Chronic rejection of human kidney allografts.

Human chronic rejection is an uncommon feature of chronic allograft nephropathy, the major cause of late graft loss in kidney transplantation. Chronic rejection is a complex process, mediated, at least in part, by immunological mechanisms. T cells with donor, tissue or pathogen specificities that react with donor cells damage the graft tissue in synergy with antibodies. The cellular and humoral components of the immune system may be preformed or appear at both early and late time points following kidney transplantation. Both slowly contribute to the development of vasculopathy, glomerulopathy,

Induction Regimens and Kidney Re-Transplant Outcomes after BK Nephropathy-Related Graft Loss: A U.S. Cohort Study.

This study investigated the association between induction type and long-term outcomes in kidney retransplant recipients who experienced graft loss due to BK virus-associated nephropathy (BKVAN). Using Scientific Registry of Transplant Recipients data (2003-2021), we identified 277 adult kidney-alone retransplant recipients with BKVAN-related graft loss and categorized them by induction regimen: depletional (n = 217) and nondepletional (n = 60). The groups were similar overall, except the nondepletional cohort had lower panel-reactive antibodies, a shorter time between transplants, and more liv

Pretransplant dialysis status and outcome of renal transplantation in North American children: a NAPRTCS Study. North American Pediatric Renal Transplant Cooperative Study.

BACKGROUND: There are no large studies of the effect of pretransplant dialysis status on the outcome of renal transplantation (Tx) in children. This study evaluated the North American Pediatric Renal Transplant Cooperative Study (NAPRTCS) registry data for the outcome of Tx in pediatric patients who either (1) received their transplants preemptively or (2) were maintained on dialysis before receiving their transplants. METHODS: We compared graft survival and patient survival rates, incidence of acute tubular necrosis (ATN), acute rejection episodes, and causes of graft failure in peritoneal d

Predictive factors for delayed graft function (DGF) and its impact on renal graft survival in children: a report of the North American Pediatric Renal Transplant Cooperative Study (NAPRTCS).

We define delayed graft function (DGF) as the need for dialysis during the first post-transplant week. We analyzed 5272 transplants, of which 2486 were of living donor (LD) and 2786 were of cadaver donor (CD) origin. Twelve per cent (620/5272) of all patients developed DGF. Donor specific rates were 5.6% for LD and 19.1% for CD patients. Factors predictive of DGF in CD patients were: African-American race (25%), prolonged cold ischemia (24%), absence of T-cell induction antibody therapy and absence of HLA-DR matching. The relative risk (RR) for graft failure due to DGF was 6.02 (p < 0.001) in

Risk factors for renal allograft loss in patients with systemic lupus erythematosus.

Controversy exists regarding the risk factors for renal allograft loss in patients with systemic lupus erythematosus (SLE). This study is a retrospective evaluation of each of these independent risk factors in 80 renal transplants for ESRD secondary to SLE done at our institution between 1971 and 1994. Our entire non-diabetic cohort of 1,966 renal transplants is used as a comparison group. Our results showed equivalent graft survival rates between lupus patients and the cohort at 1, 5 and 10 years (P = 0.56). However, an analysis of cyclosporine-era cadaver grafts revealed that the lupus group

Kidney transplantation in children.

Over the past decade, the success rate of pediatric kidney transplantation has dramatically increased, based on improvements in histocompatibility testing, immunosuppressive management, and diagnosis and treatment of rejection. The 1-year graft survival rate now approaches 90%. The optimal age for recipients as well as for donor organs is greater than six years. Recurrence of primary disease in the graft accounts for approximately 5% graft loss in children. Sequential immunosuppressive protocols have been particularly successful in children, and maintenance immunosuppression routinely includes

Immunological aspects of corneal transplant.

Corneal transplant is the most common solid tissue transplant in humans. Advances in microsurgical techniques, eye banking and the use of corticosteroids have improved the success of corneal transplants. Over 65,000 corneal transplants are being performed worldwide annually. Most of these transplants are performed in developed countries. Cornea is considered an immune privileged site. Despite this, immune mediated graft rejection is the most single cause of cornea graft failure and is one of the major postoperative complications. Incidences from as low as 2% to as high as 50% have been reporte…

Effects of PUVA therapy on kidney allografts: results of a randomized prospective double-blind study.

