Foot complications are common in people with diabetes. Ulcers often become infected and, if not treated promptly, can result in sepsis and gangrene requiring amputation. The specialised nursing intervention in this study varied according to the assessed risk. Patients with diabetic neuropathy received treatment every six months. This comprised toenail cutting, trimming of trichophytic nail with a grinder, removal of keratinised layers of callus with a corn-cutter and grinding of the surface of the callus.

The arthroscopic approach to small joints of the foot has made many advances in recent years, which can be directly related to the improvement of the surgical equipment. This improvement has led to more indications for the use of arthroscopy as well as minimizing the complications. Several articles recently have presented experiences in arthroscopic surgery in the small joints of the foot; however, its use is still relatively limited. Approaches to small joints of the foot involve the first metatarsophalangeal joint, tarsometatarsal joint, and Chopart joint, as well as the interphalangeal join…

In summary, varus deformity of the foot and ankle encompasses a spectrum of deformities from mild to severe. The cause of this deformity may be bone, muscle imbalance, or a combination of both. Surgical intervention should be planned only after the patient's anatomy is understood. Uncorrected symptomatic varus deformities may have significant consequences on gait kinematics and foot biomechanics.

The retinacula of the ankle are distinct structures defined as regions of localized thickening of the crural fascia covering the deep structures of the distal portion of the leg, ankle, and foot. Their role is to maintain the approximation of the tendons to the underlying bone. The retinacula of the ankle and foot contain the extensor retinaculum, the peroneal retinaculum, and the flexor retinaculum. Knowledge of the normal anatomy and imaging appearance of these structures is essential for the depiction of their injuries. Even though they may provide an explanation for chronic and unexplained…

Clinical decision making for the management of foot deformities in children with cerebral palsy is based on the collection and integration of data from 5 sources: the clinical history, physical examination, plain radiographs, observational gait analysis, and quantitative gait analysis (which includes kinematic/kinetic analyses, dynamic electromyography, and dynamic pedobarography). The 3 most common foot segmental malalignments in children with CP are equinus, equinoplanovalgus, and equinocavovarus. The 2 most common associated deformities are ankle valgus and hallux valgus. Foot and ankle def…

BACKGROUND: A high incidence of infections has been reported in rheumatoid arthritis (RA) patients, either due to intrinsic factors or as a … athered and analysed.
RESULTS: Of the 36 cases in total, infection was localized in the rheumatoid foot in 23 patients (64%) and at the lower extremities in 32 (89%). The bacterial spectrum was heterogenous, with Staphylococcus aureus representing the most frequent causative agent. In total, 34 of 36 cases were cured.
CONCLUSIONS: Since approximately …

Stress fractures are common athletic injuries of the foot and ankle, described in every bone except the lesser toes, and reviewed here. Early diagnosis usually allows for simpler treatment and quick recovery. Early clinical presentations can be subtle, so a high degree of suspicion and a systematic approach, coupled with an understanding of the diagnostic limitations present in early injury, is required. Such a rigorous approach ultimately pays dividends for these patients, who are usually keen to return quickly to athletic activity. "High-risk" fractures include the medial malleolus, the talu…

As many as 1% of older people in the Western world are at risk of developing a foot ulcer as a complication of diabetes mellitus. The resultant debility and disability constitutes a burden for both individuals and their health services. When peripheral arterial insufficiency complicates neuropathy there is a tenfold risk of ulceration progressing to infection, gangrene and amputation. Patient education and the vigilant implementation of preventive measures offer the best prospects for containment of the problem. Patients faced with ulceration and limb loss require access to a co-ordinated and …

BACKGROUND: Squamous cell carcinomas (SCC) of the foot are relatively common, but have been infrequently reported in the orthopaedic literature.
MATERIALS AND METHODS: Twelve patients with SCC of the foot treated at a single institution from 1998 to 2005 were studied retrospectively with regard to risk factors for the disease, treatment, and functional and oncologic outcomes. The mean duration of postoperative followup was 43 (range, 24 to 105) months.
RESULTS: Eight of the 12 patients had identifiable risk factors for SCC. Inadequate or inappropriate procedures had been previously performed…

Multifocal skeletal Ewing sarcoma is very rare. A 10-year-old boy presented with multiple independent lesions involving the first phalanx of the middle toe, the second metatarsal bone, the cuneiform and cuboid bones, and the talus. Diagnosis of Ewing sarcoma was confirmed by open biopsy of 2 nonadjacent bones. Chest computed tomography disclosed the presence of multiple lung metastases. The patient underwent chemotherapy, below-knee amputation, and lung irradiation. To the best of the authors' knowledge, no similar case has been previously reported in the literature.

