Cancer as an evolutionary process at the cell level: an epidemiological perspective.

Germ-line mutations (present in all cells) in genes that are crucial for the cell cycle cause cancer only in specific cell lines (e.g. mismatch repair genes in the colon; BRCA1-2 in breast and ovary; other cancers in Bloom syndrome, neurofibromatosis and xeroderma pigmentosum). The mutation rate of genes other than mismatch repair or p53 is the same in colon cancer and in normal cells, indicating that a 'mutator phenotype', increasing the rate of mutations in many genes, is not an essential feature of sporadic cancers; conversely, fusion genes, TEL-AML1/AML1-ETO, typical of leukemia, are 100 t

Pulsatility as a Potential Regulator of Cardiovascular Biology: Molecular, Cellular, and Hemodynamic Remodeling During Continuous-Flow Left Ventricular Assist Device Support and Following Heart Transplantation.

Pulsatile blood flow is a fundamental characteristic of cardiovascular physiology that regulates endothelial function, vascular homeostasis, microcirculatory integrity, and organ adaptation through complex mechanobiological pathways. The widespread use of continuous-flow left ventricular assist devices (CF-LVADs) has created a unique clinical model of chronic pulsatility deprivation, whereas heart transplantation restores physiological pulsatile hemodynamics. This review examines the molecular, cellular, and systemic consequences of these contrasting circulatory states. Evidence from experimen

Recognition diversity and effector mechanisms of immunoglobulin superfamily proteins in invertebrates.

Traditionally, highly specific immune responses are considered to rely on the vertebrate adaptive immune system centered on immunoglobulins, while invertebrates defend themselves solely through innate immunity. Nevertheless, accumulating studies have demonstrated that diverse immunoglobulin superfamily (IgSF) proteins play indispensable roles in the recognition and defense against pathogenic invasion in invertebrates. This review systematically summarizes the latest advances in the specific recognition mediated by IgSF proteins via diversified mechanisms, with a focus on the generation of tens

Shared quasispecies architecture in experimental and natural RNA virus populations.

RNA viruses form genetically diverse populations structured as mutant spectra, or quasispecies, whose internal organization influences their evolutionary and adaptive dynamics. While genetic diversity has been extensively characterized, the structural organization of viral populations in sequence space remains less explored. Here, we compare genotype network architectures in two RNA viruses with markedly different evolutionary contexts: bacteriophage Qβ evolving in controlled laboratory conditions and severe acute respiratory syndrome coronavirus 2 evolving within infected human hosts. Using d

One action and multiple results: Influenza A virus and SARS-CoV-2 poly-epitope-based hybrid vaccine exploration by reverse vaccinology and immunoinformatic methods.

INTRODUCTION: Influenza A virus (IAV) and severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) coinfection have been shown to intensify symptoms and, therefore, could be considered a significant threat to susceptible populations. Although vaccination is an effective and safe strategy to be used against viruses, large amounts of mutations in these two viruses remain an issue for vaccine design, as some strains of the IAV and SARS-CoV-2 are still present worldwide, regardless of the introduction of various vaccines to protect against the viruses. METHODS: We employed a bioinformatic app

Age-Related Trajectories of Autistic Traits in Children With Angelman Syndrome.

Angelman syndrome (AS) is a rare neurogenetic disorder. Previous studies indicate a high prevalence of autism spectrum disorder (ASD) with considerable variability. Little is known regarding the longitudinal trajectory of autistic traits. We aim to investigate autistic traits, the effect of age on these traits, and associated features in AS children. This (partly) longitudinal clinical record study at the ENCORE Expertise Center involved 107 AS children aged 2-18 with one (N = 107), two (N = 49), or three (N = 14) measurements. Autistic traits and sensory processing issues were assessed using

Pathways between childhood trauma, clinical symptoms, and functioning in new-onset psychosis: novel insights from a network analysis approach.

Childhood trauma (CT) has been linked to various domains of outcomes in individuals with new-onset psychosis, but the intricate relationships between different types of trauma, clinical symptoms, and functioning remain poorly understood. The aim of this study was to identify patterns of relationships between these three domains in first-episode psychosis (FEP). The study sample consisted of 277 patients from the Treatment and Early Intervention in Psychosis Program (TIPP) at Lausanne University Hospital. Symptom severity was assessed using the Positive and Negative Syndrome Scale (PANSS), func

Rituximab in primary podocytopathies: efficacy and safety in a retrospective cohort.

INTRODUCTION: Podocytopathies are an important cause of nephrotic syndrome, and immunosuppression plays a pivotal role in disease management. The efficacy of biological agents such as rituximab (RTX), however, remains unclear, especially in adults. This study hypothesized that RTX is efficient and safe in the treatment of steroid-sensitive and steroid-resistant primary podocytopathies. METHOD: A retrospective cohort study was conducted based on medical records before the first infusion of RTX (T0) and 1 to 3 months (T1) and 3 to 6 months after infusion (T2). Patients had biopsy-proven podocyt

Narrative review on decoding the crosstalk between lipid homeostasis and immune pathways in membranous nephropathy.

