Rickets secondary to phosphate depletion. A sequela of antacid use in infancy.

Two infants presented with growth failure and were found to have generalized osteomalacia (rickets) due to phosphate depletion from prolonged administration of an aluminum-containing antacid given for the symptoms of colic. One of the infants developed bilateral proptosis due to craniosynostosis related to the underlying metabolic bone disease. The chronic use of aluminum-containing antacids in infants has potential risk for the growing skeleton and is not innocuous. Therefore, antacid therapy should be used in low doses and very cautiously, with routine monitoring of serum calcium and phospho

[The effect of the increased absorption of food protein antigens from the intestines on the development of sensitization in guinea pigs].

Adult guinea pigs received intragastrically chicken egg ovalbumin (OA), its absorption into the blood was evaluated by radioimmunoassay, and 21 days later the degree of sensitization was estimated by active anaphylactic shock reaction and by the level of circulating specific antibodies. It was shown that the increased permeability of the gastrointestinal tract barrier to OA macromolecules led to a rise in the humoral immune response and anaphylaxis aggravation in the animals. Similar dependence was not observed when the heterologous antigen-bovine serum albumin was used to test the permeabilit

[Physiology and pathophysiology of bile secretion].

After introductory anatomical considerations a survey is given of the composition of bile and the physiology of bile secretion. According to the modern conception the bile acids give the osmotic power for the excretion of water and electrolytes. Apart from this, a bile fraction, which is also secreted by the liver cells into the canaliculi, is present, and in the biliary ducts a further volume fraction independent of bile acid is added, the secretion of which is determined by gastrointestinal hormones. Processes of reabsorption play a role particularly in the gallbladder. By formation of micel

Macropinocytosis and Fast Endophilin-Mediated Endocytosis Mediate Absorption of Garlic Chive-Derived Vesicle-like Nanoparticles in Human Intestinal Epithelial Cells.

Dietary extracellular vesicles (EVs) or vesicle-like nanoparticles (VLNs) have been shown to exert beneficial functions in a wide range of diseases such as cancer, colitis, and metabolic diseases. They have also been used as natural carriers for medications. Despite the promising translational potential of dietary EVs or VLNs, the molecular mechanisms of their absorption in the gastrointestinal tract are not well understood. In this study, we investigated the absorption mechanisms of garlic chive-derived VLNs (GC-VLNs) using C57BL/6J mice and a human intestinal epithelial cell line, Caco-2 cel

Impact of tailored dietary interventions on suspected carbohydrate intolerance patients based on genetic testing.

Background: carbohydrate intolerance presents a complex scenario where symptoms arise following the consumption of specific substrate and alleviate upon their elimination from the diet. Lactose Intolerance is one of the most prevalent types of food intolerance. Primary lactose intolerance is linked to genetic factors, Lactase Non-Persistence phenotype, while secondary lactose intolerance might be a temporary condition resulting from intestinal damage and loss of disaccharidase activity. Fructose absorption is an energy-independent process, leading to limited and variable absorption. Fructose u

Effects of free-fatty-acid content and saturation degree of the dietary oil sources on lipid-class content and fatty-acid digestibility along the gastrointestinal tract in broilers from 22 to 37 days of age.

The aim of the present study is to assess the effect of the free-fatty-acid (FFA) content and saturation degree of dietary fat (added at 6%) on the fatty-acid (FA) digestibility and lipid-class content along the gastrointestinal tract and excreta in broilers from 22 to 37 d of age. This is essential to determine the potential use of acid oils (refining by-products rich in FFA) in broiler diets as an alternative to crude oils. The study consisted of a 2 × 4 factorial arrangement, which included 2 fat sources (soybean oils - unsaturated, or palm oils - saturated) and 4 levels of FFA (5, 15, 35,

Liquid Levothyroxine Formulation Taken during Lunch in Italy: A Case Report and Review of the Literature.

Levothyroxine (L-T4) is among the most widely prescribed medications in the world, and it is considered by the World Health Organization an essential medicine for basic health care. Replacement therapy has always been considered straightforward although different factors may interfere with intestinal absorption of L-T4, including food, dietary fibre, coffee, drugs, and gastrointestinal diseases. For these reasons, current guidelines recommend that L-T4 should be taken in a fasting state because its absorption is maximised when it is taken on an empty stomach, reflecting the importance of gastr

Evaluation of methods for assessing the oral bioavailability of inorganic mercury in soil.

The risks associated with environmental exposures to inorganic mercury are typically assessed based on toxicity studies conducted with the soluble salt, mercuric chloride (HgCl2). Evidence indicates, however, that inorganic mercury is present in soil as a variety of compounds and that oral absorption of inorganic mercury decreases with a decrease in the solubility of the mercury compound being studied. Thus, while HgCl2 is approximately 15-20% bioavailable, the bioavailability of cinnabar (HgS) may be 30- to 60-fold less. The solubility and, hence, bioavailability of inorganic mercury in soil

Small Intestinal Bacterial Overgrowth: Nutritional Implications, Diagnosis, and Management.

Small intestinal bacterial overgrowth (SIBO), characterized by the presence of excessive bacteria in the small intestine, is typically described as a malabsorptive syndrome occurring in the context of gut stasis syndromes. SIBO is now considered to be a disorder associated with diverse clinical conditions without classic risk factors for SIBO and a cause of several nonspecific gastrointestinal and nongastrointestinal symptoms. Because there is currently no gold standard for diagnosing SIBO, its prevalence and role in the pathogenesis of other diseases remain uncertain; as does optimal treatmen

Rapid gastric and intestinal transit is a major determinant of changes in blood glucose, intestinal hormones, glucose absorption and postprandial symptoms after gastric bypass.