Fractures in the dance population are common. Radiography, CT, MRI, and bone scan should be used as necessary to arrive at the correct diagnosis after meticulous physical examination. Treatment should address the fracture itself and any surrounding problems such as nutritional/hormonal issues and training/performance techniques and regimens. Compliance issues in this population are a concern, so treatment strategies should be tailored accordingly. Stress fractures in particular can present difficulties to the treating physician and may require prolonged treatment periods. This article addresse

The term "cavus foot" is used to describe a spectrum of foot shapes that have in common a high arch. The components of cavus are increased pitch and varus of the hindfoot, plantar flexion of the midfoot, and varus and adduction of the forefoot. The cavus shape is associated with changes in the mechanics of the foot. It is less flexible and balanced than a neutral foot. Although one fifth to one quarter of the population has a cavus foot, there is little anatomic description in the literature. This manuscript describes the anatomy and function of cavus foot.

Diabetic foot disease is an important cause of morbidity and mortality in persons with diabetes mellitus. The commonest presentation of diabetic foot is an ulcer. Neuropathy, ischaemia and infection are the main pathogenic factors involved. Clinical examination and investigations are focused on identifying the aetiology as well as the extent of foot disease. The monofilament test is a simple, bedside test that can predict the risk of neuropathic ulceration. Treatment of diabetic foot ulcer should focus on antibiotic therapy, dressings, debridement and timely surgery. Glycaemic control and mana…

BACKGROUND: Synovial chondromatosis (SC) is a benign condition where the synovial lining of joints, bursae, or tendon sheaths undergoes metaplasia and ultimately forms cartilaginous loose bodies. Synovial chondromatosis of the foot and ankle is exceedingly rare, and outcomes following surgical excision are largely unknown.
MATERIALS AND METHODS: An Institutional Review Board-approved retrospective review of our institution's surgical database from 1970 to 2006 revealed 8 patients with SC of the foot and/or ankle confirmed by pathology.
RESULTS: Eight patients (4 female and 4 male) presented …

Surgery of the rheumatoid foot and ankle Initial presentation of rheumatoid arthritis (RA) will be foot and ankle symptoms in 20% of cases. Eventually, 50-90% of patients will develop foot and ankle alterations. Typical deformities of the forefoot include hallux valgus and dorso-lateral dislocation of the lesser metatarso-phalangeal joints. RA involvement of the hindfoot frequently leads to plano-valgus deformity. Approximately 20% of surgery performed for RA is done at the foot and ankle. Early surgery is efficient against pain and deformity. Patient's satisfaction rate after surgery is high …

BACKGROUND: Talonavicular joint fusion has been successfully applied for the surgical treatment of the rheumatoid foot. Several fixation techniques have been suggested for this purpose, however, with high rates of non-union.
METHODS: Based on seven cases operated in our division, talonavicular joint fusion with two 3.5 mm compression screws and autologous bone grafting is discussed. Pain in the operated foot under weight-bearing conditions was assessed before surgery and after a mean follow-up of 35 months (range 3-58 months).
RESULTS: Solid talonavicular fusion was achieved clinically and r…

The biomechanical effects of talectomy on the foot were investigated in seven fresh below-the-knee amputation specimens using pressure-sensitive films placed on the facets of the calcaneus, footprints, and loading-pattern diagrams in the intact foot and after talectomy with anterior and posterior displacement of the foot. Both talectomy techniques distorted the loads carried by the facets of the calcaneus. In the intact foot, 65.6% of the loads were carried by the posterior facet of the calcaneus and 34.4% by the anterior and middle facets. After talectomy with anterior displacement of the foo…
