Molecular and environmental drivers of antimicrobial resistance: global epidemiology, resistome dynamics, and one health strategies.

Antimicrobial resistance (AMR) has evolved into a critical global health security challenge, threatening the effectiveness of modern medicine and increasing morbidity and mortality worldwide. This review integrates current evidence on the molecular and environmental drivers of AMR, alongside global epidemiological patterns, resistome dynamics, and one health-based intervention strategy. Recent surveillance data indicate that AMR contributes to approximately 4.7 million deaths annually, with the highest burden concentrated in low- and middle-income countries, where resistance rates in key patho…

Global Perspectives on China's Iodine Dietary Reference Intakes: Revisions, Public Health Implications, and Future Strategies.

Iodine is an essential trace element for the human body and a fundamental precursor for the synthesis of thyroid hormones, playing a critical role in maintaining thyroid function. Since iodine cannot be synthesized by the body, it must be obtained from external sources. The development of dietary reference intakes for iodine has evolved over time, with various countries establishing guidelines based on local dietary surveys, iodine status assessments, and health outcomes. The dietary reference intakes of iodine vary widely across different regions due to differences in dietary habits, food for

Microplastics and Nanoplastics in Health Concerning Cellular Toxicity Mechanisms, Exposure Pathways, and Global Mitigation Strategies.

Microplastics (MPs) and nanoplastics (NPs) have emerged as ubiquitous environmental contaminants that pose significant threats to human health, with multiple exposure pathways (e.g., ingestion and inhalation) contributing to systemic exposure. Although growing evidence highlights their biological effects, the underlying mechanisms by which these particles induce cellular dysfunction remain incompletely understood. This review synthesizes current knowledge on the MPs/NPs-induced cellular toxicity mechanisms, including investigations into cellular uptake pathways, disruption of molecular signali

JAHA at Scientific Sessions 2024: Climate Change-Related Cardiovascular Health Effects in the Global South.

Climate change poses an escalating threat to cardiovascular and cerebrovascular health in the Global South, where vulnerability is amplified by rapid urbanization, poverty, and weak infrastructure. Air pollution (driven by fossil fuel use, industrial growth, and poor regulation) remains a major contributor to cardiovascular disease and respiratory illness, with regions such as South Asia and Sub-Saharan Africa experiencing the highest burdens. Extreme heat, floods, and natural disasters further compound cardiovascular risks through direct physiological stress and disruption of health care syst

Constructing Success: The World Bank, Onchocerciasis Control, and What Lies Beneath Triumphalist Global Health Narratives.

Using a historical case study of the World Bank and World Health Organization (WHO)'s Onchocerciasis Control Programme (OCP, 1972-2002), I explore how success is conceptualised in global health and why it matters for policy and priority-setting. First, I summarise the 'dominant' OCP success narrative that has emerged since the 1980s, which is based on public health, socio-economic and humanitarian justifications for the programme's effectiveness. Next, I analyse how socio-economic metrics linking the programme's disease control to increased labour productivity and agricultural land availabilit…

Global epidemiology, viral evolution, and public health responses: a systematic review on Mpox (1958-2024).

BACKGROUND: Monkeypox (Mpox), a zoonotic viral disease caused by the Mpox virus (MPOXV), was first identified in 1958 and remained largely confined to Central and West Africa for decades. While it usually exhibited limited international transmission, recent outbreaks, including in the USA in 2003 and globally in 2024, highlight significant epidemiological shifts. We aimed to systematically evaluate the evolution of Mpox from 1958 to 2024, focussing on its epidemiology, viral evolution, and public health responses. METHODS: We conducted a systematic review using data from global health reports…

Prophylactic Antibiotics After Cleft Lip and Palate Reconstruction: A Review From a Global Health Perspective.

Orofacial clefts are common congenital deformities. Global initiatives have increased access to cleft care and reconstruction surgeries for cleft lip with or without cleft palate (CL/P), but there is no consensus on the use of postoperative prophylactic antibiotics. We conducted a narrative review using PubMed on the use of postoperative prophylactic antibiotics in CL/P surgery. A search of PubMed identified 30 potentially relevant articles, of which 15 were reviewed. There was no consensus among surgeons on prescribing patterns, but there was limited evidence that postoperative antibiotics re

The Global Health Threat of Monkeypox Virus: Understanding Its Biology, Transmission, and Potential Therapeutic Interventions.

Monkeypox virus (MPXV), a member of the Orthopoxvirus genus, shares its genus with Variola virus (VARV), the causative agent of smallpox, and Vaccinia virus (VACV), used for smallpox vaccination. While smallpox has been eradicated, MPXV and related poxviruses continue to pose a global health threat. Monkeypox (Mpox), similar in clinical presentation to smallpox but milder, is endemic in Central and West Africa. Sporadic outbreaks emphasize the potential for wider dissemination. Understanding their biology, transmission, immune evasion, and clinical features informs disease control strategies. …

Air pollution control efficacy and health impacts: A global observational study from 2000 to 2016.

Particulate matter with aerodynamic diameter ≤2.5 μm (PM2.5) concentrations vary between countries with similar carbon dioxide (CO2) emissions, which can be partially explained by differences in air pollution control efficacy. However, no indicator of air pollution control efficacy has yet been developed. We aimed to develop such an indicator, and to evaluate its global and temporal distribution and its association with country-level health metrics. A novel indicator, ambient population-weighted average PM2.5 concentration per unit per capita CO2 emission (PM2.5/CO2), was developed to assess c

Multiplexed technologies for sexually transmitted infections: global evidence on patient-centered and clinical health outcomes.

INTRODUCTION: Conventional care packages around screening for sexually transmitted infections (STIs) entail multiple clinic visits and precipitate losses to follow-up. To prevent these losses, multiplexed technologies for STIs (immunochromatographic tests/devices/assays and molecular assays that can screen multiple pathogens or multiple strains of one STI) can yield same-day results in a single visit. Research evidence of patient-centred (preference, satisfaction) and clinical health outcomes (feasibility, case positivity, uptake, impact) has not been synthesised. We conducted a systematic rev

Diabetic retinopathy: global prevalence, major risk factors, screening practices and public health challenges: a review.

Diabetes retinopathy (DR) is a leading cause of vision loss in middle-aged and elderly people globally. Early detection and prompt treatment allow prevention of diabetes-related visual impairment. Patients with diabetes require regular follow-up with primary care physicians to optimize their glycaemic, blood pressure and lipid control to prevent development and progression of DR and other diabetes-related complications. Other risk factors of DR include higher body mass index, puberty and pregnancy, and cataract surgery. There are weaker associations with some genetic and inflammatory markers.
