In a consecutive study of 106 patients with a history of ankle sprain, 40.5% showed clinical signs of damage to the ligamentum bifurcatum of the midfoot. A significantly larger percentage of these patients complained of maximum tenderness localized to the instep compared with the patients without signs of damage to the ligamentum bifurcatum (P less than 0.001). Radiologic examination revealed 20 lesions related to damage to the ligamentum bifurcatum: laxity in the transverse tarsal joint, 12; avulsion from the anterior process of the calcaneus, 5; avulsion from the cuboid, 2; avulsion from the

This article reviews the treatment of a pediatric patient following head and neck trauma from a dog bite. The patient had an avulsed mandibular incisor, a fractured maxillary lateral incisor, and various facial lacerations. The avulsed tooth was replanted and secured for two weeks by using a physiologic splint. Both traumatized teeth received endodontic therapy and were returned to normal function. Trauma patients require a thorough evaluation and a multidisciplinary approach for the formation of a proper treatment plan.

We report about the case of a 4(1/2)-year delayed screw fixation of a tibial posterior cruciate ligament (PCL) avulsion fracture in combination with a posterolateral stabilization. After open reduction and rigid screw fixation, combined with a posterolateral stabilization using a semitendinosus tendon autograft, the patient returned to full activity in sports and daily life. If the PCL substance is sufficient, a delayed refixation in an old PCL avulsion injury seems to be a viable alternative to PCL reconstruction.

BACKGROUND: Segond fracture is considered a component of the anterolateral complex (ALC) injury, yet the underlying cause and clinical outcomes of this bony avulsion remain subjects of debate. Additionally, MRI measurements of altered tibiofemoral position in anterior cruciate ligament (ACL)-injured patients with a Segond fracture have not been reported. The purpose of this study is to measure the rotational tibiofemoral position on MRI in ACL-injured patients with a Segond fracture.
METHODS: A total of 44 patients with a primary ACL injury and a concomitant Segond fracture were included in t

Avulsion fractures of the fibular head, commonly manifesting as the "arcuate sign" on radiographs, are indicative of posterolateral corner (PLC) injuries of the knee and, if inadequately managed, may lead to chronic instability and degenerative joint changes. This technical note describes a novel surgical technique for the fixation of arcuate fractures following failed anchor repair. The procedure involves dual tunnel formation in the fibula and strategic FiberTape passage to achieve anatomical reduction and stable fixation of the avulsed fragment. Key advantages of this approach include a low

Mallet fingers with an avulsion fracture of the distal phalanx or rupture of the terminal tendon of the extensor mechanism is known as a common injury, while mallet thumb is very rare. In this paper, the case of a 19-year-old woman with a sprained left thumb sustained while playing basketball is presented. Plain radiographs and computed tomography revealed an avulsion fracture involving more than half of the articular surface at the base of the distal phalanx. Closed reduction and percutaneous fixation were performed using the two extension block Kirschner wires' technique under digital block

Traumatic avulsion of the right diaphragm from the lumbocostal arch is a very rare lesion. The authours report the case of a 27-year-old man who had suffered a severe polytrauma with blunt thoracic injury, fracture of the lumbar spine, Malgaigne-type fracture of the pelvis and fracture of the femoral shaft on the right side, 10 years before. At the time of injury the lesion of the diaphragm went unnoticed. The diagnosis was made 10 years later when the patient was referred for chronic right thoracic pain combined with postprandial abdominal distension and crampy pain in the abdomen. The chest

Throwing and racquet sports produce large stresses at the elbow joint consisting of tension on the medial or ulnar side, and compression on the lateral or radial side. Medial tension can lead to tears of the common flexor origin or medial collateral ligament, ulnar nerve subluxation, or pronator teres syndrome. Bony lesions include fragmentation and partial avulsion of the medial humeral epicondyle (little league elbow) or even complete avulsion of the epicondyle. The possible sequelae of excessive lateral compression include osteochondritis dissecans of the capitellum, osteochondral fractures

Fractures of the distal radius are most commonly caused by hyperextension injuries of the wrist. Tensile forces and force vectors, strength of impact, bone strength and soft tissue tension create individually different fracture patterns. Metaphyseal comminution, loss of cortical support, ligament avulsion and shear fragments are defining parameters for fracture instability. The dislocation of the articular fragment follows the force vectors of the extrinsic forearm muscles bridging the joint. The goal-plan-standardized (GPS) treatment strategy has proven to be helpful in choosing the ideal ind