OBJECTIVE: To evaluate the effect of modulating pouch emptying (PE) and SI transit of glucose after Roux-en-Y gastric bypass (RYGB) on blood glucose, incretin hormones, glucose absorption and gastrointestinal (GI) symptoms. METHODS: Ten RYGB patients were studied twice in random order, receiving either a 150 ml glucose drink (200 kcal) or the same solution infused into the proximal Roux-limb at 4 kcal/min. Data were compared with 10 healthy volunteers who received a 4 kcal/min duodenal infusion. PE, cecal arrival time (CAT), blood glucose, plasma 3-O-methylglucose (3-OMG), insulin, glucose-de

Gastroparesis in Parkinson Disease: Pathophysiology, and Clinical Management.

Patients with Parkinson disease (PD) experience a range of non-motor symptoms, including gastrointestinal symptoms. These symptoms can be present in the prodromal phase of the disease. Recent advances in pathophysiology reveal that α-synuclein aggregates that form Lewy bodies and neurites, the hallmark of PD, are present in the enteric nervous system and may precede motor symptoms. Gastroparesis is one of the gastrointestinal involvements of PD and is characterized by delayed gastric emptying of solid food in the absence of mechanical obstruction. Gastroparesis has been reported in nearly 45%

Iron Treatment Strategies in Nondialysis CKD.

In contrast to managing patients on hemodialysis in whom iron strategies are more focused on intravenous iron, nondialysis chronic kidney disease (CKD) patients may receive either oral or intravenous iron. There are advantages and disadvantages for both strategies. Oral iron is simple and cheap to administer and does not require hospital visits, but is poorly absorbed in advanced CKD and is associated with unpleasant gastrointestinal side effects. Intravenous iron, on the other hand, guarantees iron bioavailability and avoids problems of variable absorption of iron from the gastrointestinal tr

[Positive effects of wheat bran for digestive health; scientific evidence].

Wheat bran (ST) is very rich in insoluble fiber, consisting mainly arabinoxylans and, to a lesser extent, cellulose and β-glucans, as well as vitamins, minerals and antioxidants polyphenolic. The ST is involved in the regulation of gastrointestinal physiology and health: delayed gastric emptying and intestinal transit speeds and increases fecal bulk. The ST has an effect on faecal bulking greater than other grains such as oats or vegetables and fruits. However, phytic acid present in the bran may reduce the absorption of certain minerals (Ca, Mg, Fe and Zn), due to formation phytate-mineral co

Clinical Analysis and in Vitro Correlation of BCRP-Mediated Drug-Drug Interaction in the Gastrointestinal Tract.

Breast cancer resistance protein (BCRP) is a drug efflux transporter expressed on the epithelial cells of the small intestine and on the lateral membrane of the bile duct in the liver; and is involved in the efflux of substrate drugs into the gastrointestinal lumen and secretion into bile. Recently, the area under the plasma concentration-time curve (AUC) of rosuvastatin (ROS), a BCRP substrate drug, has been reported to be increased by BCRP inhibitors, and BCRP-mediated drug-drug interaction (DDI) has attracted attention. In this study, we performed a ROS uptake study using human colon cancer

Pumpkin polysaccharide preparation, simulated gastrointestinal digestion, and in vivo biodistribution.

We employed a single factor and response surface methodology based on Box-Behnken design (BBD) to optimize the extraction of pumpkin polysaccharides. We then simulated pumpkin polysaccharide gastrointestinal digestion in vitro and investigated their biodistribution in mice. The optimal extraction conditions, with a yield of 7.38 ± 0.21%, were as follows: a concentration of NaOH 1.20%, a ratio of material to liquid of 1:11, and an extraction time of 2.1 h, respectively, according to single factor and BBD experiments. In the gastrointestinal experiment in vitro, the molecular weight of the polys

Therapeutic Drug Monitoring of Immunosuppressants in Cardiac Transplantation: Current Strategies, Evidence Gaps, and Future Directions.

Therapeutic drug monitoring (TDM) remains central to managing immunosuppression in cardiac transplantation, especially during the dynamic early post-transplant period, when shifting hematocrit, gastrointestinal absorption, and hemodynamics drive pronounced variability. This review outlines the current TDM practices and highlights how sample collection accuracy, assay-specific analytical considerations, and clinical context shape the interpretation of immunosuppressant levels. Key physiologic, genetic, and methodological factors contributing to variability are summarized, along with the practic…

Cancer pain management with concurrent complex gastrointestinal pathology: A case for sublingual buprenorphine.

A 35-year-old male with a complex gastrointestinal history and metastatic intrahepatic cholangiocarcinoma was hospitalized with intractable cancer-associated pain, unrelieved by oral opioids at escalating doses. Transdermal options were contraindicated due to ozone bath therapy. Given the poor response to previous opioid trials and concerns regarding oral absorption, sublingual buprenorphine was initiated, leading to significant improvement in functional pain control. Despite recurrent malignant bowel obstructions with intolerance to oral intake, the patient consistently maintained tolerable p

Tenapanor: An Update on Evidence of Effectiveness and Guidance on Practical Use.

Hyperphosphatemia remains a major challenge in managing patients with kidney failure and is associated with significant morbidity and mortality. Traditional approaches such as dietary phosphate restriction, oral phosphate binders, and dialysis modifications, even if combined, often provide insufficient control of serum phosphate levels. Tenapanor, a novel therapy that targets intestinal phosphate absorption, has shown promise in reducing serum phosphate as monotherapy and in combination with binders, even though gastrointestinal side effects may occur. Here, we review the available information
