochemical changes. Six-week old female guinea pigs fed control and folacin deficient diets with and without 0.1% sulfonamides for 6 to 7 weeks. A total of 123 twin pregnancies were studied, 68 by records analysis only, and 55 by serial haematological assessment in detail. The incidence of low haemoglobin levels is found to be greater than in singleton pregnancy; evidence of iron and folic acid deficiency is common in sternal marrow aspirates but not in peripheral blood films. Routine haematinic prophylaxis in twin pregnancy is not advocated.

BACKGROUND: Deficiencies of cobalamin (vitamin B12) or folate are common conditions that predispose for anemia and chronic diseases. An elevated concentration of methylmalonic acid in plasma/serum is an indicator of cobalamin deficiency, whereas an increased concentration of total homocysteine in plasma occurs with deficiency of both cobalamin and folate. The biomarkers methylmalonic acid and homocysteine are therefore complementary, and the combination is often requested when conventional tests fail to provide an unambiguous diagnosis.
