OBJECTIVE: Acute adrenocortical insufficiency is an unusual cause of isolated shock. The purpose of this study is to describe the cardiovascular changes in 6 patients with acute adrenal insufficiency presenting with hemodynamic instability.
DESIGN: Retrospective and prospective study.
SETTING: Medical intensive care unit in a university hospital.
PATIENTS: 6 patients studied by right cardiac catheterization.

We determined the hemodynamic effects of brief hemostasis in isolated blood-perfused rat lungs held at constant inflation pressure. Three periods of hemostasis, each lasting 20 min, alternated with equal durations of perfusion. Pulmonary vascular resistance (PVR), calculated as the ratio of the pulmonary arterial- (Ppa) left atrial (Pla) pressure difference to flow, was determined at baseline and after the third hemostasis period. Felodipine is a new calcium antagonist that was developed aiming particularly at reduction in vascular resistance-without any decrease in heart pump function. Hemodynamic changes in the vascular bed of the affected testis were studied in 45 sterile and 6 fertile patients with varicocele. The examination method used was modified abdominal angiography. Aortic impedance, wave reflection magnitude, and compliance were measured in patients with Marfan's syndrome during diagnostic cardiac catheterization. Meningococcal shock still is associated with high mortality. Along with antibiotics and control of metabolic derangements, standard therapy involves monitoring and maintaining numerous cardiorespiratory variables. From 1980 to 1985, 39 children with meningococcal shock were treated in our pediatric ICU. We obtained full hemodynamic monitoring in 18 (12 survivors, six nonsurvivors). Three patients with eclampsia and four with severe preeclampsia underwent pulmonary artery catheterization before either labor or significant volume infusion.
