Evaluation of efficacy and safety of subcutaneous acetaminophen in geriatrics and palliative care (APAPSUBQ).

INTRODUCTION: Subcutaneous infusion (SC) or hypodermoclysis is an old perfusion technique that is often used off-label although it has been shown to be effective. Acetaminophen (paracetamol) subcutaneous injection is performed in some centers despite the lack of conclusive evidence on its effectiveness. This study aims to evaluate the efficacy of subcutaneous infusion of Acetaminophen in the treatment of pain and fever in geriatrics and in palliative care and to determine its safety profile and possible side effects. MATERIAL AND METHODS: This experimental study was conducted between 2018 and

Significance of dementia and frailty on postoperative care trajectories and burden among older adults undergoing radical cystectomy for bladder cancer.

INTRODUCTION: Bladder cancer (BC) is predominantly a disease of older adults who frequently have comorbidities, and radical cystectomy (RC), which is the gold standard treatment, carries considerable morbidity. We sought to determine the impact of baseline dementia and frailty on the care trajectory beyond the immediate postoperative period. We hypothesized that frail patients and those with dementia undergoing RC for BC will have poorer care trajectories. MATERIALS AND METHODS: We identified Medicare beneficiaries ≥66 years old who underwent RC for BC in 2017 with 12 months of pre- and post-

Analysis of nutritional status and functional recovery outcomes in stroke patients under nursing care.

BACKGROUND: The nutritional status is a very vital predictor of stroke patient recovery, functional independence, muscle strength, and rehabilitation outcomes. Nevertheless, little is known about the association between the state of nutrition and functional recovery during the usual nursing care in hospitalized stroke patients especially when carried out in retrospective studies. OBJECTIVE: To examine the association between baseline nutritional status and functional recovery in stroke patients under hospital nursing care. METHODOLOGY: A retrospective cohort study of 500 stroke patients admi

Ultrasound-guided, nursing-led, phase-based swallowing rehabilitation in elderly patients with MIBG-positive medullary thyroid carcinoma following 131I-MIBG therapy.

BACKGROUND: Medullary thyroid carcinoma (MTC) is a rare neuroendocrine malignancy originating from parafollicular C cells and often requires multimodal management, including surgery, targeted therapy, and radionuclide treatment. In elderly patients with MIBG-positive disease, 131I-MIBG therapy remains clinically valuable but is frequently associated with mucosal injury and neuromuscular dysfunction, resulting in dysphagia and malnutrition. Conventional swallowing rehabilitation often provides limited benefit in this population, underscoring the need for individualized, technology-assisted appr

The impact of comorbidity on perioperative outcomes of hip fractures in a geriatric fracture model.

BACKGROUND: Older adults who sustain hip fractures usually have multiple coexisting medical problems that may impact their treatment and outcomes. The geriatric fracture center (GFC) provides a model of care that standardizes treatment and optimizes outcomes. The purpose of this study is to determine whether GFC patients with a higher burden of comorbidity or specific comorbidities are at risk for worsened perioperative outcomes, such as increased time to surgery (TTS), postoperative complications, and longer length of hospital stay (LOS). METHOD: A total of 1077 patients aged 60 years and ol

Improving Nursing Oral Care Practice for Community-Dwelling Care-Dependent Older People.

AIM: Oral care is one of the most neglected nursing practices owing to a lack of training, standard nursing oral care guidelines and low confidence and skills. There is little evidence to inform the development of oral care training and guidelines, which ideally should be done in collaboration with oral health professionals. Further, there is a lack of research, particularly in New Zealand, with nursing staff caring for care-dependent older people, including those ageing in their own homes. This pilot study aimed to determine the impact of oral care training on nursing staff knowledge, attitud

Exploring influences on community-based case managers' advance care planning practices: facilitators or barriers?

Community-based geriatric case managers work with an increasingly frail older population in need of advance care planning throughout the end of life. However, little is known regarding factors that influence their professional practices in this area. This qualitative study used four focus groups to explore case managers' (N = 27) perceptions of facilitators and barriers to advance care planning practices. Themes from the study suggest that case managers view five key influences that appear to either assist or impede practices, depending on case manager's perceptions and experiences. Themes inc

Pressure ulcers: the Minimum Data Set and the Resident Assessment Protocol.

As Americans age and live longer with chronic disabilities, more elderly citizens will reside in nursing homes. The nationally mandated Minimum Data Set (MDS) and Resident Assessment Protocol (RAP) were designed to help plan for the care of the frail elderly residing in nursing homes. One of the areas specifically targeted was pressure ulcers. This study was designed to describe the prevalence, incidence, and current management practices relating to pressure ulcers in nursing homes based on MDS data. In addition, a cross-sectional investigation was performed to examine the MDS items found in t

The Single Question in Delirium as a suitable tool to improve delirium detection in hospitalized older patients.

PURPOSE: Delirium remains underdiagnosed in hospitalized older patients. The Single Question in Delirium (SQiD) has emerged as a promising screening tool with a high negative predictive value in previous studies, but only a few have investigated the SQiD in geriatric patients. This study aimed to investigate the added value of combining the 4AT and SQiD for detecting delirium in hospitalized older patients. Secondary objectives included determining the point prevalence of delirium and healthcare professionals' awareness of delirium. METHODS: On World Delirium Awareness Day 2024, all patients
