As the number of elderly persons in the United States continues to increase, geriatric abuse has become the most recent manifestation of domestic violence seen in the emergency department. Recent data suggest that 1 million elderly persons are battered, neglected, or exploited each year by family members or caretakers. This maltreatment may be more difficult to identify than child or spouse abuse because of the relative isolation of the victims and their reluctance to report abuse. Because the primary care physician often is the first health professional the elderly patient will consult about a foot problem--and because many conservative therapeutic measures are available and should be attempted before resorting to surgery--the primary care office can be an appropriate setting for foot care for the elderly. A clinical survey on foot deformities among 166 geriatric inpatients was carried out. Direct measurements on hallux valgus, great toe movements, great toe deformities, lesser toe deformities, arch of the foot, callosities, ankle movements, inversion, and eversion were done by using the goniometer. Plasma concentrations of cefotaxime and desacetyl cefotaxime were determined by HPLC in geriatric patients with multiple diseases. Comparison with a younger control group of healthy volunteers showed a prolongation of half-life of CTX and dCTX in the older patients. A significant correlation between pharmacokinetic parameters of dCTX and other clinical and chemical parameters was found. In 17 gynaecological departments of West-Berlin hospitals, 2940 laparotomies were done between 1960 and 1969 in women of at least 60 years of age, of which 331 (10,9%) were exploratory. Of these the postoperative mortality was 39,2% (130 cases). The following is a report on 189 cases involving surgical treatment of uterine myoma in women who were at least 60 years old. The cases were collected from 18 gynecological departments in West-Berlin over a 10-year period (1960-1969). 66 of these patients (38,1%) have never been pregnant, 17 (9,8%) had only abortions, while 90 (52,1%) had given birth to one or more children. An essential instrument for evaluating older adults' nutritional status is the geriatric nutritional risk index (GNRI).
