BACKGROUND: Palliative care is about more than providing high-quality medical care; it is about maximizing quality of life while dying. The … based drawing techniques, semi-structured interviews with patients (n = 4), caregivers (n = 3), and hospital administrators and food services decision-makers (n = 4), and self-reflexive journal entries by researchers. Data from all sources were triangulated, and Reflexive Thematic Analysis was used to inductively identify key themes.
RESULTS: The three overarching theme …

BACKGROUND: Achieving a successful transition from pediatric to adult care for young adults with special needs, especially rare genetic diseases such as osteogenesis imperfecta (OI), is a prominent issue in healthcare research. This transition represents a challenge for patients with OI, their families, clinicians and healthcare managers because of the complex nature of the process and the lack of evaluation of existing transition programs. We evaluated a transition program for adolescents and young adults with OI from a pediatric orthopedic hospital to adult care.
METHODS: Data were collecte

IMPORTANCE: The presence of management consultants in the US health care industry has increased dramatically in recent decades and is now higher than in most other sectors of the US economy. Hospitals hire management consultants to provide external expertise and advise on strategic planning, organizational change, cost cutting, and revenue-enhancement activities. Despite the prominence and influence of these firms, there is no empirical evidence in the US documenting either how much is spent on these services or whether that spending leads to measurable improvements.

PURPOSE: Patient-centred care and patient involvement are increasingly central concepts in health policy in the UK and elsewhere. However, there is little consensus regarding their definition or how to achieve "patient-centred" care in everyday practice or how to involve patients in service redesign initiatives. The purpose of this paper is to explore these issues from the perspective of key stakeholders within National Health Service (NHS) hospitals in the UK.
DESIGN/METHODOLOGY/APPROACH: Semi-structured interviews, covering a range of topics related to service redesign, were conducted with

A feasibility study has shown that it is possible to apply the principles of stroke unit management to patients and their families on general wards. Ninety-one consecutive stroke patients were managed by a multidisciplinary stroke therapy team which was co-ordinated by a medical registrar, met weekly, and developed a family support service. This was achieved within the pre-existing hospital timetable and staffing level, without providing a specialist 'stroke ward'. (Only 3-4 medical hours a week were needed.) The advantages of the service were: speedy referral to therapists, development of a t

BACKGROUND: Recent studies have shown intercostal cryoablation(IC) during the Nuss procedure decreases hospital length of stay(LOS) and opioid administration. However, few studies have also evaluated the risk of postoperative complications related to IC.
METHODS: We performed a single center retrospective analysis of all patients who underwent Nuss procedure by one surgeon from 2/2016 to 2/2020, comparing intraoperative IC to other pain management modalities(non-IC). Primary outcomes were postoperative complications, hospital LOS, and opioid administration. Multivariate analysis was performed

Background Intravenous admixture preparation errors (IAPEs) may lead to patient harm. Insight into the prevalence as well as the determinants associated with these IAPEs is needed to elicit preventive measures. Aim The primary aim of this study was to assess the prevalence of IAPEs. Secondary aims were to identify the type, severity, and determinants of IAPEs. Method A prospective observational study was performed in a Dutch university hospital. IAPE data were collected by disguised observation. The primary outcome was the proportion of admixtures with one or more IAPEs. Descriptive statistics

On the hospital wards in Finland, nurses generally reconstitute intravenous medicines, such as antibiotics, analgesics, and antiemetics prescribed by doctors. Medicine reconstitution is prone to many errors. Therefore, it is important to identify incorrect practices in the reconstitution of medicine to improve patient safety in hospitals. The aim of this study was to audit the compounding and reconstituting of intravenous medicines on hospital wards in a secondary-care hospital in Finland by using an assessment tool and microbiological testing for identifying issues posing patient safety risks

Therapeutic drug monitoring (TDM) is a developing field in hospital pharmacy that contributes to good clinical response. Optimal concentrations of immunosuppressants, anti-epileptic drugs, and glycopeptide antibiotics are narrow, and these drugs are widely monitored by fluorescence polarization immunoassay (FPIA) in clinical settings. Recently, a chemiluminescence immunoassay (CLIA) has also become available. We compared these two assays in monitoring serum concentrations of cyclosporin A (CsA), carbamazepine (CBZ), phenobarbital, phenytoin, and valproic acid. FPIA estimates of CsA and CBZ con

BACKGROUND: Fundamental to complex systems are interconnected processes involved in providing high-quality patient care. A case study and a root cause analysis (RCA) illustrate a patient safety effort with unintended consequences.
CASE REPORT: A 38-year-old woman presented to the hospital for odynophagia and vomiting. The patient developed Mobitz type 2, second-degree heart block temporally associated with the administration of intravenous ondansetron. RESPONSE TO THE EVENT: An Ishikawa, or fishbone, diagram conducted to enumerate potential contributing factors indicated that a key factor app

BACKGROUND: Generative AI (GenAI) is increasingly entering health care through documentation support, communication tools, educational content generation, and other knowledge-intensive functions. However, organizational adoption remains uneven, and concerns related to privacy, security, output reliability, governance, workflow fit, and infrastructure continue to limit broader implementation. Although the literature increasingly discusses the skills health care workers may need in the GenAI era, less is known about how hospital managers view the organizational readiness required before such ski

AIM: To describe medication safety, transformational leadership and their relationship as evaluated by Registered Nurses.
BACKGROUND: Medication safety is one of the aspects of patient safety and an important part of nurses' daily practices. Nurse managers can ensure medication safety by adopting a transformational leadership style. However, little evidence exists regarding how transformational leadership relates to medication safety.
METHODS: Data were collected from Registered Nurses (n = 161) in three Finnish hospitals in May 2016 using an electronic questionnaire incorporating the Transf

BACKGROUND: The public health sector in South Africa has been facing severe financial cutbacks and financial constraints in recent times. The nurse manager (NM) is faced with the task of managing and reducing expenditure in the nursing sector without compromising the quality care. This requires skills and understanding of financial management.
AIM: This study aimed to explore the financial management roles of NMs and to identify financial management development needs necessary for NMs' practice.
SETTING: The study was conducted in KwaZulu-Natal. A total of eight hospitals from the five healt

Correctional nurses are trained to care for prisoners in a controlled security environment; however, when a convict is transferred to a noncorrectional health care facility, the nurses there are often unfamiliar with custody requirements or how to safely care for these patients. The care of prisoners outside of prison has not been adequately investigated, and a gap exists between research and nursing education and practice. Nurses rarely have to consider how providing care for a prisoner in custody affects their practice, the potential dissonance between routine nursing care and the requiremen