INTRODUCTION AND IMPORTANCE: Tillaux fracture occurs as a result of a forced external rotation mechanism involving the anterolateral tibial plafond. The occurrence of this fracture in adults is rare because the ligament usually disrupts before causing bone avulsion.
CASE PRESENTATION: We report the case of a 31-year-old man who, following a trauma in a foot-ball game, was admitted for management of an isolated anterolateral tibial plafond fracture. The diagnosis was made by X-ray, supported by CT scan, which investigated the fracture and excluded other associated injuries. Management was base

Foot and ankle injuries are common musculoskeletal disorders. In the acute setting, ligamentous injuries are most common, whereas fractures, osseous avulsion injuries, tendon and retinaculum tears, and osteochondral injuries are less common. The most common chronic and overuse injuries include osteochondral and articular cartilage defects, tendinopathies, stress fractures, impingement syndromes, and neuropathies. Common forefoot conditions include traumatic and stress fractures, metatarsophalangeal and plantar plate injuries and degenerations, intermittent bursitis, and perineural fibrosis. Ul

BACKGROUND: Acute ankle fractures can lead to high rate of concomitant intra-articular lesions which may compromise clinical results. The purpose of this study was to evaluate the incidence of concomitant intra-articular lesions in acute ankle fractures with arthroscopy. We also sought to analyze the relationship between intra-articular lesions and the fracture type, as well as the severity of the fracture.
METHODS: It was a retrospective cohort study. From April 2014 to December 2015, we have chosen arthroscopy-assisted open reduction and internal fixation (AORIF) for the treatment of unstab

Segond fracture is an avulsion fracture of the lateral side of the tibial plateau. In most cases, this fracture is associated with serious injuries to the knee such as a rupture of the anterior cruciate ligament (ACL). This highlights the importance of recognizing and diagnosing such fractures on X-ray images followed by the use of computed tomography (CT) and magnetic resonance imaging (MRI), in order to accurately diagnose potential additional injuries of the knee joint. This report shows relevant images as well as the outcome of a 59-year-old woman with a right-sided Segond fracture.

The mandible is one of the most frequently fractured facial bone, most commonly by automobile accidents followed by fall from heights. However, segmental avulsion of mandible, that too in pediatric population is rarely reported. We report a case of 10-year-old male child sustaining facial injury following fall from a tree, resulting in complete segmental avulsion of the mandible highlighting the importance of early plastic surgery referral to optimize treatment outcomes. After clinical and radiological evaluation, the patient underwent free fibula osseocutaneous flap reconstruction. Given the

Avulsion fracture of coracoid process with acromioclavicular joint dislocation is extremely rare. We report a case of coracoid avulsion with acromioclavicular disruption in a 24-year-old man who sustained injuries in a road traffic accident. Although acromioclavicular (AC) dislocation was obvious from an initial radiograph, coracoid avulsion was picked up in a CT scan. Open reduction and internal fixations of the coracoid with a 4 mm cannulated screw, an AC joint with two K-wires and an anchor suture, resulted in rapid recovery. The patient had complete range of shoulder movements at the end o

The anterolateral tibial rim with the anterior tibial tubercle (Tubercule de Tillaux-Chaput) serves as an insertion site of the anterior inferior tibiofibular ligament (AITFL). It can also be termed the anterior malleolus or fourth malleolus. Fractures of the anterolateral tibial rim typically result from an external rotation or abduction mechanism of the talus within the ankle mortise. They are frequently overlooked in plain radiographs. Computed tomography (CT) is needed for an exact visualization of the fracture anatomy and treatment planning. A total of three main types can be differentiat

A patient with spontaneous bilateral fracture of the mandibular coronoid process is presented. The definite cause is unknown, but possible contributing factors include osteoporosis, bruxism and changed kinetics of the temporalis muscle after bilateral sagittal split osteotomy (BSSO) resulting in an avulsion fracture. The patient was treated surgically with bilateral coronoidectomy.

PURPOSE: To investigate the radiologic and clinical outcomes of direct internal fixation for unstable atlas fractures.
MATERIALS AND METHODS: This retrospective study included 12 patients with unstable atlas fractures surgically treated using C1 lateral mass screws, rods, and transverse connector constructs. Nine lateral mass fractures with transverse atlantal ligament (TAL) avulsion injury and three 4-part fractures with TAL injury (two avulsion injuries, one TAL substance tear) were treated. Radiologic outcomes included the anterior atlantodental interval (AADI) in flexion and extension cer

Aside from the more common dorsal avulsion fractures, isolated triquetral body fractures are a rare injury and often missed. When they are identified, conservative treatment via immobilization is often the standard of care for initial treatment. Rarely, triquetral body fractures can develop into symptomatic nonunions, causing considerable pain and disability. Multiple classification schemes have been described to categorize triquetrum fractures; however, distal triquetrum fractures fit into none of the established models. There is scarce literature describing treatment of triquetral body fract
