Bone biology in the elderly: clinical importance for fracture treatment.

Age-related bone impairment often leads to fragility fractures in the elderly. Although excellent surgical care is widely provided, diagnosis and treatment of the underlying bone disorder are often not kept in mind. The interplay of the three major bone cells - osteoblasts, osteoclasts, and osteocytes - is normally well regulated via the secretion of messengers to control bone remodeling. Possible imbalances that might occur in the elderly are partly due to age, genetic risk factors, and adverse lifestyle factors but importantly also due to imbalances in calcium homeostasis (mostly due to vita

[Complication management for failed bone fracture healing: pseudarthrosis].

Delayed fracture healing for more than 6-8 months is defined as non-union (pseudarthrosis). Non-unions are classifiable as septic, aseptic, hypertrophic and atrophic non-unions. In case of septic non-unions the infection is treated primarily followed by treatment of the delayed fracture healing. Aseptic non-unions may be treated non-operatively (e.g. shock wave therapy and/or ultrasound) or by various surgical strategies to stimulate bone regeneration and healing.

Spontaneous Resorption of a Penetrating Orbital Bone Fracture Fragment.

The authors describe a 20-year-old man who sustained multiple facial fractures in a high-speed motor vehicle crash, including a bone fragment from a skull base fracture that penetrated the orbital soft tissues superomedially. Serial CT scans documented spontaneous resorption over a 6-month period. While it is known that autologous bone grafts used in craniofacial reconstruction exhibit variable amounts of bone resorption, the complete resorption of an intraorbital fracture fragment has not been documented in the literature. His clinical care and the report of his case were undertaken in a fash

[Medication and bone metabolism: Clinical importance for fracture treatment].

INTRODUCTION: The improvement and acceleration of fracture healing has been a component of medical practice since fractures have been treated. The aim is not only to fulfill the basic principles of fracture healing, such as reduction, retention, soft tissue coverage and infection prevention but also to reduce negative influences on fracture healing and promote positive factors. Nicotine, alcohol, diabetes and malnutrition can negatively affect fracture healing and should be appropriately controlled during fracture treatment; however, it is far more difficult to develop medicinal treatment stra

Anatomoradiological identification of intrasutural bones for importance of cranial fracture.

AIM: Intrasutural bones are often found within sutures and fontanelles of the skull. It is important that the anatomy of these bones in the field of non-accidental injury with respect to medicolegal complications. Skull fracture lines are critical in their differential diagnosis, and the features of bone formations, their appearance and position give us a more definitive diagnosis. MATERIAL AND METHODS: Intrasutural bone location, its number and symmetry were investigated in totally 300 adult skulls. RESULTS: 27 intrasutural bones were identified as interparietal (2%), preinterparietal (3%)

Open Fracture of the Forearm Bones due to Horse Bite.

INTRODUCTION: Fractures have been described mainly following falling accidents in horse-related injuries. Horse bites are uncommon accidents. We present a case of open fracture of the forearm due to horse bite. CASE REPORT: A 35-year-old male farm-worker presented to the emergency room with alleged history of horse bite to the right forearm about 2 hours prior to presentation while feeding the horse. There was deformity of the forearm with multiple puncture wounds, deep abrasions and small lacerations on the distal-third of the forearm. Copious irrigation with normal saline was done and he wa

Stress fracture of the lateral cuneiform bone: a case report.

Stress fractures are common overuse injuries of the bone that are most often seen in athletes and military personnel. These types of injuries seem to be commonly missed or delayed in diagnosis. Physicians should consider the possibility of such an injury while determining the diagnosis of a long-lasting foot pain. The aim of this paper was to report an isolated stress fracture of the lateral cuneiform bone. A review of the literature failed to find a similar case.

Cochlear implantation in posttraumatic bilateral temporal bone fracture.