Membranous nephropathy (MN) is a leading cause of nephrotic syndrome in adults and is characterized by subepithelial immune deposits driven by autoantibodies, most commonly anti-PLA2R, with subsequent complement activation and C5b-9–mediated podocyte injury. Emerging evidence suggests that this immune-mediated process is closely intertwined with disturbances in lipid homeostasis. In this review, we propose an immunometabolic framework for MN that highlights the reciprocal interactions between lipid dysregulation and immune activation. Three interrelated mechanistic domains are emphasized. Firs

Epidemiology, classification and clinical burden of pancreatic cystic lesions.

Pancreatic cystic neoplasms (PCNs), once considered rare, are increasingly detected due to advances in imaging, with prevalence estimates ranging from 13 % to nearly 50 % and rising with age. They comprise a heterogeneous group, including intraductal papillary mucinous neoplasms (IPMNs), mucinous cystic neoplasms (MCNs), serous cystic neoplasms (SCNs), solid pseudopapillary neoplasms (SPNs), cystic neuroendocrine tumors, and pseudocysts. Their biological behavior differs substantially: SCNs and pseudocysts are usually benign, MCNs and IPMNs may progress to cancer, and SPNs generally require re

Quantitative and Qualitative Radiological Assessment of Sarcopenia and Cachexia in Cancer Patients: A Systematic Review.

Sarcopenia, an extremely common condition in cancer patients, is described as a progressive and generalized musculoskeletal disorder that is associated with an increased likelihood of adverse outcomes, including falls, fractures, physical disability, and mortality. By contrast, cachexia is defined as a syndrome characterized by weight loss with the concomitant loss of muscle and/or fat mass. Cancer cachexia leads to functional impairment, reduced physical performance, and decreased survival, and is often accompanied by cancer progression and reduced response to therapy. The literature states t

Developmentally programmed obesity: Is there a role for anti-inflammatory nutritional strategies?

Pregnancy represents a period of immense maternal physiological adaptation, with progressive increases in lipid storage potential and insulin resistance to support fetal/placental growth. This requires significant change in the adipose tissue. Women living with obesity/overweight are more susceptible to these changes causing complications such as gestational diabetes. This is particularly worrying as up to 60% of European women are living with overweight/obesity at the onset of pregnancy. Furthermore, less than 1% meet all nutrition guidelines. There is now evidence that these deep metabolic c

Increased affective reactivity to daily social stressors is associated with more severe psychotic symptoms in youths with 22q11.2 deletion syndrome.

BACKGROUND: Increased reactivity to minor stressors is considered a risk factor for psychosis, especially in vulnerable individuals. In the present study, we investigated affective and psychotic stress reactivity as well as its link with psychotic symptoms and psychopathology in youths with 22q11.2 deletion syndrome (22q11DS), a neurogenetic condition associated with a high risk for psychosis. METHODS: A 6-day ecological momentary assessment protocol was used to assess perceived daily-life stress as well as affective and psychotic reactivity to stress in participants with 22q11DS (n = 38, age

ACR Appropriateness Criteria® Radiologic Management of Mesenteric Ischemia: 2022 Update.

Mesenteric ischemia is a serious medical condition characterized by insufficient vascular supply to the small bowel. In the acute setting, endovascular interventions, including embolectomy, transcatheter thrombolysis, and angioplasty with or without stent placement, are recommended as initial therapeutic options. For nonocclusive mesenteric ischemia, transarterial infusion of vasodilators, such as papaverine or prostaglandin E1, is the recommended initial treatment. In the chronic setting, endovascular means of revascularization, including angioplasty and stent placement, are generally recomme

Designing an evidence-based Bayesian network for estimating the risk versus benefits of AstraZeneca COVID-19 vaccine.

Uncertainty surrounding the risk of developing and dying from Thrombosis and Thrombocytopenia Syndrome (TTS) associated with the AstraZeneca (AZ) COVID-19 vaccine may contribute to vaccine hesitancy. A model is urgently needed to combine and effectively communicate evidence on the risks versus benefits of the AZ vaccine. We developed a Bayesian network to consolidate evidence on risks and benefits of the AZ vaccine, and parameterised the model using data from a range of empirical studies, government reports, and expert advisory groups. Expert judgement was used to interpret the available evide

Can technological advancements help to alleviate COVID-19 pandemic? a review.

The COVID-19 pandemic is continuing, and the innovative and efficient contributions of the emerging modern technologies to the pandemic responses are too early and cannot be completely quantified at this moment. Digital technologies are not a final solution but are the tools that facilitate a quick and effective pandemic response. In accordance, mobile applications, robots and drones, social media platforms (such as search engines, Twitter, and Facebook), television, and associated technologies deployed in tackling the COVID-19 (SARS-CoV-2) outbreak are discussed adequately, emphasizing the cu

Long-chain fatty acid oxidation and respiratory complex I deficiencies distinguish Barth Syndrome from idiopathic pediatric cardiomyopathy.

Barth syndrome (BTHS) is an X-linked disorder that results from mutations in the TAFAZZIN gene, which encodes a phospholipid transacylase responsible for generating the mature form of cardiolipin in inner mitochondrial membranes. BTHS patients develop early onset cardiomyopathy and a derangement of intermediary metabolism consistent with mitochondrial disease, but the precise alterations in cardiac metabolism that distinguish BTHS from idiopathic forms of cardiomyopathy are unknown. We performed the first metabolic analysis of myocardial tissue from BTHS cardiomyopathy patients compared to age
