[The naked king in the pandemic: about the production and communication of scientific knowledge at the time of SARS-CoV-2.].

The SARS-CoV-2 pandemic has lifted the veil about how medical knowledge is produced and disseminated. Action Bias, together with economic, academic and media-related interests, has concurred to generate and spread low-value and even unreliable information about some hypothetical therapeutic interventions for CoViD-19. Not only this "infodemic" has weakened people's ability to make informed health choices, but it also has influenced the process of new evidence generation through the violation of the equipoise principle. The CoViD-19 infodemic has further highlighted the need for reliable health

Salmonella Bloodstream Infections.

Salmonella is a major foodborne pathogen of both animals and humans. This bacterium is responsible for considerable morbidity and mortality world-wide. Different serovars of this genus cause diseases ranging from self-limiting gastroenteritis to a potentially fatal systemic disease known as enteric fever. Gastrointestinal infections with Salmonella are usually self-limiting and rarely require medical intervention. Bloodstream infections, on the other hand, are often fatal even with hospitalization. This review describes the routes and underlying mechanisms of the extraintestinal dissemination

Cognition and technology in health education research.

There is an explicit relationship between knowledge generation, transmission (dissemination and diffusion), knowledge acquisition, and utilization. In order to disseminate and diffuse information for health promotion and disease prevention, there is an assumption that we just need to change the behaviours of people. This paper challenges this notion and suggests that the thinking behind the behaviours (cognition) also needs to be addressed for successful acquisition and utilization of information. Studies focusing on cognition or thought processes and how sociocultural factors influence or int

Disseminated cystoisosporiasis in an HIV-infected patient: long-term clinical, parasitological and molecular follow-up.

Disseminated Cystoisospora belli infection is an uncommon but potentially fatal manifestation in patients with AIDS, and information on extended follow-up is limited. A 39-year-old man with HIV/AIDS and disseminated cystoisosporiasis was monitored clinically, parasitologically and molecularly during hospitalization and for three subsequent years. Oocyst excretion ended by day six, while C. belli DNA remained detectable in blood until day 30. All later samples were negative and no relapse occurred. Serial molecular testing in blood proved useful for monitoring the course of disseminated cystois

Unlocking the Role of Metabolic Pathways in Brain Metastatic Disease.

The dissemination of malignant cells to the brain is a late-stage complication of cancer, leading to significant morbidity and mortality. Brain metastases (BM) affect 20-30% of cancer patients, primarily originating from lung cancer, breast cancer, and melanoma. Despite advances in molecular-targeted therapies, brain metastatic disease remains incurable, with a poor median survival of ≤12 months if left untreated. The lack of therapeutic efficacy is mainly attributed to the presence of the blood-brain barrier (BBB) and genetic differences between BM and their primary tumors. Previously publish

From insight to impact: A co-produced public engagement and dissemination strategy from the UK UNBIASED study.

BACKGROUND: Persistent inequities in access to diabetes technologies remain evident among children and young people (CYP) with type 1 diabetes (T1D), particularly among those from ethnic minority groups and socioeconomically deprived areas. The UK UNBIASED study identified barriers including limited awareness, language and communication challenges, inconsistent service delivery and reduced confidence in healthcare interactions. METHODS: To respond to, and support dissemination and implementation of these findings, a series of stakeholder consultation and public engagement activities was under

Dissemination patterns in non-gastric MALT lymphoma.

BACKGROUND: In contrast to gastric extranodal marginal zone mucosa associated lymphoid tissue (MALT) lymphomas, there is little consensus on the value and clinical consequences of extensive staging at diagnosis and during follow-up in non-gastric MALT lymphomas. DESIGN AND METHODS: Complete clinical information at presentation and during follow-up was collected for 72 patients with non-gastric MALT lymphoma treated at the Netherlands Cancer Institute between 1977 and 2005. Dissemination patterns at presentation were studied for nine primary dominant organ groups in our series of 72 patients a

Attitudes toward free speech, content moderation, and punishment of misinformation spreaders during infectious disease outbreaks.