Temporal bone (TB) fractures can cause loss of audiovestibular function. Four cases of profound hearing impairment following bilateral TB fracture are presented in this report. All patients received a cochlear implant. All 4 patients became regular users of their implants. None of the patients experienced facial nerve stimulation. Implant-aided audiometry demonstrated a hearing threshold of 28 dB HL. The performance in speech understanding was comparable to standard postlingual adult patients implanted. We believe that cochlear implantation in patients suffering from profound sensorineural hea

Fracture mimics on temporal bone CT: a guide for the radiologist.

OBJECTIVE: The objective of this article is to discuss and illustrate commonly visualized fissures and sutures in the temporal bone. This topic is important because a thorough knowledge of normal anatomy is necessary to avoid misinterpretation as fractures. CONCLUSION: Small normal anatomic fissures are now routinely visualized with the increasing use of MDCT in trauma patients. An awareness of these structures is required by radiologists interpreting studies with fine temporal bone slices to prevent erroneous interpretation.

Intramedullary nailing for paediatric diaphyseal forearm bone fracture.

BACKGROUND: Though most of the pediatric diaphyseal forearm bone fracture can be treated with closed reduction and cast application, indications for operative intervention in pediatric both-bone forearm fractures include open fractures, irreducible fractures, and unstable fractures. Controversy exists as to what amount of angulation, displacement, and rotation constitutes an acceptable reduction. OBJECTIVE: To review union time and functional outcome of pediatric diaphyseal forearm bone fracture managed with intramedullary rush pin by closed or open reduction. METHODS: Fifty patients with bo

Long-term outcomes of fractures of both bones of the forearm.

BACKGROUND: Previous studies identified limited impairment and disability several years after diaphyseal fractures of both the radius and ulna, although the relationship between impairment and disability was inconsistent. This investigation studied skeletally mature and immature patients more than ten years after injury and addressed the hypotheses that (1) objective measurements of impairment correlate with disability, (2) depression and misinterpretation of nociception correlate with disability, and (3) patients injured when skeletally mature or immature have comparable impairment and disabi

New maneuver for fixation of pediatric nasal bone fracture.

Surgical treatment of nasal bone fractures in children is a complex and challenging task not only to maxillofacial surgeons but exceeding to ears, nose, and throat and plastic surgeons. Twelve child patients including 9 boys (75%) and 3 girls (25%) with nasal bone fractures were seen at the Maxillofacial Surgery Department at Ramadi Teaching Hospital during the period of December 2009 to December 2010. Fractured nasal bone is reduced to its anatomic position with Walsham forceps, and the dislocated or disrupted nasal septum with Asch forceps. The most common cause of the injury was road traffi

Temporal bone fracture: evaluation and management in the modern era.

Temporal bone fractures occur from high-energy mechanisms, typically but not limited to motor vehicle accidents. However, as the automotive industry continues to introduce improved safety measures, violence and falls account for a larger proportion of cases. Given the great forces involved, temporal bone fractures rarely occur in isolation and initial evaluation must focus on the fundamental ABCs of Advanced Trauma Life Support with special attention to Glasgow Coma Scale, intracranial injury, and cervical spine injury. Subsequent evaluation relies on physical examination, high-resolution CT,

Chronic kidney disease and bone fracture: a growing concern.

Susceptibility to fracture is increased across the spectrum of chronic kidney disease (CKD). Moreover, fracture in patients with end-stage kidney disease (ESKD) results in significant excess mortality. The incidence and prevalence of CKD and ESKD are predicted to increase markedly over the coming decades in conjunction with the aging of the population. Given the high prevalence of both osteoporosis and CKD in older adults, it is of the utmost public health relevance to be able to assess fracture risk in this population. Dual-energy X-ray absorptiometry (DXA), which provides an areal measuremen

[The treatment of extremity bone fractures in the cases of combined injuries].

The experience of treatment of 76 injured with fractures of two or more extremities in combination with affection of other anatomical areas (head, abdomen, breast, pelvis) was summarized. The extrafocal osteosynthesis was preferable because this method is characterized by low traumatism, minimal blood loss, adequate reposition, stable immobilization and possibility of early load in the apparatus of external fixation. Immersed osteosynthesis is used with great caution as it can cause the development of fat embolism. One-stage operations were performed in 26.3% victims. Their treatment periods w