After successful experimental organ transplant studies on the efficacy of PUVA therapy combining donor pretreatment with the photosensitizer 8-methoxypsoralen (P) and the ex vivo irradiation of organs with long-wave ultraviolet light (UVA) prior to transplantation, we started in 1989 the first randomized, prospective, double-blind study to clarify the efficacy of PUVA therapy in human kidney transplantation. This study included 50 kidney donors, 25 of whom were PUVA-treated. A total of 75 kidneys were transplanted in Berlin, Halle and Rostock. The complete data of these 75 recipients were avai

Histopathology of cell mediated immune reaction in mouse colon--allograft rejection.

Grafts of mouse fetal colon, implanted beneath the renal capsule of adult hosts, have been used to study the growth and development of colonic isografts and the rejection of colonic allografts. Isografts grew normally and maintained a structure similar to normal colon. Grafts between strains with H2 histocompatibility differences were rejected by 13 days after transplantation. Early progressive infiltration of the grafts by lymphoid cells was followed by increasing damage to, and subsequent loss of, the epithelial cell layer and destruction of the underlying muscle, changes which parallel thos

Unveiling the Molecular Symphony: Exploring the Influence of miR-142-5p and miR-192-1 on Renal Transplant Rejection and Fibrosis.

OBJECTIVES: In the intricate symphony of molecular orchestration within the human body, microRNAs are master conductors, deftly guiding essential biological processes like immune response, inflammation, and tissue repair. Among these virtuosos, the microRNAs miR -142 -5p and miR -192 -1 emerge as stars, with melodies resonating profoundly in renal transplant pathology. We aimed to unravel the complex role of miR -142 -5p and miR -192 -1 in the fate of renal transplants. MATERIALS AND METHODS: We evaluated plasma levels of miR -142 -5p and miR -192 -1 using quantitative real -time polymerase c

Topical FK-506 prevents experimental corneal allograft rejection.

We evaluated the efficacy of topical cyclodextrin-encapsulated FK-506 in the prevention of experimental corneal allograft rejection. Two weeks after inducing corneal inflammation and neovascularization with 8-0 silk sutures, 23 albino rabbits received a unilateral 8-mm diameter central penetrating corneal allograft from pigmented donors. Rabbits were randomly assigned to no treatment (eight eyes), topical cyclodextrin four times daily for 28 days (seven eyes), or topical FK-506 0.3 mg/ml in a cyclodextrin suspension (eight eyes) four times daily for 28 days. Grafts were examined daily for degr…

Induction with interleukin-2 antagonist for transplantation of kidneys from older deceased donors: an observational study.

BACKGROUND: The most important limiting factor in kidney transplantation is the scarcity of donor organs. Consequently, there is an increased use worldwide of kidneys from older deceased donors. High donor age is a known risk factor for acute cellular rejection and premature graft failure, and the optimal immunosuppressive regimen in these circumstances remains to be established. METHODS: We investigated whether induction treatment with an interleukin 2 (IL-2) receptor antagonist improves graft survival and reduces rejection episodes in recipients of kidneys from deceased donors aged ≥ 60 yea

Radiotherapy for rejection of renal transplant allografts refractory to medical immunosuppression.

OBJECTIVE: To evaluate the outcome and prognostic factors of patients who underwent local graft irradiation for acute renal allograft rejection refractory to modern immunosuppressive medications. METHODS: From 1996 to 2005, 33 patients received local graft irradiation (LGI), with 3 patients receiving 2 courses of radiation. Graft rejection was diagnosed when a rise in creatinine prompted a renal biopsy that demonstrated acute allograft rejection. Upon failure of medical immunosuppresion to resolve rejection, patients were then referred by the organ transplant team for LGI. The median dose was…

Association of human leukocyte antigen mismatches between donor-recipient and donor-donor in pancreas after kidney transplant recipients.

The effects of HLA mismatching on pancreas outcomes among pancreas after kidney (PAK) recipients are undefined. Outcomes might potentially differ depending on whether there is a mismatch between pancreas donor and recipient (PD-R) or pancreas donor and kidney donor(PD-KD). All primary PAK at our centre were included in this study. Patients were divided into two groups based on the degree of HLA mismatching: low (L-MM) as 0-4 and high (H-MM) as 5-6. We analysed all (N = 73) PAK for PD-R mismatch and the subset of PAK for PD-KD mismatch (N = 71). Comparing PD-R L-MM (n = 39) and H-MM (n = 34) PA