INTRODUCTION: During infectious disease outbreaks, the spread of false information online may undermine public health interventions. At the same time, free speech is a core value in democratic societies, and in the early stages of a novel pathogen there is a legitimate need for open discussion among scientists, policymakers, and the public to negotiate responses under conditions of uncertainty. Any restrictions on speech on social media should therefore reflect public attitudes and normative trade-offs. Currently, no standardized instrument exists to measure public attitudes toward free speech

Epidemiological profile and north-south gradient driving baseline systemic involvement of primary Sjögren's syndrome.

OBJECTIVE: To characterize the systemic phenotype of primary Sjögren's syndrome at diagnosis by analysing the EULAR-SS disease activity index (ESSDAI) scores. METHODS: The Sjögren Big Data Consortium is an international, multicentre registry based on worldwide data-sharing cooperative merging of pre-existing databases from leading centres in clinical research in Sjögren's syndrome from the five continents. RESULTS: The cohort included 10 007 patients (9352 female, mean 53 years) with recorded ESSDAI scores available. At diagnosis, the mean total ESSDAI score was 6.1; 81.8% of patients had sy

To share or not to share data: how valid are trials evaluating first-line ovulation induction for polycystic ovary syndrome?

BACKGROUND: In our recent individual participant data (IPD) meta-analysis evaluating the effectiveness of first-line ovulation induction for polycystic ovary syndrome (PCOS), IPD were only available from 20 studies of 53 randomized controlled trials (RCTs). We noticed that the summary effect sizes of meta-analyses of RCTs without IPD sharing were different from those of RCTs with IPD sharing. Granting access to IPD for secondary analysis has implications for promoting fair and transparent conduct of RCTs. It is, however, still common for authors to choose to withhold IPD, limiting the impact o

A mixed methods study of clinical information availability in obstetric triage and prenatal offices.

OBJECTIVE: To determine the effect of availability of clinical information from an integrated electronic health record system on pregnancy outcomes at the point of care. MATERIALS AND METHODS: We used provider interviews and surveys to evaluate the availability of pregnancy-related clinical information in ambulatory practices and the hospital, and applied multiple regression to determine whether greater clinical information availability is associated with improvements in pregnancy outcomes and changes in care processes. Our regression models are risk adjusted and include physician fixed effec

Interfacing a biosurveillance portal and an international network of institutional analysts to detect biological threats.

The Early Alerting and Reporting (EAR) project, launched in 2008, is aimed at improving global early alerting and risk assessment and evaluating the feasibility and opportunity of integrating the analysis of biological, chemical, radionuclear (CBRN), and pandemic influenza threats. At a time when no international collaborations existed in the field of event-based surveillance, EAR's innovative approach involved both epidemic intelligence experts and internet-based biosurveillance system providers in the framework of an international collaboration called the Global Health Security Initiative, w

"First-hit" heart attack risk calculators on the world wide web: implications for laypersons and healthcare practitioners.

BACKGROUND: Heart attack risk calculators are readily accessible on the world wide web, offering potentially powerful means of health education and risk awareness. Laypersons may be unaware of differences in applicability, risk calculation algorithms and output formats among such calculators. This study assesses the impact of basic web searching terms on type of calculator accessed and on the resulting risk score. DESIGN: Observational study. METHODS: Seventy-two notional individual risk factor profiles were constructed, based on six combinations of presence or absence of smoking habit, hype

Translation in tobacco and drug abuse prevention research.

The purpose of this article is to describe the translation of research on tobacco and drug abuse prevention from basic science to program development to large-scale program dissemination, and from animal to human studies. Where relevant, continuity of translation is discussed by referring to two variables that have been studied for their potential relationship to drug use risk in both animals and humans: sensation and novelty seeking and low impulse control. Review of the research indicates relatively slow translation until the early 1990s. The authors recommend several mechanisms to promote m
